Background: Temporomandibular joint disorders (TMD) refer to a group of heterogeneous pain and dysfunction conditions involving the masticatory system, reducing life quality of the sufferers. Intra-articular injection of hyaluronate for TMD has been used for nearly two decades but the clinical effectiveness of the agent has not been summarized in the form of a systematic review. Objectives: To assess the effectiveness of intra-articular injection of hyaluronate both alone and in combination with other remedies on temporomandibular joint disorders. Search strategy: Intensive electronic and handsearches were carried out. The Oral Health Group's Trials Register (September 2001), The Cochrane Library CENTRAL database (Issue 3, 2001), MEDLINE (1966- May 2001), PubMed ( up to March 2002), EMBASE (1974 - August 2001), SIGLE (1980 - December 2001), CBMdisc (1983 - July 2001, in Chinese) and Chinese Medical Library were searched. All the Chinese professional journals in the oral health field were handsearched and conference proceedings consulted. There was no language restriction. Selection criteria:

It is not clear if occlusal adjustment is effective in treating TMD. Objectives: To assess the effectiveness of occlusal adjustment for treating TMD in adults and preventing TMD. Search strategy: We searched the Cochrane Oral Health Group's Trials Register (April 2002); the Cochrane Central Register of Controlled Trials (CENTRAL) (The Cochrane Library Issue 2, 2002); MEDLINE (1966 to 8th April 2002); EMBASE (1980 to 8th April 2002) and handsearched journals of particular importance to this review.Additional reports were identified from the reference lists of retrieved reports and from review articles of treating TMD. There were no language restrictions.Unpublished reports or abstracts were considered from the SIGLE database. Selection criteria: All randomised or quasi-randomised controlled trials (RCTs) comparing occlusal adjustment to placebo, reassurance or no treatment in adults with TMD. The outcomes were global measures of symptoms, pain, headache and limitation of movement.

Temporomandibular Joint Disorders
Temporomandibular joint disorder, which is also known as TMJ, is a painful condition that affects the joint connecting the jawbone to the skull. TMJ may also involve the surrounding muscles and soft tissues.
What is going on in the body?
Temporomandibular joint disorders are poorly understood. They are thought to involve a disturbance of the joint that connects the back of the jaw to the skull. This joint is located just in front of the ear and can be felt when the jaw opens and closes.
What are the signs and symptoms of the condition?
Temporomandibular joint disorders may cause a wide variety of complaints, such as: a clicking or snapping noise heard in the joint when the jaw is opened and closed headaches jaw pain joint locking open or shut limited ability to open the mouth pain behind the eye pain that spreads to the shoulders, neck, and back spasm or pain in the muscles used for chewing Certain other symptoms are often seen in people who have temporomandibular joint disorders. These symptoms may be part of the TMJ. Sometimes they may be caused by another condition that is also occurring at the same time. These symptoms include:

    Anchorage, AK (PRWEB) February 13, 2005 An article published in the January 30, 2005 issue of the Alaska Journal of Commerce described that the term temporomandibular joint disorders (TMD) is really one name used to identify two distinct diagnostic entities. Because of generic use of this term to describe disorders of the joints and surrounding musculature that connect the lower jaw to the temporal region at the sides of the skull, confusion and lack of understanding have been the norm for patients and medical practitioners alike. The article, written by Catherine Mormile, DPT, a private-practice, doctor of physical therapy practicing in Anchorage, Alaska, describes that TMD, a source of pain and dysfunction for nearly 10 million Americans and four times more women than men has a distinct muscular and joint presentation. One diagnosis, “joint” TMD presents itself with measurable orthopaedic damage to the cartilage and ligaments located within the TMJ itself. The other, “muscular” TMD, presents as a painful condition of the muscles involved in chewing and often involving the muscles of the scalp, base of the neck, and shoulders. Sometimes, both conditions occur simultaneously.

Initial treatment of temporomandibular joint disorders involves identifying actions that cause symptoms to get worse, such as clenching or grinding the teeth. Many people may not realize they are doing these things. Some other measures used to treat TMJ are as follows:

      Rheumatoid arthritis is the most common purely erosive inflammatory arthropathy. Rheumatoid arthritis is generally symmetric and affects proximal joints. There are no proliferative erosions as in erosive OA, psoriatic or Reiter's arthritides. Rheumatoid arthritis rarely includes any productive bone such as periostitis, enthesopathy, and osteophyte formation unless there is secondary osteoarthritis present. Rheumatoid factor is positive in 90-95% of the cases. Early in rheumatoid arthritis the RF may be falsely negative, and in older patients, the RF may be falsely positive. [MSK: The Requisites, 2nd edition, Manaster et al] The characteristic radiographic features of rheumatoid arthritis include soft-tissue swelling, periarticular osteoporosis, joint space narrowing, articular erosions, and marginal erosions. The marginal erosions are caused by the extension of pannus into synovial pockets overlying regions devoid of articular cartilage (which would normally protect against the toxic debris and inflammatory pannus in the synovial fluid).       Early RA will demonstrate erosions in the distal radioulnar joint, ulnar styloid, radial styloid, and triquetro-pisiform joint (the latter are best seen on ball-catcher's view). Typically, the next erosions and joint space narrowing will occur in the carpal bones and MCP joints. Ligamentous rupture can lead to wrist instability resulting in ulnar translocation of entire carpus, scapholunate instability, and dorsal and volar flexion carpal instability (DISI and VISI).
