Cancer News and Information Center Health Centers Cancer More News Saving the voice in larynx cancer Saving the voice in larynx cancer Reported by Susan Aldridge, PhD, medical journalist Results of a clinical trial suggest that simultaneous treatment with chemotherapy and radiation preserves the voice of patients with advanced larynx cancer without affecting survival.
Researchers at the Johns Hopkins Kimmel Cancer Center say that experience with combined treatment has reduced the need for complete removal of the larynx from 100 percent to about 15 percent. Removing the larynx leaves patients unable to speak with their natural voice and reliant upon speaking aids, even though it may cure the cancer. Other previously-studied treatment options included radiation therapy alone or several cycles of chemotherapy followed by radiation. Previous studies showed that the survival rate of patients treated with chemotherapy followed by radiation was just as good as for those receiving surgery. This new study of 547 patients shows that giving chemotherapy and radiation together instead of sequentially is more effective in preserving the voice box.
Eighty eight per cent of patients receiving chemotherapy and radiation together still had their voice box after two years compared to 75 percent receiving chemotherapy followed by radiation and 70 percent receiving radiation alone. For each of these three treatment options, overall survival was similar at about 75 percent after two years.

  • paranasal sinuses (small hollow spaces around the nose lined with cells that secrete mucus);
  • nasal cavity (airway just behind the nose);
  • larynx ("Adam's apple" or voice box); and
  • salivary glands (parotid, submanidular, sublingual glands that secrete saliva).
Many authorities also include skin tumors of the face and neck and tumors of the cervical lymph nodes. Excluding superficial skin cancers, but including cancer of the larynx and thyroid, it is conservatively estimated that about 60,000 people are diagnosed with head and neck cancer annually - about 5% of all cancers diagnosed in the United States. There are more than half a million survivors of oral, head, and neck cancer living in the United States today.

  • The larynx, or voice box, is a complex anatomic structure located below the oropharynx and above the trachea. Most of it lies just behind the Adam's apple. The hypopharynx lies on each side of the larynx and behind it
  • It is divided into three distinct and important sub-sites:
Glottis
  • consists of the two "true" vocal cords, these narrow fibrous bands vibrate to produce sounds which are turned into speech by the muscles of the tongue, mouth and lips
Supraglottic larynx
  • a larger area above the vocal cords which extends up to the base of the tongue. Structures within the supraglottic larynx include the false cords, ventricles, aryepiglottic folds, the arytenoids and the epiglottis (a flap that helps prevent food from going into the lungs when swallowing). The structures at the top of the supraglottis separate the air and food passages
Subglottis
  • the area between the vocal cords and the top of the windpipe or trachea Nearly all cancers arise from the lining of the larynx and are squamous cell carcinomas, but they differ widely in their behavior depending on their site of origin
