Introduction One of the more common conditions encountered in pets, especially as they age, is kidney (renal) disease. Melanie's PKD Page This page is all about PKD (Polycystic Kidney Disease). Even though very few people have heard of it, Polycystic Kidney Disease affects 600,000 people in the U.S. alone, and 12.5 million world wide. There are more people with PKD than cystic fibrosis, muscular dystrophy, hemophilia, Downs syndrome and sickle cell anemia combined. Many PKD patients find it useful to have access to the same information as their doctors because many doctors are unfortunately pretty unfamiliar with the disease. I will try to bring you as much information and sources about the disease as possible. If you have any questions about PKD for any reason, or if you know of anyone who has the disease and is looking for more information or for support groups I encourage you to contact any of the sources I have listed here, or to ask me a question . I would also appreciate an email if you have any pertinent information I should add to this page. PKD has been a profound part of my life so far, and will only continue to become more so. Likewise, the information on this page is just the beginning, and will continue to grow as time and research allows. *All times Eastern (CNN) Movie star and political activist Danny Glover is best known for his role as Sgt. Roger Murtaugh in the Lethal Weapon films, and it's his warm-hearted relationship with Mel Gibson's character which keeps movie patrons coming back. But when I caught up with Glover recently, he told me how his "real-life" relationship with his late father is one he cherishes the most. "I have great memories of my father. He was absolutely the most beautiful man I ever met in my life," Glover said. In 2001, James Glover passed away from complications related to kidney disease. His son, however, says that his father faced his six-year battle with the disease with determination and a positive attitude.

What is Polycystic Kidney Disease?
Polycystic kidney disease (PKD) is a disorder that is characterized by the growth of numerous cysts in the kidneys. The cysts are filled with fluid. PKD cysts can replace much of the mass of the kidneys, thereby reducing kidney function and leading to kidney failure The kidneys are two organs, each about the size of a fist, that are located in the upper part of the abdomen, towards the back. The kidneys filter wastes from the blood to form urine. They also regulate amounts of certain vital substances in the body. When PKD causes kidneys to fail, which usually happens only after many years, the patient requires dialysis or kidney transplantation. About one-half of people with the primary form of PKD progress to kidney failure or end-stage renal disease (ESRD).

    What is diabetic nephropathy?
    Nephropathy is the deterioration of the kidneys. The final stage of nephropathy is called end-stage renal disease, or ESRD. Diabetes is the most common cause of ESRD, accounting for more than 43 percent of cases. About 17 million people in the US have diabetes, and more than 100,000 people have ESRD as a result of diabetes. Both type 1 and type 2 diabetes can lead to diabetic nephropathy, although type 1 is more likely to lead to ESRD. There are five stages of diabetic nephropathy, or deterioration of the kidneys. The fifth stage is ESRD. Progress from one stage to the next can take many years, with 23 years being the average length of time to reach stage five.

By Joel R. Cooper Several years ago, I dated a woman whose father, a big computer company executive, had polycystic kidney disease. He received dialysis treatments at home, and was okay for a while, then developed complications and died. I remember seeing him at home on that machine which took blood from his body, then routed it into its mechanical innards and back into him after cleansing it somehow. Having never worked in a clinical environment before, seeing him lying there hooked up to a machine upon which he depended for life made me feel queasy and uncomfortable. Perhaps the experience reminded me of the fragility of life in general and my own mortality in particular. This was my first exposure to polycystic kidney disease. It had an impact. The woman, too, had the disease, which was genetically passed on to her from her father. A ballet instructor and choreographer, she's still very much alive today, but she has to be careful about her health and diet. And she knows her future is not necessarily very bright. She could die the same way her father did, if medical science can't produce a more palatable outcome for her. 600,000 Americans have the same disease she does, yet it's hardly a household term like AIDS. Polycystic kidney disease (PKD), sometimes called autosomal dominant polycystic kidney disease, is a life-threatening genetic disorder that eventually produces end-stage renal (kidney) disease in the majority of cases. Patients progressing to end-stage renal disease, must have either hemodialysis (a removal of the toxic substances in the blood by a machine that acts as an artificial kidney) or a kidney transplant to survive. According to the Polycystic Kidney Research Foundation, Kansas City, Missouri USA, an estimated 5 to 10 million people have the disease worldwide.

Chronic kidney failure is the most significant result of chronic kidney disease. When kidney function has deteriorated and is no longer adequate to sustain life and the process is considered irreversible, renal replacement therapy (RRT)—dialysis or transplantation—becomes necessary to maintain life. Treated chronic kidney failure, also called end-stage renal disease (ESRD), is the most feared consequence of kidney disease. Chronic renal insufficiency, however, is more common than treated chronic kidney failure and can also severely affect health and well-being. Therefore, ideally, programs should be directed at preventing the development of chronic renal insufficiency and its subsequent progression to ESRD.
Unfortunately, chronic renal insufficiency is usually asymptomatic, and the exact number of people affected is unknown. The best available estimates are based on national surveys. Current estimates indicate approximately 10 million persons aged 12 years and older have some form of chronic kidney disease.

When someone is on dialysis, a family lives with it; when someone dies and organ donation is a possibility, a family decides; when early identification of kidney disease occurs, a family participates in the treatment; and when someone needs a transplant, a family waits. There are many forms of Kidney Disease from Kidney Stones to Kidney Cancer. Some have long, daunting names like Glomerulonephritis, Glomerulosclerosis, or Microalbuminuria. Some are classified as "syndromes", such as Alport Syndrome, Nephrotic Syndrome, and Hemolytic Uremic Syndrome. Some require aggressive treatment, others, drug therapy, while a few conditions can be controlled with simple changes in diet and routine.

Kidney Diseases Definition Kidney diseases are disorders that affect the kidneys; the two organs that remove waste products, produce certain hormones, and regulate the level of chemicals in blood. Description Kidney Function A major function of the kidneys is to remove waste products and excess fluid from the body. These waste products and excess fluid are removed through urine. The production of urine involves highly complex steps of excretion and reabsorption. This process is necessary to maintain a stable balance of body chemicals. The critical regulation of the body's salt, potassium, and acid content is performed by the kidneys. The kidneys also produce hormones and vitamins that affect the function of other organs. For example, a hormone produced by the kidneys stimulates red blood cell production. In addition, other hormones produced by the kidneys help regulate blood pressure and others help control calcium metabolism. There are two kidneys, each about the size of a fist, located on either side of the spine at the lowest level of the rib cage. Each kidney contains about one million functioning units, called nephrons. A nephron consists of a filtering unit of tiny blood vessels, called a glomerulus, attached to a tubule. When blood enters the glomerulus, it is filtered and the remaining fluid passes along the tubule. In the tubule, chemicals and water are either added to or removed from this filtered fluid, according to the body's needs, with the final product being the urine we excrete.

Kidney disease, in the form of chronic renal failure, is a common problem in older cats. I have seen kidney failure in cats as young as 4 years, but more frequently in much older cats. The most noticeable symptom is an increase in water consumption and urination ("drink-a-lot, pee-a-lot syndrome"). A blood test should be done if you notice these symptoms, as there are several conditions that can cause this. The increase in drinking and urinating is due to loss of the kidney's ability to concentrate the urine. The kidneys have a very large reserve capacity, and symptoms of failure are not seen until approximately 75% of kidney tissue is nonfunctional. In my experience, kidney failure is the most common cause of death in older cats. Chronic renal failure is progressive and incurable. No conventional or alternative medical treatment can reverse its course, since the disease involves the loss of kidney cells and replacement by scar tissue. The rate of progression in any individual cat probably cannot be slowed to any significant degree. When the process is advanced, the kidneys become small and lumpy, and the amount of functional tissue is greatly limited. The most significant problems caused by the loss of function are build-up of blood toxins, and anemia. These can cause weight loss, lethargy, vomiting, loss of appetite, weakness, and other signs of illness. Some cats are able to maintain their body weight and live relatively comfortable lives for months to years, while others succumb to the disease more quickly. In conventional medicine, there are drugs that can minimize anemia, and phosphate binders to prevent phosphorus precipitates from further damaging the kidneys.
