  • Introduction K leine-Levin syndrome (KLS) is a relatively rare sleep disorder characterized by recurrent episodes of hypersomnia, hyperphagia, and mental and behavioral changes. Other features include apathy, irritability, confusion, sexual disinhibition, delusion, hallucination, disorientation memory impairment, incoherent speech and depression or agitation. In about 50% of cases, the syndrome has occurred following a specific factor such as thalamic infarction, Epstein Barr/herpes zoster infection, encephalitis

    General Discussion
    Kleine-Levin syndrome is a rare disorder characterized by the need for excessive amounts of sleep (hypersomnolence), (i.e., up to 22 hours a day); excessive food intake (compulsive hyperphagia); and an abnormally uninhibited sexual drive. The disorder primarily affects adolescent males. When awake, affected individuals may exhibit irritability, lack of energy (lethargy), and/or lack of emotions (apathy). They may also appear confused (disoriented) and experience hallucinations. Symptoms of Kleine-Levin syndrome are cyclical. An affected individual may go for weeks or months without experiencing symptoms. When present, symptoms may persist for days to weeks. In some cases, the symptoms associated with Kleine-Levin syndrome eventually disappear with advancing age. However, episodes may recur later during life.
    The exact cause of Kleine-Levin syndrome is not known. However, researchers believe that in some cases, the disorder may be inherited as an autosomal dominant genetic trait. It is thought that symptoms of Kleine-Levin syndrome may be related to malfunction of the portion of the brain that helps to regulate functions such as sleep, appetite, and body temperature (hypothalamus).

    Kleine-Levin Syndrome Diaries The following is the diary of Jeff Sagel. FIRST EPISODE Sat., Jan.22, 1994 NOTE* Jeff grew 6" over the summer preceding his first episode and 1 1/2" from the time he was admitted until he went back to the hospital at the beginning of March. ** Jeff had chicken pox when he was 4 years old. Then he had shingles when he was 10 years old. ** Do either of these things have anything to do with this illness? Perhaps hormones or a chemical imbalance? Perhaps chicken pox virus manifesting itself in different form? ** This episode lasted 7 days. SECOND EPISODE Fri., Nov. 4, 1994 THIRD EPISODE Sun., Aug. 11, 1996 FOURTH EPISODE-Nov. 1996 Nov., 1996 Jeff saw Dr. Sherman at Children’s. We discussed the best way to take care of the remaining cavities. We decide that since the xylocaine seemed to bring on an episode, we shouldn’t take that route for the rest because it would take so many trips and so many chances to go into sleep mode. So, we decide to do laughing gas and fill all the teeth at once and pull his wisdom teeth while he is out. It’s set for Mon., Dec.16, 1996. Sun. Dec. 15, 1996 Jeff came home from work early (1:00 P.M.) and went straight to bed. I couldn’t wake him at 6:00 for dinner. I tried again at 10:30 because he’s not allowed to eat after midnight. but could not wake him. FIFTH EPISODE Tues., April 8, 1997

In this syndrome patients can always be roused from the day-time sleep. However they are often irritable on waking and occasionally aggressive. Some patients are muddled and experience depression, disorientation and hallucinations. The combination of sleep disturbance and appetite disorder have pointed to a possible hypothalamic disorder.     A pathologic basis for Kleine-Levin syndrome

    A patient with Kleine-Levin syndrome, typical except that onset was at 39 years of age, died during a symptomatic period. Autopsy disclosed recent and old lesions in the medial thalamus involving intralaminar, medial, and some dorsal nuclei as well as the pulvinar. Despite massive microglial infiltration, there was minimal neuronal loss. The hypothalamus was not involved. The findings suggest a viral cause for Kleine-Levin syndrome.

    Kleine-Levin syndrome: A rare condition characterized by excessive need for sleep, food, and sexual disinhibition. Most people with Kleine-Levin syndrome are adolescent males. When awake, they may be confused, irritable, and lethargic, and some have hallucinations. The cause is unknown, although current theory holds that Kleine- Levin syndrome is caused by an inherited autosomal dominant trait that affects the hypothalamus gland. Dollison Purdue University Kleine-Levin syndrome is a rare disorder characterized by alternating periods of exceedingly prolonged sleep and disordered waking and normal sleep and alert waking. Wakeful periods during an episode are usually marked by social withdrawal and accompanied by a variety of maladaptive behaviors, including hallucinations, delusions, sexual disinhibition, severe anxiety, depression, memory impairment, and increased appetite. Due to the rarity and complexity of the disorder, Kleine-Levin can be easily misdiagnosed.
