Tip: Kleine-Levin Syndrome Kleine-Levin Syndrome is a rare disorder characterized by the need for excessive amounts of sleep (hypersomnolence - i.e., up to 20 hours a day); excessive food intake (compulsive hyperphagia); and an abnormally uninhibited sexual drive. The disorder primarily affects adolescent males. The cause of the problem is still a mystery, though it appears to have something to do with the lower brain, the hypothalamus.

Tip: Characteristics Of People With Noctural Sleep-Related Eating Disorder Many are severely stressed, anxious individuals who are dismayed and angry at themselves for their nocturnal loss of control. Many diet during the day which may leave them hungry and vulnerable to binge eating at night when their control is weakened by sleep. People with NS-RED sometimes have histories of alcoholism, drug abuse, and other sleep disorders like sleep walking, restless legs, and sleep apnea. Definition
Sleep disorders are a group of syndromes characterized by disturbance in the patient's amount of sleep, quality or timing of sleep, or in behaviors or physiological conditions associated with sleep. There are about 70 different sleep disorders. To qualify for the diagnosis of sleep disorder, the condition must be a persistent problem, cause the patient significant emotional distress, and interfere with his or her social or occupational functioning. The fourth edition (1994) of the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) specifically excludes temporary disruptions of sleeping patterns caused by travel or other short-term stresses. Although sleep is a basic behavior in animals as well as humans, researchers still do not completely understand all of its functions in maintaining health. In the past 30 years, however, laboratory studies on human volunteers have yielded new information about the different types of sleep. Researchers have learned about the cyclical patterns of different types of sleep and their relationships to breathing, heart rate, brain waves, and other physical functions. These measurements are obtained by a technique called polysomnography There are five stages of human sleep. Four stages have non-rapid eye movement (NREM) sleep, with unique brain wave patterns and physical changes occurring. Dreaming occurs in the fifth stage, during rapid eye movement (REM) sleep.

Imagine going to bed and not waking up for five months! Sounds like the story of Rip Van Winkle, but Kristin Brown's life is no fairy tale. She suffers from a tragic sleep disorder doctors can't predict, explain, or cure. EXTRA puts this puzzling condition in focus. For 23-year-old Kristie Brown, preparing for bed each night is frightening. She knows that once her head hits the pillow she may not wake up for days, weeks, and even months at a time. Kristie suffers from one of the world's rarest and most bizarre sleep disorders. It's called Kleine-Levin Syndrome and its cost Kristin her teenage years, her friends, and a normal life. "I missed my homecoming. I missed cheerleading. I missed all kinds of Dances and social events." Kristie says. It started when Kristie was only 15. That's when she went to bed one night and didn't fully wake up for 10 days straight. "She didn't respond to any of our questions anything that we asked her. She just stayed in bed and stayed extremely quiet." Kristie's mother tells us. "In September of 1993 she went to sleep and she did not wake up until February of 1994." We watched a home video of Kristie during an episode. She drifts in and out of a sleeplike state. Each time Kristie's mother stays by her side offering a little bit of food, a little bit of water, occasional groggy, trance-like trips to the bathroom.

Background.- Kleine-Levin syndrome is characterized by recurring episodes of hypersomnia, mental disturbances and abnormal behaviour including hyperphagia and hypersexuality. The episodes start suddenly generally after a mild viral infection, mild head injury or sometimes after a psychological stress. They last few days to several weeks and end abruptly. Between the episodes, there is no sleep disturbance and patients are physically and mentally normal. Patients sleep as long as 18 to 20 hours of the day during the episode. Hyperphagia and/or hypersexuality. are in compulsive manner. Mental disturbances include, irritability, feeling of unreality or confusion. Recently, monosymptomatic and atypic forms of the syndrome were reported. In monosymptomatic form, hypersomnia occurs without hyperphagia and hypersexuality. Atypical Kleine-Levin Syndrome includes the forms where one of the symptom is replaced by an opposite one. Observation.-

RECURRENT HYPERSOMNIA
Periods of extreme sleepiness. An excessive need for sleep, sometimes as much as eighteen to twenty hours a day. This can last for weeks, and in some cases, even months and can reoccur several times a year. Kleine Levin syndrome is an example of a type of recurrent hypersomnia. SYMPTOMS:
  • Excessive sleepiness that lasts for a month or more Can't wake up in the morning despite having slept twelve or more hours Need too sleep during the day despite sleeping long hours at night Naps still leave them feeling fatigued and mentally muddled Doing things without any awareness of having done them Aggressive and/or confused upon awakening.

  • TREATMENT No known or guaranteed treatment. Behavioral changes, like keeping naps to a minimum and keeping to a regular sleep schedule are sometimes helpful Stimulants may help to some extent.

Below are descriptions of both disorders that we hope are helpful to you. In both cases your doctor must determine treatment. Hypersomnia is a condition in which one sleeps for an excessively long time but is normal during waking hours. Chronic hypersomnia typically occurs in conjunction central nervous damage or may be secondary to some physical and mental illnesses, particularly depression. Kleine-Levin Syndrome (KLS) is a rare disorder also characterized by the need for excessive amounts of sleep. The patient becomes progressively drowsier and sleeps for most of the day and night, and in some cases requires excessive food intake. The disorder primarily strikes adolescents. When awake, affected individuals may exhibit irritability, lack of energy, lack of emotions, and they may appear to be confused. Many patients report a hypersensitivity to noise. Some patients show an abnormally uninhibited sexual drive. Symptoms of Kleine-Levin are cyclical. An affected individual may go for weeks or months without experiencing any symptoms, with perfect health and no evidence of behavioral or physical dysfunction. When present, KLS symptoms may persist for days to weeks or even months. The exact cause of KLS is not yet known. It is thought that symptoms of KLS may be related to malfunction of the portions of the brain that help to regulate functions such as sleep, appetite and body temperature. It appears to be self-limiting with episodes ending by early adulthood.
