Feline Inflammatory Bowel Disease By Margaret Muns, DVM In cats, inflammatory bowel disease (IBD) is the most common cause of chronic vomiting and diarrhea. The term actually refers to a group of diseases that are characterized by the invasion of inflammatory cells into the cat's intestinal wall. Symptoms of IBD One or many of the following symptoms can be found in a cat with inflammatory bowel disease (IBD):
  • Vomiting Diarrhea Weight loss Normal/increased appetite Stomach rumbling Belching Black, tarry stools Flatulence (from digested blood) Halitosis Increased thirst Abdominal pain Weight loss
In severe cases, weight loss can be extreme. Vomiting cats will seldom produce food in a cat's vomit. Instead, the vomit usually consists of bile-stained mucus. The presence of hair or partially digested food in the vomit indicates that the disease also involves the cat's stomach. The most common form of inflammatory bowel disease in cats is the presence of lymphocytes and plasma cells, which produce a diagnosis of lymphocytic-plasmacytic enteritis (LPE). Causes of LPE This disease can develop in one of two ways. The inflammatory cells can enter the intestinal wall in response to an injury or infection. Or, parasites, food intolerance, bacteria, fungi, or cancer can cause activation of the immune system and subsequent inflammation.

What is inflammatory bowel disease?
Inflammatory bowel disease (IBD) is not a specific disease. Rather, it is a specific reaction that the stomach or intestines have to chronic irritation.
What are the clinical signs of IBD?
If the stomach is involved, your dog will have chronic vomiting. If the intestines are involved, chronic diarrhea will occur. This is the most common form. In some dogs, both parts of the digestive tract are involved so both vomiting and diarrhea occur.
If the disease occurs for several weeks to months, weight loss and poor appetite are common.
When is the most common occurrence?
IBD is most common in middle-aged to older dogs.
How is IBD diagnosed?
The chronic irritation that causes IBD stimulates the body to send cells from the immune system to the area. The most commonly found cells are lymphocytes and plasmacytes. Occasionally, eosinophils and neutrophils will be found. Thus, the disease is diagnosed when these cells are identified in abnormal levels in the tissue. A pathologist is responsible for this part of the diagnosis; his or her report usually calls the disease lymphoplasmacytic gastritis (stomach) or lymphoplasmacytic colitis (colon) .
In order to obtain these cells, a biopsy is required. In most cases, an endoscope is passed into the dog's stomach or colon (with the dog under anesthesia). A tiny biopsy instrument is passed through the endoscope and used to take small samples of the lining (mucosa) of the affected organ.

    WHAT IS ULCERATIVE COLITIS AND INFLAMMATORY BOWEL DISEASE?
    Inflammatory Bowel Disease
    Inflammatory bowel disease (IBD) is a general term that covers two disorders: ulcerative colitis and Crohn's disease. There is some evidence to suggest that they are part of a biologic continuum, but at this time they are considered to be distinct disorders with somewhat different treatment options. The basic distinctions are location and severity. It should be noted that as many as 10% of patients with IBD have findings and symptoms that match the criteria for both disorders, at least in the early stages (a situation called indeterminate colitis).
    Crohn's disease, also called regional enteritis, is a chronic inflammation of the intestines which is usually confined to the terminal portion of the small intestine, the ileum. Ulcerative colitis is a similar inflammation of the colon, or large intestine. These and other IBDs (inflammatory bowel disease) have been linked with an increased risk of colorectal cancer.
