Juvenile Rheumatoid Arthritis is a type of inflammatory arthritis that affects almost 200,000 children in the United States. JRA is a disease that causes painful, swollen, and stiff joints in children, most commonly in large joints like the knee. JRA has three well-defined subsets: a monoarticular form, which means that that the disease affects only one joint; a polyarticular form, which means that it affects many joints, and a systemic form, which means that it affects other organs in the body besides the joints. The systemic form of the disease is most often associated with high fevers and rash, in addition to arthritis. The polyarticular and systemic forms of the disease are the two types that commonly affect the cervical spine.
Symptoms
The signs and symptoms of JRA are caused by inflammation of the joints, which produces painful, swollen, and stiff joints. Over time, the inflammatory process can cause joint contractures and joint damage, which alters the normal growth of the joint. The polyarticular and systemic forms of JRA are the two types of the disease that most commonly affect the cervical spine. Approximately 50% of patients will complain of neck stiffness, but pain is a more unusual symptom. In general, less than 20% of patients with either of these two types of JRA will complain of neck pain, and neck symptoms are even more rare in patients with monoarticular JRA.

Juvenile rheumatoid arthritis
Juvenile rheumatoid arthritis is a form of arthritis that affects thousands of children. It is the most common form of arthritis in children. As many as half the children with this condition have a mild form and have few problems. Some forms of this disease can, however, produce long-term problems. Juvenile rheumatoid arthritis is related to abnormalities of the immune system. It can cause joint inflammation, damage joints, and alter growth. Like the adult forms of arthritis, arthritis in children share the symptoms of pain, swelling, stiffness, and loss of motion to joints throughout the body. Juvenile rheumatoid arthritis is different, however, from adult rheumatoid arthritis. It can start anytime after birth. It may be suspected in children who complain of joint pain and stiffness for more than 6 weeks. Onset and severity of symptoms varies between the different types of this form of arthritis. 3 Types of juvenile rheumatoid arthritis (JRA):
  • Pauciarticular JRA affects 4 or fewer joints and may cause eye inflammation. Likely to be affected are the knees, elbows, wrists, or ankles. This is the most common type, affecting as many as half of the children with JRA. Regular examinations by an ophthalmologist (a doctor who specializes in eye diseases) are very important for this type of arthritis. Many children with this type of JRA may outgrow the arthritis by adulthood; however, eye problems may continue and joint problems can return.

Treatment for juvenile rheumatoid arthritis
Polly Ferguson, M.D. History: Juvenile Rheumatoid Arthritis (JRA) is the most common rheumatic condition of childhood. There are three main categories of JRA: pauciarticular, polyarticular, and systemic. The three types differ in the number and distribution of joints involved as well as in the frequency of associated symptoms. The etiology of JRA is unknown, but it appears that a combination of genetic predisposition and environmental triggers are involved. New facts: Pauciarticular JRA predominantly affects young girls. By definition, four or fewer joints are involved. This group is more likely to have a positive anti-nuclear antibody test and to be at highest risk of developing chronic uveitis. Treatment for pauciarticular disease often involves the use of intraarticular corticosteroid injections, antimalarial drugs, methotrexate, and nonsteroidal anti-inflammatory drugs alone or in combination. Frequent ophthalmologic visits are a necessary part of the treatment plan. Most children have a good response to therapy and a good outcome. Children with polyarticular JRA have five or more joints inflamed. They typically have a more prolonged and severe course than the pauciarticular patients. Systemic-onset disease may present at any age; high-spiking fevers and a typical salmon-colored rash are the most common presenting features. Systemic symptoms may precede the development of arthritis by months to years. Polyarticular and systemic disease require treatment with a disease-modifying drug, typically methotrexate. Most children improve while taking methotrexate, but many continue to have active synovitis despite treatment.

Juvenile Rheumatoid Arthritis
Introduction Juvenile rheumatoid arthritis (JRA) is a condition of chronic synovitis in children of which there are several distinct subgroups (Table 1). The classic adult-type rheumatoid arthritis does occur in children but is quite rare, the JRA subgroups seronegative* polyarthritis and systemic-onset disease also occur in adults but not frequently, the pauciarthritis of early childhood has not been recognized in adults. Incidence and Epidemiology The true incidence of JRA is not known. Based on the estimate that approximately 5% of all cases of chronic so-called "rheumatoid" arthritis begin during childhood, there would be about 250,000 children who have JRA. No causes or risk factors have been identified for JRA. Antecedent causes that frequently are mentioned include infectious agents, immunologic abnormalities of the host, physical trauma to joints, psychological trauma to the child, and allergy. Pathophysiology The synovial tissue, or lining tissue of the joint, is the target for inflammation in JRA. Early results of synovial inflammation include hypertrophy.

Arthritis affects 1 out of 1000 children a year. One child in 10 thousand children will have a severe form of the disease. Arthritis in children is similar to arthritis in adults. It can start anytime after birth. We used to call all arthritis found in children, Juvenile Rheumatoid Arthritis. We now know that there are four different forms of arthritis affecting joints and connective tissues in children. They are:
* Polyarticular: Many small joints on both sides of the body may be involved. Fever, weight loss, morning stiffness and flu-like symptoms may occur. Joint stiffness and pain may get worse with strenuous activity. This is a chronic disease which may affect the lower spine.
* Pauciarticular (pronounced paw-see-ar-tic-u-lar) : This type most often affects girls from ages two to four. Symptoms include dry eyes, joint pain and swelling. This type often goes away after a short period of time.
* Systemic: This type occurs weeks to months after a high fever. A salmon colored rash occurs at night along with high fevers. Swelling of the liver, lymph nodes and spleen may occur.

What is going on in the body? JRA is an autoimmune disorder. This is a condition in which the body produces antibodies against its own tissue. The autoimmune response can attack the joints, eyes, heart, liver, and other body organs. What are the causes and risks of the disease? Juvenile rheumatoid arthritis is an autoimmune disorder . Many experts believe that this autoimmune response in children has two parts. They believe that some children have a genetic makeup that makes them vulnerable to the condition. When these children are exposed to a factor in the environment, such as a virus, it triggers the autoimmune response of JRA.

PACHMAN, MD ANDREW K. POZNANSKI, MD WE HAVE BEEN TAKING care of children with arthritis for the past two and a half decades. It has been a very rewarding experience to participate in the progress that has been made in the treatment of these children and the resultant improvement in function. The emergence of the discipline of pediatric rheumatology (first sub-speciality certification awarded in 1992) has been associated with an increased understanding of some of the factors that regulate the inflammatory process leading to the use of a wider variety of therapies. The combination of earlier, more aggressive intervention with more informative diagnostic tools (serologic and radiologic) has resulted in better patient outcomes. In this brief article, we review some of the symptoms of the child with juvenile rheumatoid arthritis, diagnostic tests that may be helpful (old and new) and summarize data concerning newer modes of therapy and their pitfalls.     This is a 4 year old female who has been limping with swelling of her right knee for several months. Her parents note that she cannot fully extend her right knee. She sometimes does not want to walk in the morning, but seems fine later in the morning and the rest of the day. Her past medical history is unremarkable. Physical examination demonstrates swelling (effusion) of her right knee, flexion contracture of 10 degrees and flexion to 120 degrees. No increased heat or pain upon range of motion is present. She appears unconcerned about her limp and swelling. Labs: WBC 8,600 with 45 polys, 47 lymphs, 8 monos. Hgb 12. ESR 20. UA normal. Rheumatoid factor negative, ANA 1:640 speckled. Juvenile rheumatoid arthritis (JRA) is a chronic arthritis of childhood comprised of several different subgroups. It is one of the most common rheumatic diseases of childhood. Because the majority of children are rheumatoid factor negative, it is also known as juvenile idiopathic arthritis or juvenile chronic arthritis. Although the true incidence and prevalence of JRA are unknown, its incidence is estimated at 2 to 20 per 100,000 children per year based on the American College of Rheumatology (ACR) criteria (1,2). Neither the etiology nor risk factors of JRA have been identified. It is considered to be an autoimmune disease. Chronic synovitis, T-cell abnormalities, abnormal immunoregulation and cytokine production, autoantibodies, immune complexes, and complement activation suggest that cell mediated and/or humoral processes are involved. Infection may have a possible role in developing JRA.
