What is Laryngitis?
Laryngitis is an inflammation of your voice box (larynx) due to overuse, irritation or respiratory infection. The larynx is a framework of cartilage, muscle and mucous membrane that forms the entrance of your windpipe (trachea). Inside the larynx are your vocal cords - two folds of mucous membrane covering muscle and cartilage.
Normally, your vocal cords open and close smoothly, forming sounds through their movement and vibration. But when air escapes between the cords when it is not supposed to, your voice sounds breathy, raspy and hoarse. Your voice may sound higher or lower than normal. The sound may be due to a weak or slightly paralyzed vocal cord on one side; polyps, which are small, soft growths; or nodules, which are harder growths.

LARYNGITIS
LARYNGITIS , an inflammation of the mucus of the larynx. There are three chief varieties: acute, c/ironic, and oedematous. The larynx is also liable to attacks of inflammation. in connection with tubercle or syphilis. Oedematous Laryngitis is a very fatal condition, which may occur, though rarely, as a sequence of acute laryngitis. It is far more commonly seen in syphilitic and tubercular conditions of the larynx, in kidney disease, in certain fevers, and in cases of cellulitis of the neck. The larynx is also one of the sites of Angeioneurotic oedema. In this form of laryngitis there are all the symptoms of acute laryngitis, but on a very much exaggerated scale. The dyspnoea, accompanied by marked stridor, may arise and reach a dangerous condition within the space of an hour, and demand the most prompt treatment. On examination the mucous membrane round the epiglottis is seen to be enormously swollen. The treatment is ice round the throat and internally, scarification of the swollen parts, and should that not relieve the asphyxial symptoms, tracheotomy must be performed immediately. Syphilitic Laryngitis.Invasion of the larynx in syphilis is very common. It may occur in both stages of the disease and in the inherited form. In the secondary stage the damage is superficial, and the symptoms those of a slight acute laryngitis. The injury in the tertiary stage is much more serious, the deeper structures are invaded with the formation of deep ulcers, which may when they heal form strong cicatrices, which produce a narrowing of the air-passage which may eventually require surgical interference. Occasionally a fatal oedema may arise. The treatment consists of administering constitutional remedies, local treatment being of comparatively slight importance.

What is croup?
Croup is an infection of the throat and vocal cords (or larynx). The infection is caused by a virus. When children younger than 5 years of age have the infection, it is called croup. In those older than 5, it is called laryngitis. What are the symptoms of croup?
Croup often begins like a cold, but then fever, cough and difficulty breathing develop. The infection causes the lining of the throat and larynx to become red and swollen. The child develops a hoarse voice and a cough that sounds like a bark. The air passage below the vocal cords may narrow, making it difficult for the child to move air in and out. Breathing may then become very rapid and noisy. In most cases, croup sounds worse than it actually is. However, the child may become very tired because of the extra work it takes to breathe. In severe cases, the child's breathing can be obstructed. Some children become so sick, in fact, that they have to be treated in hospital. Antibiotics are not effective because the infection is caused by a virus. What can parents do?

Practical advice...
avoid alcohol and smoking
Aromatherapy...
Essential oils of sandalwood and lavender are soothing. To use in a gargle: add one drop of each to 1/2 glass of warm water; add 2 drops of essential oil of lemon or peperment if you have an infection. Herbalism.... decoction of echinacea is effective against viral and bacterial attacks of laryngitis. Put two teaspoond of the powdered root in one cup of water, bring to a boil, and simmer for 15 minutes; drink 3 times a day. Both rosemary and sage are aromatic, anticeptic, and rich in potent healing oils. Ladys mantle can also help reduce inflammation. Try making a gargle using 15g dried, or 45g fresh purple sage leaves, 10g dried, or 20g fresh rosemary leaves, and 5 g dried ladies mantle leaves. Add freshly boiled water to the herbs, steep for ten minutes, store in a cool place. Gargle with a wineglass full 2-3 times a day. Swallow the mixture after garling. Take 2x200 mg capsules of garlic 3 times a day to support the imuune system.

Chronic Laryngitis.
Synonym Definition Etiology Pathology Symptoms When the disease is fully established, there is a constant uneasy sensation in the region of the larynx; the voice is seriously altered, and there is a constantly annoying cough, with expectoration. The expectoration is at first scanty and mucous; but as the disease advances it is muco-puriform, sanious, concreted into lumps, or consists of almost pure pus. Hemorrhage occurs in the latter stages, sometimes in very large quantities. If the throat is examined, we notice the evidence of chronic inflammation of the fauces, pharynx, epiglottis, and we reasonably suppose that the mucous membrane of the larynx corresponds in appearance; with the laryngoscope we are enabled to view the internal surface of the larynx, and determine its condition quite accurately. A person suffering from "ministers' sore-throat," or chronic laryngitis, is very subject to cold, and every change in the weather or slight exposure is followed by an increase of the disease. A very important part of the treatment of every case, therefore, will be directed to obviate this. The impairment of the general health is usually in direct ratio to the severity of the local affection. At the commencement, the patient complains simply of debility, with some failure of the digestive organs, and sometimes torpor of the secretions. When it has progressed for some months, he is unable to attend to business; there is loss of flesh and strength; there is marked impairment of the digestive functions and of the excretion.

Laryngitis is a common problem in which the vocal cords inside the larynx are inflamed. The main symptom is a hoarse voice. This ranges from mild to complete loss of voice. There may also be some pain in speaking or swallowing. The causes include infection, overuse of the voice, inhaling smoke or fumes, or a tumor in the throat. A common cold or flu can sometimes spread to the vocal cords. Other infections include tuberculosis, bronchitis, or sinusitis. Viruses that tend to cause croup in children may cause laryngitis in adults. Excessive use of the voice is common in certain jobs, such as teaching or public speaking. It can also result from cheering at sports events, singing, or shouting. Causes of chronic laryngitis include:
* other chronic disorders, such as allergies or sinusitis,
* breathing mostly through the mouth,
* smoking

Peer Review Status: Externally Peer Reviewed by Lauren D Holinger, MD, Robert J. Winter, MD and the AMA The croup syndromes refers to the abrupt onset of a barky cough, along with various combinations of inspiratory or biphasic stridor, hoarseness, and/or respiratory distress. It can be quite variable in course and symptoms, from a few days with mild upper respiratory tract infection (URI) symptoms to fulminant, rapidly progressive disease with death in a few hours. It consists of four distinct entities:
  • Acute laryngotracheobronchitis, ie, viral croup
  • Acute spasmotic laryngitis, ie, spasmotic croup
  • Acute infectious laryngitis
  • Acute epiglottitis, ie, acute supraglottitis
The first three will be discussed in this section, but acute epiglottitis, because of its potentially life-threatening nature, will be discussed separately. The croup syndromes has a characteristic sequence of events and clinical presentation caused by progressive edema of the airway. Generally, the onset of a mild, brassy, barky cough - often described as "barking like a seal" - is preceded by rhinorrhea and sniffles for 1-2 days. Intermittent respiratory stridor then occurs

Reveiz L, Cardona AF, Ospina EG ABSTRACT What's new in this issue Search abstracts Browse alphabetical list of titles Browse by Review Group A substantive amendment to this systematic review was last made on 17 November 2004. Cochrane reviews are regularly checked and updated if necessary. Background: Acute laryngitis is a common illness worldwide. Diagnosis is often made by case history alone and treatment is often directed towards controlling symptoms. Objectives: The aim of this review was to assess the effectiveness of different antibiotic therapies in adults suffering acute laryngitis. A secondary objective was to report the rates of adverse events associated with these treatments. Search strategy: We systematically screened the following electronic databases: the Cochrane Central Register of Controlled Trials (The Cochrane Library Issue 3, 2004); MEDLINE (January 1966 to June Week 3 2004); and EMBASE (1974 to June 2004), LILACS (from 1982 to the 40th edition) and BIOSIS (1980 to June 2002). Other strategies included hand searching relevant journals, searching ongoing trial databases and general databases such as Alta Vista. Selection criteria: Randomized controlled trials comparing any antibiotic therapy with placebo in acute laryngitis. The main outcome measurement was objective voice scores.
