Isaacs syndrome with laryngeal involvement: an unusual presentation of myokymia

A woman with widespread myokymia presented initially at age 19 with hoarseness and mild dyspnea on exertion. The diagnosis of Isaacs syndrome was suggested by the clinical findings of widespread continuous muscle activity and depressed tendon reflexes, although she lacked the usually prominent increased muscle tone. The diagnosis was confirmed by the electromyographic demonstration of continuous spontaneous muscle action potentials that were abolished by neuromuscular blockade but not by local nerve blockade. Isaacs Syndrome
A rare neuromuscular disorder with onset usually in late childhood or early adulthood, characterized by intermittent or continuous widespread involuntary muscle contraction s; FASCICULATION ; hyporeflexia; MUSCLE CRAMP MUSCLE WEAKNESS HYPERHIDROSIS TACHYCARDIA ; and MYOKYMIA . Involvement of pharyngeal or laryngeal muscles may interfere with speech and breathing. The continuous motor activity persists during sleep and general anesthesia (distinguishing this condition from STIFF-PERSON SYNDROME ). I have a friend who had just been told that she has Isaacs Syndrome, after being to several doctors that are in the neuromuscular specialities. She has been told 3 different diagnosis, from possible MD or MS, now this one. Abstract Neuromyotonia (NMT) is an uncommon syndrome of spontaneously occurring muscle activity of peripheral nerve origin, which can be triggered by voluntary or induced muscle contraction in which hyperexcitability of peripheral nerves leads to incapacitating muscle twitching, cramps, and weakness. Although NMT may sometimes accompany hereditary neuropathies, most cases are acquired (Isaacs syndrome) and most often as a paraneoplastic syndrome in patients with cancer of the immune system. Antibodies against voltage-gated potassium channels have been detected in some patients. We report two patients with NMT no associated to malignancies that developed acute renal failure while were under treatment with herbal medications by their traditional healer in the former Transkei (South Africa). One of those patients died due to acute renal complications and the other one got a complete recovery of her renal function and further relief of the NMT's manifestation with 1200 mg daily of Gabapentin orally.

Editorial Comment:
The author argues that the current tools for diagnosis and classification of stress urinary incontinence include history (to document the symptom), physical examination (to document the sign, that is by a stress test), voiding diary (to corroborate the symptom), some sort of pad test (to document the volume of urinary loss), leak point pressure (to quantitate sphincter strength) and a measure of urethral hypermobility. He states that all patients with stress incontinence have some element of sphincteric weakness, the degree of which is best measured by abdominal leak point pressure. The 3 different kinds of urethral hypermobility are described. He argues cogently in favor of putting aside the terms genuine stress incontinence and intrinsic sphincter deficiency, and instead defining stress incontinence by the parameters of leak point pressure and degree of hypermobility.
