What is irritable bowel syndrome?
Irritable bowel syndrome (IBS) is a common problem with the intestines. In people with IBS, the intestines squeeze too hard or not hard enough and cause food to move too quickly or too slowly through the intestines. IBS usually begins around age 20 and is more common in women. IBS is also called functional bowel syndrome, irritable colon (the large intestine is also called the colon), spastic bowel and spastic colon. It's not the same as inflammatory bowel diseases like ulcerative colitis. Return to top

By Carol Lewis It's the most common gastrointestinal disorder seen by doctors. Abdominal pain, cramps, gas, bloating, diarrhea and constipation are among the symptoms. For years, many thought it was a psychological condition, rather than a physical one. But the discomfort and inconvenience that result from the gut-wrenching pain of irritable bowel syndrome (IBS) are very real. "It feels like you're giving birth over and over again," says Hope Marcus of Miami, Fla. Indeed, people with IBS can't stay out of the bathroom or their stomachs feel like they're tied in knots. Their bowel movements are either frequent and runny or sporadic and unusually hard. Mike Hoffman of West Palm Beach, Fla., says his intestines feel like they're constantly twisting and untwisting, "as though they're being wrung out like a dishrag." Jeffrey Roberts of Hallandale, Fla., says the sudden recurring knife-like pain "doubles you over," while others claim it brings you to your knees. Some liken the pain to a charley horse, but in the gutpain that is horrendous and then intensifies. "And don't picture a diarrhea attack happening in the privacy of your own clean bathroom," says Cynthia Huschle of Tolland, Conn. "Picture it in the bathroom at work, the store, the movie theater, the park, the baseball stadium, or the port-o-potty at the fair."

Irritable Bowel Syndrome
Irritable bowel syndrome (IBS) is a condition which causes the intestines (or bowels, or gut) to be over sensitive. This can cause a variety of abdominal and bowel symptoms. There is no cure, but most people can, with the help of their doctor, learn to control and live with their irritable bowel.
Symptoms
  • Abdominal pains, especially on the left side, or across the lower abdomen.
  • Bloating and fullness of wind.
  • Constipation
  • Diarrhoea.
  • Alternating between one and the other.
  • A sensation of having to rush to the toilet.
  • Difficulty opening the bowels, or feeling as though they haven't emptied adequately.
  • Feeling sick with it.
These are the main symptoms that occur, and it seems that the bowel is more sensitive to outside events. Thus, in the way that someone with sensitive skin comes out in a rash when stressful events occur, a person with irritable bowel syndrome finds that when they are stressed it is their bowels that "come out in sympathy". This means that symptoms may be worse when under psychological stress or when being physically stressed eg by an infection, exhaustion or other conditions. The problem may also irritated by certain foods and drinks (which vary from person to person).

New hope for those with irritable bowel syndrome
By Rhonda Rowland CNN Medical Unit ATLANTA, Georgia (CNN) The first effective medication for women with a type of irritable bowel syndrome (IBS) could be on the market soon. The drug, called Zelnorm, improves a number of symptoms and is safe according to researchers attending the Digestive Disease Week conference in Atlanta. "When taken twice a day we saw relief of abdominal pain and discomfort, bloating and constipation," said Dr. Martin Lefkowitz with Novartis Pharma, the drug's maker. Zelnorm is a new type of medication that activates or stimulates the serotonin 4 receptor. RESOURCES Learn more about irritable bowel syndrome "The drug increases movement, so it helps those with irritable bowel syndrome characterized by constipation," said Dr. Christine Frissora, who reviewed the safety and efficacy data on Zelnorm at the Weill Medical College of Cornell University. "If you don't give the drug to the right patient, it could make their condition worse." It's estimated that 15 million Americans suffer from IBS. In some cases the symptoms are so debilitating that patients are unable to work, socialize and in some cases are afraid or embarrassed to leave their homes. One-third of the cases are characterized by constipation, another third by diarrhea and the other third suffer from both. Zelnorm is designed to help those with IBS characterized by constipation.

LOS ANGELES, California (Reuters) Irritable bowel syndrome, a chronic condition believed to plague 20 percent of the adult population in developed countries, may be caused by too much bacteria in the small intestine, researchers said Wednesday. Irritable bowel syndrome (IBS) is one of the most common ailments of the bowel (intestines) and affects an estimated 15% of persons in the US. The term, irritable bowel, is not a particularly good one since it implies that the bowel is responding irritably to normal stimuli, and this may or may not be the case. The several names for IBS, including spastic colon, spastic colitis, and mucous colitis, attest to the difficulty of getting a descriptive handle on the ailment. Moreover, each of the other names is itself as problematic as the term IBS. IBS is best described as a functional disease. The concept of functional disease is particularly useful when discussing diseases of the gastrointestinal tract. The concept applies to the muscular organs of the gastrointestinal tract; the esophagus, stomach, small intestine, gallbladder, and colon. What is meant by the term, functional, is that either the muscles of the organs or the nerves that control the organs are not working normally, and, as a result, the organs do not function normally. The nerves that control the organs include not only the nerves that lie within the muscles of the organs but also the nerves of the spinal cord and brain.

  • History . Patients often describe a long history of chronic or intermittent diarrhea, which usually starts before age 50 and is exacerbated by anxiety or stress. Diarrhea is often worse in the morning and after meals. These patients often complain of a sensation of incomplete evacuation, distention, passage of mucus, or associated abdominal, pelvic, and back pain. Pain is relieved by evacuation. Alternating constipation, diarrhea, and bloating is typical. Diarrhea is not bloody unless accompanied by an anorectal lesion such as hemorrhoids or a fissure. Systemic symptoms or weight loss are not present. Nutritional status is not compromised. Proposed criteria for diagnosis of IBS are found in Box 5-1 Etiology . The following factors are believed to play a mechanistic role in IBS and more than one may operate in a given individual: (1) abnormal motility, (2) abnormal visceral perception, (3) psychologic factors, (4) luminal compounds (lactose, short chain fatty acids, food allergens, and bile acids), which may irritate the bowel.
