The Facts about Juvenile Rheumatoid Arthritis
Juvenile rheumatoid arthritis is arthritis that causes joint inflammation and stiffness for more than 6 weeks in a child of 16 years of age or less. Inflammation causes redness, swelling, warmth, and soreness in the joints, although many children with JRA do not complain of joint pain. Any joint can be affected and inflammation may limit the mobility of affected joints. One type of JRA can also affect the internal organs. Doctors classify JRA into three types by the number of joints involved, the symptoms, and the presence or absence of certain proteins in the bloods. These classifications help the doctor determine how the disease will progress and whether the internal organs or skin are affected.
  • Pauciarticular means that four or fewer joints are affected. Pauciarticular is the most common form of JRA; about half of all children with JRA have this type. Pauciarticular disease typically affects large joints, such as the knees. Girls under age 8 are most likely to develop this type of JRA.

It occurs in 50-100 per 100,000 children in the United States. JRA is a general term for the most common types of arthritis in children. It is divided into several categories:
  • Systemic JRA: This form, which occurs in about 10% of cases, involves joint pain and swelling as well as fevers and rash. It is similar to Adult Still's Disease . The cause of this form of JRA is unknown. Polyarticular JRA: This form occurs in about 40% of cases and involves multiple joints that are painful and swollen. The cause of this form of JRA is unknown. Some children may have a positive rheumatoid factor and the condition may evolve into rheumatoid arthritis.

    What is juvenile rheumatoid arthritis?
    Juvenile rheumatoid arthritis (JRA) is the most common form of arthritis in children. As many as half of the children with this condition have a mild form and have few problems. However, some forms of this disease can produce long-term problems. There are three types of JRA:
    • Pauciarticular JRA affects 4 or fewer joints and may cause inflammation of the eye. The joints most likely to be affected are the knees, elbows, wrists, or ankles. This is the most common type of JRA. Half of the children with JRA have this type. Polyarticular JRA affects many joints (5 or more). About 30% of children with JRA have this type. The smaller joints like those in the hands and feet may be affected as well as large joints.

    Arthritis in childhood? Isn't that only an old person's disease?
    For both children with arthritis and the doctors who care for them public education is one of the greatest challenges. Most lay people and many doctors fail to realize that the problem exists. Many children suffer for months or years before the diagnosis of arthritis is thought of and proper treatment begun. But the problem doesn't end there. Children with arthritis frequently experience difficulty because their teachers and schoolmates don't believe children can get arthritis and have no idea what to expect from the child with arthritis or about the nature of the illness. As a result, when the child is finally diagnosed with arthritis the family may be told just to put them in a wheel chair because 'nothing can be done.' This is entirely wrong!!
