P atients with irritative voiding symptoms and negative urine cultures frequently present a diagnostic challenge for primary care physicians. Interstitial cystitis is a severely debilitating disease of the urinary bladder. Symptoms of interstitial cystitis include excessive urgency and frequency of urination, suprapubic pain, dyspareunia and chronic pelvic pain. Interstitial cystitis interferes with employment, social relationships and sexual activity. The course of the disease is usually marked by flare-ups and remissions. Interstitial cystitis is a chronic disease that persists throughout the patient's life. On clinical evaluation, patients with interstitial cystitis should have no other definable pathology, such as urinary infections, carcinoma, or radiation-induced or medication-induced cystitis (Parsons CL. Interstitial cystitis, new concepts in pathogenesis, diagnosis and management. Presentation at 3rd annual meeting of the American Urological Association, June 4, 1998, San Diego, Calif.).

Interstitial Cystitis: A Bladder Problem
What is interstitial cystitis? Interstitial cystitis is a chronic bladder problem. About 750,000 Americans have interstitial cystitis. Most of them are women. People with interstitial cystitis have a bladder wall that is inflamed and irritated (red and sore). This inflammation can scar the bladder or make it stiff. A stiff bladder can't expand as urine fills it. There may be pinpoint bleeding from the walls of the bladder. A few people get sores in the bladder lining. People with interstitial cystitis may have many of the following symptoms:
  • An urgent need to urinate, both in the daytime and during the night

Introduction: Interstitial cystitis
Interstitial cystitis: Interstitial cystitis is an odd disease that is difficult to diagnose. It causes pain and irritation to the bladder and pelvic area, and thereby causes various urination symptoms. Its cause is unclear, but may be autoimmune, or perhaps only some cases are autoimmune. Diagnosis of IC is often by ruling out all other possible causes of bladder symptoms. Confirmation of a diagnosis is difficult, and the most compelling evidence for diagnosis is often from surgery and biopsy, rather than any specific urine or blood tests. Interstitial cystitis: Interstitial cystitis (IC), one of the chronic pelvic pain disorders, is a condition resulting in recurring discomfort or pain in the bladder and the surrounding pelvic region.

Transcript: Norman Swan: Now your letters are always welcome here on the Health Report and one particularly sparked my interest recently. It came from Diane Turner in Warragul, Victoria who asked us to cover the hidden, painful and debilitating problem of interstitial cystitis. Since it primarily affects women (and isn't by the way to be confused with normal cystitis), I went to Dr Helen O'Connell. Helen is at the Royal Melbourne Hospital and is the only female urologist in Australasia. Dr O'Connell: Interstitial cystitis is a condition where the patients predominant symptom is bladder related discomfort. It's a non malignant, non infective condition which may be associated with changes that are apparent when you look at the bladder, but sometimes the bladder may appear absolutely normal, that the patient may be crippled with discomfort. Norman Swan: And what sort of discomfort do they complain of. What are the typical range of stories?

This is a chronic painful bladder disorder and researchers in the U.S. have made some fascinating findings which may make it easier to understand the condition. Program Transcript Norman Swan: Let’s stay with cystitis but of another kind and one which can be extremely disabling and many would say incurable. It’s called interstitial cystitis and is under researched and probably under diagnosed. It’s said that it can take years for a woman to discover the reason for her symptoms. One person who does study interstitial cystitis is Susan Keay – she’s Professor of Medicine at the University of Maryland School of Medicine in Baltimore. She and her colleagues have made some fascinating findings which could unlock this condition’s secrets. Susan Keay: The typical story is someone who notices an onset of symptoms very much like a urinary tract infection which is pain, increased urgency and increased frequency of urination. But when this patient goes to their doctor, the doctor often doesn’t find any evidence of a urinary tract infection and not only does he not find evidence even if they try empiric antibiotic therapy generally speaking it’s not helpful and it never is curative as far as we can tell, even if it does help alleviate the symptoms slightly. So that’s part of the difference between interstitial cystitis and a urinary tract infection. The other difference is when it presents these women rather than having pain as they’re passing their urine which is typically what we find for a urinary tract infection, urination actually relieves the pain, it’s bladder filling that causes the pain in patients with interstitial cystitis.

What is IC / PBS?
Interstitial cystitis (IC) is a condition that results in recurring discomfort or pain in the bladder and the surrounding pelvic region. The symptoms vary from case to case and even in the same individual. People may experience mild discomfort, pressure, tenderness, or intense pain in the bladder and pelvic area. Symptoms may include an urgent need to urinate (urgency), a frequent need to urinate (frequency), or a combination of these symptoms. Pain may change in intensity as the bladder fills with urine or as it empties. Women's symptoms often get worse during menstruation. They may sometimes experience pain with vaginal intercourse. Because IC varies so much in symptoms and severity, most researchers believe that it is not one, but several diseases. In recent years, scientists have started to use the term painful bladder syndrome (PBS) to describe cases with painful urinary symptoms that may not meet the strictest definition of IC. The term IC / PBS includes all cases of urinary pain that can't be attributed to other causes, such as infection or urinary stones. The term interstitial cystitis, or IC, is used alone when describing cases that meet all of the IC criteria established by the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK).

What is interstitial cystitis (IC)?
The urinary system consists of the kidneys, ureters, bladder, and urethra. The kidneys, a pair of purplish-brown organs, are located below the ribs toward the middle of the back. The kidneys remove liquid waste from the blood in the form of urine, keep a stable balance of salts and other substances in the blood, and produce erythropoietin , a hormone that aids the formation of red blood cells. Narrow tubes called ureters carry urine from the kidneys to the bladder, a triangle-shaped chamber in the lower abdomen. Like a balloon, the bladder's elastic walls relax and expand to store urine and contract and flatten when urine is emptied through the urethra. The typical adult bladder can store about 1 1/2 cups of urine. Adults pass about a quart and a half of urine each day. The amount of urine varies, depending on the fluids and foods a person consumes. The volume formed at night is about half that formed in the daytime.

Welcome to The Angry Bladder . This site is partly my personal ongoing story on living with Interstitial Cystitis and partly a resource for others who also suffer from this annoying and debilitating disease. The name pretty much speaks for itself - I am angry that there is no cure for this yet. I am angry that many doctors still don't know or won't admit that IC exists. I am angry that I got stuck with a disease that makes it hard to live a "normal" life.
What you will find on this site is lots of opinion - this site it not sponsored by anyone so I have no qualms telling the skinny on things that have or have not worked for me. Do not get discouraged by my results. IC is very personal and everyone reacts differently. What doesn't work for me may work for you and vice versa.
What you won't find on this site is a bunch of "weeping" or sad women graphics. I am attempting to show that it is possible to still retain your sense of humor even while dealing with pain or discomfort on a daily basis. There are no victims here.

May 3rd, 2005 After many years of culturing and testing patients Dr. Fugazzotto announces that he will no longer be able to provide these tests. He recommends that you read the "Diagnosis and Monitoring" page to find other locations that provide similar testing This site is dedicated to helping Interstitial Cystitis patients, or those who suspect they may have IC, or those having chronic bladder problems such as pelvic pain, urinary frequency, burning, urethral symptoms, or urgency and pressure.
Dr. Paul Fugazzotto The intestinal bacilli recovered in the conventional colony count method of culture and diagnosis mature six times faster than the infecting COCCUS strains which are also present in the cases of cystitis. These bacilli mask the appearance of the cocci on the agar plates. The broth culture screen exposes all of the damaging elements present in the patient's body. From this culture the laboratory personnel can separate out the index organisms for appropriate remedy. The specific technology for this requires no less than four (4) days of processing by personnel academically experienced in pathogenic microbiology. All medical labs are equipped and capable of performing the above testing. Dr. Fugazzotto wishes to point out that two labs in Northern VA do an approximated version of this test, but this should not imply that they duplicate his procedures exactly. Dr. Fugazzotto has written down his procedures and this protocol is available from him upon request.
