Keratoconus is a non-inflammatory thinning and bulging of the cornea. This bulging results in distortion of the cornea, and an associated reduction in visual acuity. Corneal scarring occurs in more advanced cases and further reduces acuity. This page contains facts and explanations about keratoconus, and hopes to answer questions more deeply than other resources the patient may have available.

Keratoconus
Overview Keratoconus is a degenerative disease of the cornea that causes it to gradually thin and bulge into a cone-like shape. This shape prevents light from focusing precisely on the macula . As the disease progresses, the cone becomes more pronounced, causing vision to become blurred and distorted. Because of the cornea's irregular shape, patients with keratoconus are usually very nearsighted and have a high degree of astigmatism that is not correctable with glasses. Keratoconus is sometimes an inherited problem that usually occurs in both eyes. Illustration by Mark Erickson
Signs and Symptoms
  • Nearsightedness Astigmatism Blurred vision - even when wearing glasses and contact lenses Glare at night Light sensitivity Frequent prescription changes in glasses and contact lenses Eye rubbing

Detection and Diagnosis
Keratoconus is usually diagnosed when patients reach their 20's. For some, it may advance over several decades, for others, the progression may reach a certain point and stop.

Keratoconus creates an irregular cornea that is a contraindication for most refractive surgery, but can be treated with Intacs inserts and/or C3-R. As keratoconus advances, rigid gas-permeable (RGP) contact lenses maybe the only non-surgical way to achieve clear vision. Contact lens fitting of a patient with keratoconus requires much care and frequent check-ups, because the condition can worsen.. Keratoconus is a contraindication for conventional or custom wavefront LASIK IntraLASIK PRK ... Epi-LASIK , or any refractive surgery technique that removes tissue, due to the high probability that these procedures may further weaken a cornea affected by keratoconus. RLE and P-IOL are lens-based procedures that neither add nor subtract from the cornea, however advanced keratoconus may require other surgery that would be best performed before RLE or P-IOL. CK has been successfully used to correct hyperopia and astigmatism in patients with keratoconus. CK simply makes vision better, however does not apparently help the underlying problem.

Keratoconus creates an irregular cornea that can be treated with Intacs inserts and/or C3-R.
The theory behind C3-R treatment is that the fibrils of the keratoconus cornea lose their ability to link to each other.
C3-R treatment causes more cross-linking of the fibrils, making the cornea stronger.
The image shows a map of an eye with keratoconus. The off-center red and yellow area shows the forward bulging due to keratoconus.
The same eye after a Intacs and a single 30-minute C3-R treatment showing reduction of the cone and centration of the keratoconus effects.
Intacs are tiny plastic rings surgically added to the cornea. Keratoconus is a disease of the cornea that makes the cornea become weak and may gradually bulge outward. Most often, this bulging is in the lower half of the cornea and first presents as astigmatism , however not all astigmatism is due to keratoconus. In mild or early stages of keratoconus (forme fruste keratoconus), eyeglasses may correct the astigmatic vision.

Keratoconus and Cornea Transplants
Introduction
I have an eye condition called keratoconus. It's a disease in which the cornea (the outer surface of the eye in front of the pupil) becomes cone-shaped rather than spherical, and this results in extreme astigmatism. Glasses can't correct this vision problem, but hard contact lenses often can. Unfortunately, the distortion of the cornea makes it very difficult to fit contacts comfortably for a keratoconic my eye doctor described it as "trying to balance a saucer on the end of a football." Ultimately, the cornea may suffer scarring which results in vision loss that can't be corrected by contacts. At that point, the next step is to replace the damaged cornea with a new one through cornea transplant surgery. This is only necessary for about 10 percent of keratoconics, but I'm one of them. There's much more about that a bit further down this page... The purpose of this web site is to provide information and resources to those who have keratoconus, and particularly those who have had, or are considering, a cornea transplant.

Subscribing to the KC-Link Mailing List
The following information from the National Keratoconus Foundation describes how to subscribe to the KC-Link mailing list, which is an absolutely vital resource for any KC patient or supporter.
Keratoconus-Link
An Internet List for Keratoconus Patients and their Doctors
Dear potential subscriber: Keratoconus-link is an interactive forum for people with keratoconus worldwide. The link offers those with keratoconus a unique opportunity to share their "KC" experiences and concerns with others who can truly understand the frustrations of this condition. The camaraderie they share and support they offer each other is invaluable. In addition, a cadre of eye care practitioners -ophthalmologists, optometrists, and contact lens fitters - regularly participate. Keratoconus-link numbers over 800 participants from 30 countries the world over. E-mail messages, or "postings", are automatically sent to all subscribers on the list creating a worldwide keratoconus family. In addition to providing a forum for discussion of issues related to keratoconus, keratoconus-link provides information on other relevant Internet resources.

Launched in early 2000, patients in Los Angeles now have an opportunity to be part of cutting-edge clinical research into new devices and drugs to treat eye problems. Helping patients with eye disorders through their participation in quality research studies is the mission of the Ophthalmic Clinical Trials Center. Since its opening, the Clinical Trials Center has contracted with a number of ophthalmic companies to undertake research projects of great importance to the general public. Along with the Morris S. Pynoos Ophthalmology Research Laboratories at the University of California, Irvine, our team of researchers is paving the way for a unique relationship to unfold - the combination of basic science and clinical research to promote the understanding and management of eye disorders that currently lack effective treatment. Topics for future clinical trials include dry eyes, laser vision correction to prevent "lazy eye" in children, bifocal intraocular lenses, intraocular lenses to correct high myopia and hyperopia, and contact lenses and solutions. Revised: What is Keratoconus?
Keratoconus is an eye condition in which the shape of the cornea becomes distorted. The cornea is a clear structure that covers the front of the eye and allows light to enter the eye. In a healthy eye, the cornea curves like a dome. In an eye with keratoconus, the center of the cornea slowly thins and bulges, so that it sags and has a cone shape (see illustration). In keratoconus, the posterior layer may tear in rare cases. When this happens, the cornea may suddenly become swollen with water (termed hydrops).Wrinkles and scars may also form on a keratoconus cornea.
How does keratoconus interfere with vision?
People with keratoconus may lose vision in two ways: l. From distortion of the cornea: Seeing through a misshapen cornea is like taking pictures with a camera whose lens has an irregular (not smooth) surface. Parts of the picture or field of vision are in focus and parts are out of focus. This visual problem is called irregular astigmatism.

Feel free to contact us if you require information beyond what you find here or in the Keratoconus Links offered at the bottom of this page. For detailed information about keratoconus read Dr. Epstein's text in PDF format:
What is Keratoconus?
Keratoconus is a disorder that causes progressive thinning and distortion of the cornea - the clear dome at the front of the eye. The cornea normally has a rounded shape, but in keratoconus the thinned area bulges forward to produce a cone like protrusion. This results in progressive distortion and reduced vision. Many keratoconus patients experience blurred distance vision, glare, light sensitivity, disturbed night vision, headaches and reading problems, Keratoconus may affect only one eye but more commonly occurs in both eyes. It affects people of all races and both sexes. Most patients develop keratoconus in their late teens to early twenties although it can begin at any age. Pellucid marginal degeneration (PMD) is a corneal thinning disorder related to keratoconus that primarily affects the inferior cornea.
