    Abstract
    Kernicterus, currently used to describe both the neuropathology of bilirubin-induced brain injury and its associated clinical findings, is a complex syndrome. The neurobiology of kernicterus, including the determinants and mechanisms of neuronal injury, is discussed along with traditional and evolving definitions ranging from classical kernicterus with athetoid cerebral palsy, impaired upward gaze and deafness, to isolated conditions, for example, auditory neuropathy or dys-synchrony (AN/AD), and subtle bilirubin-induced neurological dysfunction (BIND). The clinical expression of BIND varies with location, severity, and time of assessment, influenced by the amount, duration and developmental age of exposure to excessive free bilirubin. Although total serum bilirubin (TSB) is important, kernicterus cannot be defined based solely on TSB. For study purposes kernicterus may be defined in term and near-term infants with TSB

    A physiologic approach to identifying neonates at risk for kernicterus

    University of Texas Health Science Center, School of Nursing, San Antonio 78284. Nurses routinely assess skin color and serum bilirubin levels to identify newborns at risk for kernicterus. High serum bilirubin levels can be toxic to the central nervous system. Through careful evaluation, nurses can help prevent the occurrence of adverse consequences from hyperbilirubinemia. By understanding the physiologic principles involved in bilirubin metabolism and the transition that must be made at birth, nurses can help in identifying and assessing risk factors in the newborn.

The activities of choline acetyltransferase (CAT) and glutamic acid decarboxylase (GAD), markers of cholinergic and GABAergic terminals, were measured in growing and adult Gunn rats, which possess an autosomal recessive gene for jaundice or kernicterus. In the olfactory tubercle, hippocampus, neostriatum, thalamus, and hypothalamus of the animals with kernicterus, the development of the cholinergic pathways was delayed, but by the adult stage it was normal, while there was practically no action on the innervation by GABAergic neurons, at least as indicated by the chemically measured parameters.     Alternate Names : Erythroblastosis Fetalis, Kernicterus, Hydrops Fetalis, Rh Isoimmunization Attribution Rh incompatibility is a condition that occurs when the mother of a fetus or newborn has Rh-negative blood type and the fetus or newborn has Rh-positive blood. This incompatible blood reaction may cause problems in a newborn as well as life-threatening problems for future pregnancies What is going on in the body? The Rh factor, or rhesus factor, is a marker that may or may not be present on the surface of a person's red blood cells. When a woman has the Rh component in her blood, she is considered Rh positive. When she does not have the Rh factor, she is considered Rh negative. When a person who has Rh-negative blood is exposed to Rh-positive blood, that person's body does not recognize the Rh factor and considers it something foreign. The body builds antibodies against it as it would for any foreign substance that is introduced into the blood. If the person who is Rh negative is ever exposed in the future to Rh-positive blood, his or her body is armed to attack the red blood cells that have the Rh factor. Problems may arise if a woman with Rh-negative blood conceives a baby who has Rh-positive blood. (This may occur if the father of the baby has Rh-positive blood.) The pregnant woman's body can become sensitive to the Rh factor and build up "antibodies" to attack the Rh factor.

    De aanwezigheid van kernhoudende rode bloedlichaampjes wijst op een versnelde afbraak van bloedlichaampjes. Normaal komt deze jonge vorm van het rode bloedlichaampje niet in het circulerend bloed voor. De geelkleuring van de kern is het gevolg van ophoping van galkleurstof in het bloed.
    Door de verhoogde afbraak van bloedkleurstof komen bovendien grote hoeveelheden van het giftige afbraakproduct bilirubine in het bloed. Deze stof wordt normaal door de lever en de milt verder afgebroken tot onschadelijke stoffen.
    Bij kernicterus is deze leverwerking echter gestoord. Lever en milt zijn vergroot. Het kind vertoont soms al binnen enkele uren na de geboorte een ernstige geelzucht. De stijgende hoeveelheid bilirubine vergiftigt voornamelijk de zenuwkernen in de hersenstam en de basale zenuwkernen van de grote hersenen. Het kind gaat daardoor zenuwstoringen vertonen. Het ligt met het hoofd achterover, is geheel verstijfd en kan stuipen vertonen.
    De belangrijkste behandeling van kernicterus bestaat uit wisseltransfusies waarbij men het bloed van de baby vervangt door bloed zonder bilirubine.

    Pediatricians are being asked to renew their efforts to keep kernicterus in check. To that end, AAP members are advised to follow the AAP practice parameter titled, Management of Hyperbilirubinemia in the Healthy Term Newborn Pediatrics. 1994;94:558-565 [Medline]
    Dr. Maisels "Kernicterus is rare but it still occurs and pediatricians need to be cognizant of this," said Jeffrey Maisels, M.D., FAAP, chair of the AAP Subcommittee on Neonatal Hyperbilirubinemia. He added that the guidelines are under revision, but are still current and, if followed, can help pediatricians prevent kernicterus from occurring. Newborns with kernicterus can develop a severe form of athetoid cerebral palsy, hearing loss and intellectual and other handicaps.
