WHAT IS KLINEFELTER SYNDROME? Top In 1942, Dr. Harry Klinefelter and his coworkers at the Massachusetts General Hospital in Boston published a report about nine men who had enlarged breasts, sparse facial and body hair, small testes, and an inability to produce sperm. By the late 1950s, researchers discovered that men with Klinefelter syndrome, as this group of symptoms came to be called, had an extra sex chromosome, XXY instead of the usual male arrangement, XY. (For a more complete explanation of the role this extra chromosome plays, see the accompanying section, "Chromosomes and Klinefelter syndrome." In the early 1970s, researchers around the world sought to identify males having the extra chromosome by screening large numbers of newborn babies. One of the largest of these studies, sponsored by the National Institute of Child Health and Human Development (NICHD), checked the chromosomes of more than 40,000 infants. Based on these studies, the XXY chromosome arrangement appears to be one of the most common genetic abnormalities known, occurring as frequently as 1 in 500 to 1 in 1,000 male births. Although the syndrome's cause, an extra sex chromosome, is widespread, the syndrome itself-the set of symptoms and characteristics that may result from having the extra chromosome-is uncommon. Many men live out their lives without ever even suspecting that they have an additional chromosome.

Causes, incidence, and risk factors: A person's sex is determined by the X and Y chromosomes. Normally, men have an X and a Y and women have two X's. In Klinefelter syndrome, a male has two X's and a Y. The condition is common and affects 1 in 500 men. The infant appears normal at birth, but the defect usually becomes apparent in puberty when secondary sexual characteristics fail to develop (or develop late). At this time, testicular changes occur that eventually result in infertility in the majority of those affected. Some mild cases may go undetected, with no abnormalities present except infertility. One risk factor for this condition is the mother giving birth at an older age. In most cases, the problem develops during fetal development and is not inherited from one or both parents.

Many physicians underestimate the prevalence of Klinefelter syndrome and so fail to recognize its more obvious features. Increased awareness of its effects on physical, psychological, and social development should help to dispel persistent misconceptions about the condition and enable earlier diagnosis and more effective treatment.
Dr. Smyth is in private practice and is also Clinical Instructor, Department of Internal Medicine, University of Washington, Seattle. Klinefelter syndrome is one of the most common causes of hypogonadism, affecting one in 500 men and boys. In two thirds of cases, the chromosomal karyotype is 47,XXY; in the remainder, variant and mosaic karyotypes predominate (Figure 1). Expression of the syndrome varies with the number of X chromosomes and the degree of endocrinologic dysfunction. The patient's history and physical examination provide enough information to diagnose the condition in puberty, but most patients do not seek medical attention until adulthood, when problems such as impotency or infertility become an issue. The following case is typical.
Case Presentation
A 44-year-old man presented to his primary care physician complaining of erectile dysfunction and infertility. Although he and his wife were able to have intercourse, they had been unable to have children. On questioning, he said that he was aware that he did not have the same libido as many of his friends. He had gone through puberty late and had not begun to shave until age 17. He had had academic, psychological, and social difficulties as a teenager and had been treated for depression as an adult. He had not had major medical problems such as high blood pressure or diabetes and did not drink or smoke.

Klinefelter's Syndrome There is a wide variety of symptoms of which any number may be expressed with varying severity. They include lack of secondary sexual characteristics, development of female characteristics, infertility, language impairment, and mental retardation. Mental retardation can be caused by Klinefelter Syndrome, but not in the usual 47, XXY karyotype. Instead, it results from multiple aneuploidies such as 48, XXXY; 48, XXYY; and 49, XXXXY (Robinson et. al., 1994). These individuals generally demonstrate more cognitive and behavioral problems, and therefore, can not be considered simultaneously with the more common 47, XXYcases (Utah Department of Health, 1993). Another possible variant is a mosaic individual which means that he possesses two or more different cell lines differing in the number of chromosomes (Robinson et. al., 1994). A mosaic Klinefelter male, which is much less common than the other types, would have a 46, XY/47, XXY karyotype. Klinefelter syndrome in these individuals can be more or less severe, depending on which cell line is used for the symptomatic traits. The prognosis for Klinefelter patients is bright in some areas and unclear in others. To deal with the lack of secondary sexual features and other problems caused by low quantities of testosterone, there is testosterone therapy. Testosterone should be administered starting at eleven to twelve years of age, when puberty normally begins. The benefit is a more normal appearance including weight gain, increased muscle mass, increased hair distribution, acne, and less feminization (Slywka, 1994). If the syndrome is diagnosed later in life, testosterone treatmentsshould still be helpful and effective and usually increase sex drive. These treatments are available in oral, injectable, and patch therapy form. If gynecomastia is already prevalent, breast reduction surgery is a feasible option.

Abnormalities of the sex chromosomes occur in about 1.5 per 1000 births, meaning that about 1200 children are born each year with one of these conditions. Turners syndrome, in which all or part of the X chromosome is missing, affects only girls, and is easily recognised, with short stature being an obvious symptom. At birth, infants are often smaller in respect of birthweight, length, and head circumference. During childhood, growth tends to be ahead of the child's peers, particularly in terms of leg length. Even by the age of three years leg length may be disproportionate to sitting height. In a clinical study of XXY boys, two thirds of the boys developed steadily increasing obesity, while the rest displayed lean and lanky stature. In three boys, who had high growth velocities, malposition of the testes was observed, with ascent of one testis occuring, requiring orchidopexy. Testicular development was normal up to the age of one year, apart from a reduction in the number of spermatogonia. Later, tubular diameter became reduced, and germ cells were not seen. Testicular volume increased in all boys at puberty, but this was followed by involution in those with the greatest development, resulting in a slight reduction in final volume.
