23. In Focus: A Photojournalist's Journey Through Kidney Failure
John F. Martin, a photojournalist, documented his bout with kidney failure, dialysis and the livedonor transplant he received from his father. Over 100 photos take the reader through the year-long process step-by-step.
http://www.johnfmartin.net
idney disorders currently affect some 3 million people nationwide. In the United States, more than 44,000 people are on The List, patiently waiting to get a phone call telling them they're a near-perfect match for a cadaveric kidney. Hospitals across the country are flooded with more than 100,000 dialysis patients, who sit for hours at a time as man-made machines do what their own kidneys can't: clean toxins and fluid from their blood. In the fall of 1998, these numbers were little more than a jumbled set of statistics for me. I knew early in life there was a chance my kidneys could fail, but I never gave it a second thought. I was too busy growing up and pursuing a career in photojournalism. 5-year anniversary update
When I learned my kidneys were failing, I turned to the Web for more information. I was disappointed in what I found. While there are a ton of sites filled with statistics and jargon about renal failure and countless kidney organizations and support groups, there was no detailed story of one person's battle with kidney failure, dialysis and transplant. I was looking for someone's complete story that could give me some idea of what was ahead.
Who better to undertake such a project than a photojournalist? I admit the whole idea of turning the camera around made me uncomfortable. After all, my place is behind the viewfinder. Thankfully, all the shooters involved are friends and they made it easy for me to become the subject, and after a while, I forgot they were there. The chapters of this Web site chronicle my journey through the kidney transplant process, from early discussions with my doctor, to dialysis and ultimately to the surgery on March 31, 2000. My goals with this site are to help patients and families better understand what may be ahead for them on the kidney failure road and to demystify the transplant process. But please remember, this is just what happened to me. Every patient is different and there are countless twists, turns and pot holes for everyone along the way. ... Contact Information

Chronic Kidney Disease Overview
Normal Kidneys and Their Function
The kidneys are connected to the urinary bladder by tubes called ureters. Urine is stored in the urinary bladder until you are ready to empty your bladder. The bladder is connected to the outside of the body by another tubelike structure called the urethra. The main function of the kidneys is to remove waste products and excess water from the blood. The kidneys process about 200 liters of blood every day and produce about 2 liters of urine. The waste products are generated from normal metabolic processes including the breakdown of active tissues, ingested foods, and other substances. Your kidneys allow you to continue to consume a variety of foods, drugs, vitamins and supplements, additives, and excess fluids without worry that toxic by-products will build up to harmful levels. The kidney also plays a major role in regulating levels of various minerals such as calcium, sodium, and potassium in the blood.
  • As the first step in filtration, blood is delivered into the glomeruli by microscopic leaky blood vessels called capillaries. Here, blood is filtered of waste products and fluid while red blood cells, proteins, and large molecules are retained in the capillaries. In addition to wastes, some useful substances are also filtered out. The filtrate collects in a sac called Bowman capsule and drains into the tubule.

Scrapbook: The Journal The Allegis Times Faye won Maryland's Most Beautiful People volunteer award in November 1993. I have known Faye all my life, from my birth to her death. I remember my first day of pre-school, when Faye brought me to my classroom. I held on her and cried and screamed; I did not want to let go. I remember when in some fit of childish anger, I bit Faye on the shoulder and lost one of my front teeth. I cried as much in sorrow for my act as in pain. I remember when, during one recess in pre-school, absorbed in my own thoughts, I paced endlessly in circles. I did not notice when everyone else went inside. Finally I did look up everyone was gone. I ran inside. Faye was there to comfort me. I remember Faye's car accident. It was January, I think, in 1988. When she returned to work after a month in the hospital, I pretended to be sick and stayed home from kindergarten so I could see her once more. I remember when my parents would go on trips and leave me with Faye for a day or two. After her accident, Faye rented the basement of the house of a women named Yo, who lived only a few minutes from us. Sometimes I would sleep over with Faye in her tiny basement-apartment. I remember Faye's TV shows wrestling, soap operas like

What is kidney failure?
The function of the kidneys is, among other things, to get rid of the waste products that result from the body's metabolism.
One of the major by-products of the metabolism of protein (muscle) is urea. The kidneys remove the waste products by extracting them from the blood and sending them along the ureter to the bladder, from where they are excreted in the urine.
If the kidney function fails, the waste products accumulate in the blood and the body. The term for this build-up is azotaemia.
Term watch
Azotaemia is the build up of waste products that accumulate in the blood and the body when the kidneys fail to function.

Often called CRF Chronic Renal Failure, it is seen most often in the older dog or cat. Kidney failure has it origins in a wide variety of causes. For example, some animals are born with poorly constructed or functioning kidneys and never reach totally optimum health. Eventually, these individuals usually fall into kidney failure at an early age. Another type of kidney failure can occur after accidental ingestion of antifreeze that contains ethylene glycol. These situations cause sudden and often irreversible renal (kidney) failure. Quite commonly, kidney failure results from low grade, long term (chronic) inflammation of kidney tissues called chronic nephritis ; the damage caused by this inflammation renders permanent damage to delicate renal tissues that are not able to repair themselves or heal as some other body tissues do. Eventually, not enough normal functioning tissue remains to support the body's need for metabolic regulation and waste elimination. The dog or cat becomes UREMIC, a condition where body waste material builds up in the blood. These toxins promote vomiting, poor appetite, depression, and eventually death. Bacterial invasion of the kidneys is a common cause of chronic nephritis and kidney failure.

Kidney Disease
Home Conditions It is estimated that more than 1.2 million people worldwide suffer from end-stage renal disease (ESRD), a number that is growing at a rate of approximately six-to-seven percent annually. Additionally, the National Kidney Foundation (NKF) predicts that the number of kidney failure patients in the U.S. will double in the next ten years. This growth is influenced in part by diseases associated with the aging population such as diabetes and high blood pressure, two leading causes of ESRD. Current treatment options for ESRD include peritoneal dialysis (PD), hemodialysis (HD) and kidney transplantation. Worldwide, more than one million people suffering from chronic kidney failure are treated with some form of dialysis. However, approximately three million people with kidney failure currently go undiagnosed or untreated, particularly in developing countries. Baxter is committed to helping kidney disease patients across the globe receive the best treatment options available to them. Chronic Kidney Failure Chronic kidney failure occurs when the kidneys are slowly damaged - this damage is over the long term and is not reversible. Most people with chronic kidney failure will require treatment to replace the function of their own kidneys.
