Uveitis
Overview Symptoms Treatment Prevention Definition: An inflammation of the uveal tract which includes the iris ciliary body , and the choroid of the eye.

Causes, incidence, and risk factors: Uveitis affects the uvea , the layer between the sclera and the retina , which also includes the iris , the ciliary body , and the choroid . The uvea is very vascular and provides most of the blood supply to the retina. Causes of uveitis can include autoimmunity, infection, or toxin exposure. The cause often remains unknown.
The most common form of uveitis is anterior uveitis, which involves inflammation of the iris. This condition is often called "iritis." The inflammation may be associated with autoimmune diseases such as rheumatoid arthritis or ankylosing spondylitis , but most cases have no other diseases associated. The disorder may affect only one eye and is most common in young and middle-aged people. A history of an autoimmune disease is a risk factor. Pars planitis is inflammation of the pars plana, a narrow area of the choroid in between the iris and the choroid. This usually occurs in young men and is not associated with any other disease. It is usually mild in severity.

INFO -Iritis- What is Iritis? Iritis, also known as Uveitis, is the inflammation of the iris. The iris is the colored portion of the eye. What causes Iritis? In the majority of cases there is no specific cause. Occasionally, iritis is just on symptom of a disease that affects other organ systems. These are diseases of the connective tissue/autoimmune diseases. These diseases include: rheumatoid arthritis, sarcoid, lupus, scleroderma, anklylosing spondylitis, Chrohn’s disease, Behcet’s disease, and ulcerative colitis. Sometimes, it is necessary to establish whether iritis is a manifestation (symptom) of one of these diseases or some other underlying disease. The doctor may ask you for a detailed health history, and may order a series of tests including several possible blood tests, a chest x-ray, x-rays of the lower back and possibly a skin test for tuberculosis. While iritis sometimes occurs only once, most cases are recurring. Once treated the attack will usually respond to various medications. Iritis is the inflammation of the iris. If this is associated with inflammation of the choroid, it is called as uveitis (pronounced as you-v-i-tis). Many times retina also gets infected, termed as chorio-retinitis (Pronounced as ko-rio -retinitis) Causes Infections like Tuberculosis, Leprosy, Herpes virus infection, AIDS, Toxoplasmosis etc. Non- infectious causes like eye injury, Sarcoidoisis, Ankylosing Spondylitis, Juvenile Rheumatoid Arthritis etc. How do I know it is Iritis? Besides redness, there will be pain in the eye, watering, photophobia (inability to withstand day-light), decreased vision . This tells you that it is not likely to be just Conjunctivitis. To distinguish it from glaucoma, you will have to go to an eye-doctor and get your eye examined. He will take the eye pressure, examine you on the slit-lamp and then will make the diagnosis.
Management
Tests are required to know the cause so, treatment can be given accordingly.

I am a new mother who is currently fighting my second bout of iritis. I am hla-b27 positive with ankylosing spondylitis. The iritis has not responded to 4 weeks treatment with Pred Forte, Voltaren, and cyclogyl. My ophthalmalogist gave me a choice of an injection of steroids into the lower lid of my eye, or oral prednisone, 40 mg per day. I chose the injection, because I am breastfeeding my child. The injection has had very little effect during the last week, and my ophthalmalogist wants to discuss the possibility of the oral prednisone. My question is this; what is considered a safe level of oral steroids for a lactating mother to take, and how long a course of treatment is considered safe? Thanks.
Well, as you probably know, there are not many studies with any drugs in pregnant or lactating mothers. It is likely that some of the drug will pass through the breast milk. Although iritis associated with ankylosing spondylitis can be difficult to treat it usually responds to topical or injected steroids. My first advice would be to see a uveitis specialist because they specialize in this area. They would be more familiarwith treating difficult cases of iritis. In general, I would consider repeating the injection one more time before considering oral steroids. Good luck! This information was provided for medical educational purposes only.

Iritis usually causes pain and redness of the eye, the eye may be very light sensitive, too. As one looks at near with the inflamed eye, it may hurt badly. When we look at near distance the small muscles of the iris contract. Contraction of the inflammed muscle causes the pain. In young children iritis is often quite symptom free when it occurs with inflammation of a joint, a rheumatoid reaction. Also the swelling of the joint may have developed unnoticed, so the child may come to an ophthalmologist because of decrease of vision, caused by cataractous changes in the lens. In such a case inflammation must have been present for weeks or months. Iritis should be treated as soon as possible. It is best to go to the emergency clinic or to the eye doctor's office if you can go there without delay. It is important not to make an appointment which usually means several weeks of waiting. The treatment of iritis is based on rest and steroids , cortison-like medications. Rest is instituted by using

Spritzing the Fire I hate iritis. For those of you unfamiliar with this malady, it's an inflamation of the iris that causes an achy or scratchy feeling in the eye (eventually developing into extreme pain if untreated), blurriness of vision, and sensitivity to light.
Most times they have no idea what causes it it's not a response to an infection, like conjunctivitis but the few cases with clearly identifiable causes link it to autoimmune disorders like lupus or latent viral infections like herpes. They don't know what is causing mine, and no bloodwork is likely unless it recurs (most cases are one-time deals fingers crossed!).
