Overview of IC
ICA
What causes IC?
Figure 1: 3D structure of glycosaminoglycan molecule heparin. Provided by PDB In addition to the above structure, go to the online version of Metts 2001 and check out the diagram of the bladder with labled mucin layer. (I was unable to recieve permission to use this figure on my webpage). While you're there check out the cystoscopy photographs. Activated mast cells in the bladder lining cause damage by releasing inflamatory molecules and additionally by releasing proteases. Theoharides (1998) discuss the role that lymphatic cells can play in activating neurons to relase neurotransmitters. Apparently certain neurotransmitters (ACh and Sp) can further activate mast cells. Furthermore, bladder mast cells have estrogen receptors and the presence of estrogen increases their activation (perhaps explaining the increased pain during ovulation and menstruation). The final conclusion of Theohardies is that stress can play a role in activating blader mast cells (they showed this possiblity in a rat model). Therefore we see that immune, nueral, and endocrine factors affect IC. The above information regarding the causes of IC leaves one feeling perhaps a bit confused and definitly more than a little disapointed. While we have come a long way from lumping IC in with 'female hysteria', we are still a long way from assigning specific causations of the disease. From available evidence it appears that an autoimmune causation is most likely, but we must remember that there are likely mulitple events that can lead to IC. It is also important to remember that as researchers we are upset by the lack of evidence, yet it is the patients diagnosed with IC that are really left in dark with regards to this disease.

It hurts before and after. I have been to the doctors and I do not have an STD or UTI. It has got to the point that it wakes me up out of a dead sleep. It feels like a knife is stabbing me, and I can't sit, lay down, or move at all. I was wondering if you knew what else it could be. Thanks. Dear Reader, What you describe may be a chronic inflammatory condition of the bladder called interstitial cystitis (IC). Symptoms of IC vary from case to case, but often include mild discomfort, pressure, tenderness, or even intense pain in the bladder and surrounding area. People, usually women, who have IC may also experience a frequent and urgent need to urinate. A relatively newly recognized disorder, IC has been a diagnosis for only about 20 years now, and is difficult to diagnose correctly. Because symptoms vary so widely, many scientists believe IC is a syndrome rather than a specific disease. Most scientists also do not believe that interstitial cystitis is caused by bacteria, and there are many theories on what does cause IC. We do know, however, that IC is neither a psychosomatic nor a stress-related disorder. In this case, it is important to see a urologist, who would have to diagnose IC through a process of ruling out other possibilities. An extensive list of other, similar disorders that need to be checked off the list before a diagnosis of IC can be made include bacterial cystitis (urinary tract infection), bladder cancer, kidney problems and kidney stones, tuberculosis, vaginal infections, sexually transmitted infections (STIs), endometriosis, radiation cystitis, and neurological disorders. A urologist may also perform a cystoscopy (an examination of the bladder with a tube) to check for hemorrhages on the bladder wall. These occur in about 95 percent of patients with IC.

    What is interstitial cystitis (IC)?
Interstitial cystitis (IC) is complex, chronic disorder characterized by an inflamed or irritated bladder wall. It can lead to scarring and stiffening of the bladder, decreased bladder capacity, and glomerulations (pinpoint bleeding). IC may also be known as:
    • painful bladder syndrome frequency-urgency-dysuria syndrome
    What causes IC?
The cause of IC is still unknown and patients do not respond to antibiotic medication. Researchers are investigating many theories to understand the causes of IC and to determine appropriate treatments.
What are the symptoms of IC?
The following are the most common symptoms of IC. However, each individual may experience symptoms differently. Symptoms may include:
    • frequent urination feelings of pressure, pain, and tenderness around the bladder, pelvis, and perineum (the area between the anus and vagina or anus and scrotum) painful sexual intercourse in men, discomfort or pain in the penis and scrotum

    • Interstitial cystitis (IC) is a chronic inflammatory condition of the bladder wall that frequently goes undiagnosed. The cause of IC is unknown. No treatment is uniformly effective for patients with IC. Although many treatments options are available, only one treatment (Elmiron) is specifically FDA-approved to treat the symptoms of IC. The majority of patients who are properly diagnosed and treated obtain relief. Symptoms of IC:  bladder pain, urinary urgency, day and night frequency (up to 60 times per day), suprapubic (lower abdominal) or perineal (area between scrotum and anus in men, or vagina and anus in women) pain and pressure. Sexual intercourse can be painful. More than 700,000 Americans have IC. Average age of onset is 40. Twenty-five percent of patients with IC are under the age of 30. Ninety percent of IC patients are women, however preliminary studies of men with nonbacterial prostatitis indicate that they may actually have IC as well.

How can you tell if you have interstitial cystitis?
There are no specific blood or urine tests that firmly tell your health care practitioner whether you have it or not. The diagnosis is often made after ofther diseases have been excluded. This could mean a long course of tests, doctors and frustration. The three most common symptoms of interstitial cystitis are: * frequency of urination * urgency to urinate * pelvic pain that lessens with bladder emptying Other symptoms include: * pelvic pain that worsens one week prior to menstruation * constipation or irritable bowel syndrome * slow urinary stream * pain after sexual intercourse up to a day later * depression * pain at the tip of the penis, the groin, or testicles

Definition: Interstitial cystitis is a bladder condition caused by chronic inflammation, leading to difficulties with urination.
    Alternative Names: Cystitis - interstitial
Causes, incidence, and risk factors: Interstitial cystitis (IC) is an inflammation of the tissues of the bladder wall, with no known infectious (bacterial, viral, or fungal) cause. The condition is identified through a diagnosis of exclusion. Women are 10 times more likely to be affected than men. The condition is more common in Jewish women and is rare in African-Americans. IC generally occurs during the third or fourth decades of life. For the average person with interstitial cystitis, there is a 4-year delay between the time the first symptoms occur and the diagnosis. This demonstrates both the difficulty of diagnosis and how infrequently this condition is suspected.

Interstitial Cystitis Often Difficult to Diagnose, Treat
    Q:
I'm a 37-year-old woman with a frustrating problem that has only been recently diagnosed. For years I've experienced urinary frequency, urgency and lower abdominal pain. After visits to several doctors over many years, I've been told I have interstitial cystitis and have started on a series of treatments that are showing some promise. What is interstitial cystitis and why is it so hard to diagnose and treat? A: Your experience is not an uncommon one. Interstitial cystitis, or IC, is often difficult to diagnose and to treat satisfactorily. According to Russell Lawson, MD, former professor and chief of urology at the Medical College of Wisconsin, IC should be suspected when a patient has problems that mimic a urinary tract infection such as burning, frequency and urgency but with negative urine cultures and no response to antibiotic therapy. At that point your physician should consider the possibility of IC and send you to a urologist for consultation. Urologists often use a procedure called cystoscopy to visualize the detrusor muscle and bladder wall. The detrusor muscle helps to empty the bladder and a cystoscopy will help to evaluate its current level of functioning. Key findings include the discovery of inflammation in the detrusor and a reduced bladder capacity.

INTERSTITIAL CYSTITIS The urinary system consists of the kidneys, ureters, bladder, and urethra. The kidneys, a pair of purplish-brown organs, are located below the ribs toward the middle of the back. The kidneys remove liquid waste from the blood in the form of urine, keep a stable balance of salts and other substances in the blood, and produce erythropoietin, a hormone that aids the formation of red blood cells. Narrow tubes called ureters carry urine from the kidneys to the bladder, a triangle-shaped chamber in the lower abdomen. Like a balloon, the bladder's elastic walls relax and expand to store urine and contract and flatten when urine is emptied through the urethra. The typical adult bladder can store about 1 1/2 cups of urine. Adults pass about a quart and a half of urine each day. The amount of urine varies, depending on the fluids and foods a person consumes. The volume formed at night is about half that formed in the daytime. Normal urine is sterile. It contains fluids, salts and waste products, but it is free of bacteria, viruses and fungi. The tissues of the bladder are isolated from urine and toxic substances by a coating that discourages bacteria from attaching and growing on the bladder wall. People with interstitial cystitis (IC) have an inflamed, or irritated, bladder wall. This inflammation can lead to scarring and stiffening of the bladder, decreased bladder capacity, glomerulations (pinpoint bleeding) and, in rare cases, ulcers in the bladder lining.

You have to urinate much more often than other people, even on days when you don't drink that much water; sometimes you feel you won't make it to the bathroom in time. Your doctor tells you that you have interstitial cystitis, a disease that affects between 450,000 and 700,000 people in the United States. An often misunderstood condition that is easily confused with other diseases, it wasn't recognized by doctors until about 20 years ago. What is interstitial cystitis?
An inflammation of the bladder wall, interstitial cystitis is thought to result from the breakdown of a layer of protein that normally protects the bladder lining from toxins in urine. The toxins irritate the bladder wall, which bleeds and can become scarred and stiff. At this stage, the bladder can't expand normally and can't hold as much urine as it used to. What causes interstitial cystitis?

    Interstitial Cystitis
Allergy causes soft-tissue swelling and irritation. It causes irritation of mucus membrane, in the eyes, ears, nose and throat, and in the urinary tract. When there is irritation in the throat, there is disruption of the tissue integrity and severe irritation may result. The most obvious example is the chronic "sore throat" that many allergy suffers experience. The constant mucus drainage and resultant histamine release result in tiny disruptions in the protective surface layers of the mucus membrane lining the throat. This disruption makes the sensitive subsurface layers susceptible to irritants such as spices or acid foods like hot sauce and orange juice. And that leads to even more irritation and even hemorrhagic tissue changes (bleeding). This disruption also allows the ordinarily harmless bacteria that are necessary for digestion to set up local infections in the irritated tissues.

Alternate Names : Cystitis - Interstitial Interstitial Cystitis (IC): Treatment There are no standard or consistently effective treatments for interstitial cystitis. Results vary from individual to individual. As long as the cause of IC is unknown, treatment is based on trial and error until relief is found.
Some of the treatments that have been tried include:
    • Dilation of the bladder with fluid (hydrodistention) Oral medications:
      • Elmiron (coats bladder like Pepto-Bismol) Opiates (Nalmefene) Psychiatric medications like Elavil or Vistarul (may have a pain-relieving effect, also helpful for psychiatric conditions like depression which often accompany this disorder)
