Inflammatory Bowel Disease Update
by Ronald Hoffman, M.D.
Ulcerative colitis and Crohn's disease are two conditions that have many features in common and are sometimes referred to as "inflammatory bowel disease" (IBD). While not as common as less severe conditions like "irritable bowel syndrome," Crohn's disease and ulcerative colitis now afflict hundreds of thousands of Americans. They appear to be increasing in frequency. Crohn's disease was once a very rare disorder but it is gradually becoming more common. There are about twice as many cases of it today as there were 30 years ago. Moreover, these conditions are increasingly afflicting children and teenagers.
Symptoms include periodic attacks of cramps, abdominal pain and diarrhea and a general sense of feeling ill. There may be fever, loss of appetite, and weight loss. Especially in ulcerative colitis there may be rectal bleeding, diarrhea, and mucous in the stool. These symptoms sometimes begin insidiously, gradually worsening, or sometimes they begin all at once with great severity.
These conditions must be diagnosed by a physician, usually a specialist in gastroenterology. The rectum and large intestine may be examined by sigmoidoscopy or colonoscopy. A biopsy specimen from the intestinal wall will probably be taken during one of these tests. Or an air-contrast barium enema or upper GI swallow with follow-through lower gastrointestinal x-rays may be given. A proper evaluation for inflammatory bowel disease will usually include stool tests to rule out parasite infection which can easily mimic ulcerative colitis or Crohn's disease, and blood tests to test the severity of the inflammation and blood loss.

FALL 2004 Advances in Treating Inflammatory Bowel Disease The intestinal tract is naturally exposed to more bacteria, including pathogens, than any other tissue in the body. And unlike the skin, which also is exposed to microbes, the bowel provides a warm, moist environment and is made of permeable tissue to allow the passage of water and nutrients. As a result, the bowel wall or lining always has a smoldering, low-level inflammatory response to keep these organisms in check. Problems begin when this inflammatory response runs out of control, causing inflammatory bowel disease (IBD). IBD is a blanket term that includes ulcerative colitis, which manifests itself in the colon, and Crohn's disease, which can occur anywhere in the intestinal tract. IBD can cause abdominal pain, diarrhea, fever, bleeding and nutritional deficiencies.

Feline inflammatory bowel disease (IBD) is a newly recognized disease and has been cited in several recent papers in the veterinary literature. The increased recognition of this unique disease has stimulated clinicians to include it in the differential diagnosis when a cat presents with signs of vomiting, diarrhea, or weight loss. In my experience, it is the most common chronic primary gastrointestinal (GI) disorder of cats. Etiology
The cause of (IBD) is unknown, although numerous theories have been proposed. One favored by most academicians is that this disease is an immune-mediated hypersensitivity reaction to indiscriminate antigens, including enteric bacteria and dietary components. Response to corticosteroids in the majority of the cases supports assertions of immune-mediated disease. There is no evidence that this disease is heritable.
    Pathophysiology and Histopathology
Inflammatory bowel disease is characterized by an infiltration of inflammatory cells in the mucosal lining of the bowel (ie., stomach, intestines, or colon). Depending on the degree of inflammation and their location within the bowel, these infiltrates can affect the secretions, motility, and absorptive capacity of the bowel, creating an environment in which vomiting and diarrhea may develop.

Measles, mumps, rubella vaccine, inflammatory bowel disease and autism
MMR is a combination vaccine that protects against measles, mumps, and rubella (German measles). It is given to children at 12 months and again at 4 years of age. A link between the MMR vaccine and development of bowel inflammation (IBD) and autism has been suggested by some researchers. However, numerous carefully performed studies and reviews have now been done that have found no relationship between MMR vaccine and autism or IBD.
This fact sheet answers the following commonly asked questions. More general information on the MMR vaccine is also available by following the links in Further reading below.

    What is MMR vaccine?
Since the introduction of vaccination against these diseases, measles, mumps and rubella have become rare in Australia. However, it is extremely important that children continue to be immunized because these diseases remain very common in other parts of the world, and even slight decreases in the number of immunized children can result in major epidemics of measles, mumps or rubella (including the congenital rubella syndrome which seriously affects unborn children).

    Food Allergies
A speaker once said that we can live a few weeks without food, a few days without water, a few minutes without air, but only a few seconds without hope. Hope is indeed essential to life, to living rather than just surviving or existing. Some people with food allergies discover what they are allergic to, avoid those foods, and by practicing avoidance faithfully, enjoy stable health for the rest of their lives. Some avoid their problem foods for a time, and then their allergies improve to the point that they can eat freely again. Yet others develop allergies to more and more foods as time goes on regardless of how faithful they are about their diets. I was once involved in a discussion on how to prevent the development of new food allergies. One person said that using a rotation diet and not eating any one food too often was the way to keep from developing more allergies. Another said that rotation diets could lead to eating single foods in large quantities, and that eating too much of any food could cause one to become allergic to it. Finally someone asked, "So what will prevent the development of more food allergies?" The answer given was, "Death!" This does indeed seem to be the situation for some people - a situation without hope. The purpose of this website is to give you hope. Just as a rainbow comes after the rain, so too can you enjoy good health again. By treating the underlying root causes of your food allergies, you may become healthier than you have ever been before. It might not be a quick process, and most of the time you will eat differently from those around you (at least at first), but you can again be healthy enough to enjoy your activities, friends, and family. You can truly live again rather than just survive.

The Bath University team found people with the gut disorder had an abundant number of a type of cannabinoid receptors in their body. They believe this is part of the body's attempt to dampen down the inflammation and that giving a drug that binds to these receptors could boost this. Their findings appear in the journal Gastroenterology. Cannabinoids When people have Crohn's disease or ulcerative colitis - collectively known as inflammatory bowel disease or IBD - their immune system goes into overdrive, producing inflammation in different areas of the digestive tract. This causes symptoms such as pain and urgent diarrhoea. Anecdotally, people with IBD who have been users of cannabis have reported that their symptoms get better when they use the drug. These initial results look extremely promising and exciting
Dr Derek Scott from Aberdeen University Dr Karen Wright and colleagues examined gut samples from healthy people and IBD patients and looked for the presence of two receptors known to react to natural cannabis-like compounds produced by the body.
