Insulin Resistance
Insulin resistance is often associated with abdominal obesity, is a factor of the metabolic syndrome and is implicated in the development of atherosclerosis. Insulin resistance together with beta cell dysfunction leads to the appearance and gradual progression of type 2 diabetes. Thiazolidinediones, which act on insulin resistance, can slow and perhaps even prevent the progression of type 2 diabetes, where in some cases insulin injections become necessary. Also, treatment of insulin resistance with thiazolidinediones has been shown to ameliorate several complications associated with insulin resistance, such as hypertension, dyslipidemia, hyperinsulinemia, visceral fat, microalbuminuria and hyperglycemia. The conference presentations in this section will explore in intricate detail the nature of insulin resistance, its causes and consequences.

    Each time we eat, insulin is released into the bloodstream. This vital hormone, secreted by special cells in the pancreas, encourages our tissues - our muscles in particular - to gobble up the glucose surging through the bloodstream after we eat a meal. That's good news, because glucose hanging around in the blood is dangerous stuff. It can stick to proteins and destroy their ability to do their job. Kidney damage, blindness, and amputations may result. But insulin has many other vital roles. After a meal, insulin stops the liver from releasing any fat, a potential metabolic fuel, into the blood. Why after a meal? It turns out that just like glucose, these fats, released as triglycerides , are dangerous if they hang about in the blood too long.

    Although we have known about insulin resistance for a long time, it is only recently that most of us have become interested in it. I think that there are two reasons for our greater concern. For 66 years we have known the difference between people who have insulin resistance and those who don't. In his January 1936 Lancet It took the work of Gerald Reaven, M.D., to kick off the current wave of interest in insulin resistance. An endocrinologist who taught at Stanford University from 1959 to 1995 and who since then has been senior vice president for research for Shaman Pharmaceuticals in South San Francisco, Dr. Reaven took us much closer to understanding the condition in a 1988 lecture. His Banting Lecture on the " Role of Insulin Resistance in Human Disease " in the American Diabetes Association's professional journal Diabetes was the first to link insulin resistance with related variables including high cholesterol and high blood pressure as a cluster of risk factors for heart disease. It was in this article that he bestowed the name "Syndrome X" to these variables. Four or five non-professional books about insulin resistance and Syndrome X have appeared in the last couple of years, bringing these concepts to a much broader audience.     What is Insulin?
    Insulin is a hormone secreted by the pancreas. It helps the body utilize blood glucose or "blood sugar." Insulin does so by binding with receptors on cells like a key would fit into a lock. Once the key insulin has unlocked the door, the glucose can pass from the blood into the cell. Inside the cell, glucose is either used for energy or stored for future use in the form of glycogen.
    What is Insulin Resistance?
    Insulin resistance occurs when the normal amount of insulin secreted by the pancreas is not able to "unlock the door" to cells. To compensate, the pancreas secretes additional insulin. When the body's cells resist or do not respond to even high levels of insulin, glucose builds up in the blood resulting in high blood sugar. Even people with diabetes who take oral medication or insulin injections to control their blood glucose levels, can have higher than normal blood insulin levels due to insulin resistance.

    Protein Found to Stimulate Insulin Resistance
    A protein known as retinol binding protein 4, which is overproduced by fat cells in people with obesity, appears to promote insulin resistance elevating the risk for diabetes, heart disease and early death, according to a recent study.
    This finding could lead doctors towards the development of new drugs that focus on limiting secretion of the protein among people with insulin resistance, obesity and/or risk for type 2 diabetes.
    According to the researchers from the Beth Israel Deaconess Medical Center in Boston, insulin resistance might therefore be avoided if retinol binding protein 4 levels could be reduced and controlled. Read full article.

    Insulin Resistance Key for Heart Failure
    According to a recent Swedish study, insulin resistance as a key player in the relationship between diabetes and heart failure. The study, by researchers at Uppsala University, was published in the Journal of the American Medical Association.
    1,187 men aged 70 and older were studied and researchers found that those with insulin resistance were much more likely to develop congestive heart failure (CHF) than those whose bodies responded properly to insulin.
    It has been known that obesity increases the risk of congestive heart failure, a potentially fatal condition in which the heart muscle weakens, progressively losing the ability to pump blood, the researchers wrote.
    An earlier U.S. study found that 32 percent of older men with heart failure eventually developed diabetes, compared to only 18 percent of men with normal hearts, he said.

    by Marcelle Pick, OB/GYN NP Insulin resistance is so pervasive today that we evaluate nearly every woman who visits our clinic to determine her level of risk. Most are taken aback when they learn they either already have insulin resistance syndrome or are well on their way to developing it. Experts estimate that 25% of all Americans suffer from insulin resistance. We believe the percentage is much higher among perimenopausal women. Because insulin is one of the "major" hormones, it's also impossible for your body to balance its "minor" hormones (estrogen, progesterone and testosterone among them) until your insulin metabolism is balanced first. To put it simply, if you have hot flashes and you are insulin resistant, it's going to be nearly impossible to cure the hot flashes without first healing the insulin resistance. (Cortisol is also a "major" hormone; to understand its role in hormonal balance, read our related article on adrenal fatigue The good news is that you can heal insulin resistance.
