Biomarkers For Interstitial Cystitis Identified, Could Lead To The First Test
SAN ANTONIO, May 21 University of Pittsburgh researchers have isolated two biomarkers for interstitial cystitis (IC), a chronic and painful pelvic disease for which there currently is no test. The discovery of these biomarkers could lead to a definitive test for IC and have the potential to lead to new therapies. Results of two studies are being presented today at the annual meeting of the American Urological Association (AUA) in San Antonio, and are published in abstracts 69 and 80 of the AUA proceedings. This clinical trial performs two types of acupuncture on traditional 'bladder' points to determine if one type demonstrates improvement over the other. In the first type needles stimulate acupuncture points on the skin, and in the second type the needles penetrate through the skin similar to how acupuncture is usually performed. The technique is done through shields so that people will not know what type of acupuncture they receive. Patients must have at least moderately severe disease as determined by a urologist. They must be willing not to change their medical regimen for the duration of the trial. Patients will receive twelve treatments over six to twelve weeks. They will be asked to fill out symptom questionnaires and bladder diaries at regular intervals to monitor change. All treatment is free of charge, and patients who complete the trial will receive a small stipend.

Treating interstitial cystitis
Interstitial cystitis is a chronic inflammation of the bladder that causes people to urinate sometimes painfully as often as 40, 50, or 60 times a day. Their quality of life, research suggests, resembles that of a person on kidney dialysis or suffering from chronic cancer pain. Not surprisingly, the condition is officially recognized as a disability. There's no cure for interstitial cystitis, but many treatments offer some relief, either on their own or in combination. Treatment (see chart) is aimed at relieving pain and reducing inflammation. The two main approaches are oral medications and bladder instillations drugs that are introduced into the bladder by catheter and held for 15 minutes. The procedure usually takes place in a physician's office, but in some cases these drugs can be self-administered at home.
Common treatments for interstitial cystitis
Treatment Comment Oral drugs Tricyclic antidepressants Taken at low doses, tricyclic antidepressants relax the bladder and interfere with the release of neurochemicals that can cause bladder pain and inflammation. They may also improve sleep. Amitriptyline is the medication most commonly prescribed for interstitial cystitis.

Abstract Interstitial cystitis (IC) is a chronic pain syndrome that affects close to a million people in the United States. The syndrome presents differently in many individuals, with the unifying factor being chronic pelvic pain and disruption of daily life activities. Many etiologies have been proposed as causative factors for IC, although it is likely triggered by more than one process. Treatment for many individuals revolves around symptom management and improving quality of life; however, it is imperative to remove aggravating factors such as food and daily stressors. What is interstitial cystitis (IC)?
Interstitial cystitis (IC) is complex, chronic disorder characterized by an inflamed or irritated bladder wall. It can lead to scarring and stiffening of the bladder, decreased bladder capacity, and glomerulations (pinpoint bleeding). IC may also be known as:
  • painful bladder syndrome frequency-urgency-dysuria syndrome
An estimated 700,000 people have IC, and about 90 percent of them are women. People of any age can be affected with IC, although it is rare in children.
What causes IC?
The cause of IC is still unknown and patients do not respond to antibiotic medication. Researchers are investigating many theories to understand the causes of IC and to determine appropriate treatments.
What are the symptoms of IC?
The following are the most common symptoms of IC. However, each individual may experience symptoms differently. Symptoms may include:

The discovery of these biomarkers could lead to a definitive test for IC and have the potential to lead to new therapies. Results of two studies are being presented today at the annual meeting of the American Urological Association (AUA) in San Antonio, and are published in abstracts 69 and 80 of the AUA proceedings. "IC is a frustrating disease for patients because, to this point, there is no accurate way of diagnosing the condition. Patients undergo a variety of tests to rule out other diseases, all while experiencing significant pain and discomfort. Only after these tests come back negative, can a doctor make the diagnosis of IC," said Michael Chancellor, M.D., professor, department of urology, University of Pittsburgh School of Medicine. "Finding a marker for IC can not only make developing an early test for IC possible, but it can lead to new targeted molecular therapies for the condition," said Fernando de Miguel, Ph.D., assistant professor of urology at Pitt's School of Medicine.
