Kernicterus
Bilirubin encephalopathy It is a complication of neonatal jaundice . The word kernicterus means yellow kern, kern being the structures of the brain that are most commonly affected. They are the basal ganglia, hippocampus, geniculate bodies and cranial nerve nuclei, especially the oculomotor, vestibular, and cochlear. The cerebellum can also be affected. At postmortem examination a characteristic yellow stain is seen in those structures. Acute bilirubin encephalopathy is the clinical manifestation of bilirubin toxicity. There is hypotonia followed by hypertonia, opisthotonus (backward arching of the neck), or retrocollis (backward arching of the back) or both. It is taken as synonymous with kernicterus. Much of the traditional teaching about neonatal jaundice and kernicterus has been revised. It is the premature baby who is at risk and kernicterus rarely affects a term infant unless bilirubin levels are exceptionally high. A high level of physiological jaundice, especially in breast fed babies, seems benign and resolves spontaneously without complications. Epidemiology: Kernicterus occurs in all part of the world but is more frequent where G6PD deficiency is common. Numbers fell in the 1970s and 1980s only to rise again in the 1990s.

Rh incompatibility is a condition which develops when there is a difference in Rh blood type between that of the pregnant mother (Rh negative) and that of the fetus (Rh positive).
Alternative Names: Kernicterus; Rh-induced hemolytic disease of the newborn; Hydrops fetalis
Causes, incidence, and risk factors: During pregnancy , red blood cells from the fetus can get into the mother's bloodstream as she nourishes her child through the placenta. If the mother is Rh negative, her system cannot tolerate the presence of Rh positive red blood cells. In such cases, the mother's immune system treats the Rh positive fetal cells as if they were a foreign substance and makes antibodies against the fetal blood cells. These anti-Rh antibodies may cross the placenta into the fetus, where they destroy the fetus' circulating red blood cells. First-born infants are often not affected (unless the mother has had previous miscarriages/abortions, which could have sensitized her system) as it takes time for the mother to develop antibodies against the fetal blood. However, second children who are also Rh-positive may be harmed.

The yellow staining in the brain of a neonate is known as kernicterus. There is a coronal section of medulla on the left and cerebral hemisphere on the right demonstrating kernicterus in deep grey matter of hemisphere and brain stem. Increased unconjugated bilirubin, which accounts for the kernicterus, is toxic to the brain tissue. Kernicterus is more likely to occur with prematurity, low birth weight, and increased bilirubin levels.

Kernicterus is a rare neurological disorder characterized by excessive levels of bilirubin in the blood (hyperbilirubinemia) during infancy. Bilirubin is an orange-yellow bile pigment that is a byproduct of the natural breakdown of hemoglobin in red blood cells (hemolysis). Toxic levels of bilirubin may accumulate in the brain, potentially resulting in a variety of symptoms and physical findings. These symptoms may include lack of energy (lethargy), poor feeding habits, fever, and vomiting. Affected infants may also experience the absence of certain reflexes (e.g., Moro reflex, etc.); mild to severe muscle spasms including those in which the head and heels are bent backward and the body bows forward (opisthotonus); and/or uncontrolled involuntary muscle movements (spasticity). In some cases, infants with kernicterus may develop life-threatening complications.

Kernicterus is a debilitating and potentially deadly condition that may appear in newborns. The condition typically occurs in infants suffering from jaundice . Jaundice affects the liver's ability to process bilirubin, a byproduct of red blood cell disposal. Excess bilirubin can accumulate in the brain causing Kernicterus. Kernicterus is damage to the brain centers of infants caused by jaundice. Newborn babies are often "polycythemic", meaning they have too many red blood cells. When they break down the cells, one of the by-products is bilirubin, which circulates in the blood, and causes jaundice.
In adults and older children, jaundice is harmless in and of itself. But the tissues protecting the brain (the blood-brain barrier) are immature in newborns. The bilirubin penetrates the brain, and literally stains the nerve bodies, causing irreversible damage. Depending on the level of exposure, the effects range from unnoticeable to severe brain damage.

NEONATAL JAUNDICE Because of shorter hospital stays for newborns, severe jaundice, which can cause hearing loss and brain damage or kernicterus, is going unnoticed in some infants. Caused by excess bilirubin in the blood, jaundice may not develop until several days after birth. About 60% of American newborns have jaundice, although many recover without treatment. But an accumulation of excess bilirubin is toxic to the brain.
