I rritable bowel syndrome (IBS) is a benign, chronic symptom complex of altered bowel habits and abdominal pain. It is considered a functional type of bowel disorder. That is, no organic or structural cause can be detected (using currently available diagnostic modalities) to explain its symptoms. It is the most common functional disorder of the gastrointestinal (GI) tract. Other disorders in this group include functional dyspepsia, functional anorectal pain, and noncardiac chest pain. The many forms and presentations of IBS can make the diagnosis challenging, and its functional nature can make a satisfactory treatment regimen difficult. See page 1856 for definitions of strength-of-evidence levels contained in this article.

This trial uses a manualized acupuncture treatment format that closely follows clinical practice and allows flexibility in designing individualized treatments. In addition, a second parallel qualitative study will follow a subgroup of patients throughout the trial to explore the relationships between patients' interpretations and understandings (what anthropologists call "meaning") of irritable bowel and their response to treatment. Cortisol levels (an important stress hormone) will also be assessed. This will be a 3-arm trial: Active acupuncture, placebo acupuncture, and wait list.

Summary: Irritable Bowel Syndrome affects a large percentage of the population. Researchers in Sydney are looking for volunteers for a trial which aims to break down a vicious cycle between the irritable bowel and the brain. Below the diaphragm now to one of the commonest problems affecting Australians' intestines: Irritable Bowel Syndrome. It affects a large percentage of the population who experience recurrent abdominal discomfort, sometimes along with a sense of bloating and alterations in bowel habit, from loose motions to constipation in an otherwise well person who isn't losing weight.
Researchers in Sydney are looking for volunteers in a trial which aims to break what they think is a vicious cycle between the irritable bowel and the brain. It involves gastroenterologists and Professor of Psychological Medicine, Phillip Boyce.
Phillip Boyce: We don't really know what causes it, I mean there are some facts that are coming out. We do know that people with Irritable Bowel Syndrome have visceral hypersensitive, their guts are much more sensitive than normal people, particularly to pain or discomfort.
Norman Swan: So if you take two people, one of them who's hypersensitive and you give them the same stimulus, like eating carrots for example, one person will react to that, one person won't, that's because they're more sensitive.

Irritable bowel syndrome (IBS) also called irritable or spastic colon is a common functional intestinal disorder characterized by recurrent abdominal discomfort and abnormal bowel function. The discomfort often begins after eating and goes away after a bowel movement. The symptoms can include cramps, bloating, constipation, diarrhea, and a feeling of incomplete emptying. IBS occurs in about one in five Americans, more commonly in women, and more often at times of emotional stress. It usually begins in late adolescence or early adult life and rarely starts after the age of 50. In severe cases, it can result in missed work days and curtailment of social activities. Although effective help is available, many people with IBS are too embarrassed, pessimistic, or afraid to seek medical care. Even worse, some people who consult a doctor receive insufficient guidance and conclude that nothing further can be done for them.
Why Symptoms Occur
During normal digestion, foods are broken down in the stomach and small intestine so that their nutrients can be absorbed into the body. Undigested or partially digested portions mostly in liquid form then enter the large intestine (colon) where most of the water is reabsorbed. Movement through the intestines results from peristalsis, a wavelike contraction of muscles in the intestinal walls that propel their contents forward. When all is well, the end result is stool that is solid but soft enough to be excreted easily.

What Is Irritable Bowel Syndrome?
Irritable bowel syndrome (IBS) is a fairly common problem with the way the large intestine (say: in- tes -tin) works. The large intestine (also known as the colon) absorbs water and nutrients from the partially digested food that enters the colon from the small intestine. Anything that is not absorbed is slowly moved on a pathway out of your body. These undigested and unabsorbed food particles are also known as stool, a bowel movement, or poop. Here's why an intestine gets "irritable." To have a bowel movement, the muscles in the colon and the rest of the body have to work together. If this process is somehow interrupted, the contents of the colon can't move along very smoothly. It sort of stops and starts, doesn't move, or sometimes moves too fast. This can hurt and make a kid feel awful. Doctors also believe that people with IBS may have more sensitive bowels, so what might cause a little discomfort in one person causes serious pain for someone with IBS.

Irritable bowel syndrome (IBS) is a disorder that interferes with the normal functions of the large intestine (colon). It is characterized by a group of symptoms—crampy abdominal pain, bloating, constipation, and diarrhea. One in five Americans has IBS, making it one of the most common disorders diagnosed by doctors. It occurs more often in women than in men, and it usually begins around age 20. IBS causes a great deal of discomfort and distress, but it does not permanently harm the intestines and does not lead to intestinal bleeding or to any serious disease such as cancer. Most people can control their symptoms with diet, stress management, and medications prescribed by their physician. But for some people, IBS can be disabling. Mertz, M.D. Irritable bowel syndrome, a common disorder in which bowel habits are altered in association with abdominal pain or discomfort, has a prevalence of 12 percent among adults in the United States and a similar prevalence worldwide. By definition, no mechanical, biochemical, or overt inflammatory condition explains the symptoms. Validated, symptom-based criteria for the diagnosis of irritable bowel syndrome are highly predictive in the absence of alarming symptoms such as weight loss, fever, and intestinal bleeding. The pain or discomfort experienced by patients with irritable bowel syndrome often leads to health care use and a decreased quality of life. Irritable Bowel Syndrome is a functional bowel disorder of the gastrointestinal (GI) tract.
