by Pat Baird, M.A., R.D. We're all familiar with the occasional upset stomach and indigestion after eating a meal. What most of us then do is reach for the antacid bottle in the medicine cabinet. When antacids and other over-the-counter remedies don't relieve the discomfort, or when these stomach complaints and accompanying symptoms-such as bloating and upper abdominal distress or even pain-worsen or occur more frequently, people turn to their doctors for help. About five percent of all visits to family physicians and other primary care practitioners are for indigestion, as are up to one-third of referrals to gastroenterologists.
Doctors have a name for indigestion: dyspepsia , which merely means bad digestion. People may not have all the symptoms of dyspepsia described here, but by and large they know when they have it.
Individuals with indigestion typically will complain of some combination of heartburn, regurgitation, belching, bloating, feeling full after eating even a small meal, gassiness, nausea, and perhaps vomiting. Often, symptoms increase in frequency, and the discomfort escalates. When indigestion become chronic, it is a source of constant pain in the middle-to-upper part of the stomach.
These symptoms may resemble the complaints of a person with gastroesophageal reflux disease (GERD), and it is true that the two conditions are very similar. Heartburn and regurgitation, in particular, are common to both. Most symptoms clearly are related to eating. In addition, impairment of GI motility-the underlying cause of GERD-appears to be frequent in people with dyspepsia as well.

Chronic indigestion is painful. It can erode the lining of the stomach and esophagus, make you dizzy, promote anxiety, and even lead to panic attacks, as described in Dealing With Anxiety Attacks . So eliminating indigestion has a major impact on "quality of life". A severe attack of indigestion can also mimic the symptoms of a heart attack. The good news is that it can scare a person into straightening up their life. The bad news is that it leads to an avoidable, inconvenient trip to the hospital, expensive tests to rule out heart problems, and it's one hell of a scary thing. The intestinal gas that characterizes indigestion causes the bloated sensation. It can also make you dizzy. The gas comes from food that isn't completely digestedso the common approach of restricting stomach acids with medications is counterproductive, because they impede digestion, which makes gas even more likely as the food ferments in your stomach. by Jenny Bryan Each year, about four out of ten people suffer from indigestion. But look inside their digestive system, and most will have no obvious signs that there's anything wrong.
Some will have an inflamed oesophagus and a few will have an ulcer in their stomach or upper intestine. About 30% of people over 50 have a hiatus hernia (see abc of body under 'hiatus hernia') though many of these will have no symptoms at all. 'There are three types of indigestion,' explains Professor Roy Pounder from the Royal Free Hospital, London. 'Reflux type when stuff comes up from the stomach into the chest because of a weak valve into the stomach, indigestion pain which may be improved by eating if it's an ulcer, and dysmotility pain which makes you feel full, bloated and sick.' Clearly, the pain and discomfort of indigestion is just as real to someone with definite signs of gastric disease as someone whose digestive system looks healthy. And both need help to get rid of their symptoms. As Dr Brendan Delaney, reader in primary care and general practice at Birmingham University points out, persistent reflux can be just as disruptive to sleep and other daily activities as the chest pain of angina.

    It's getting busier in the ER. The number of hospital emergency rooms declined 15 percent in the decade through 2002, while the number of ER visits rose 23 percent to 110.2 million a year, according to the Centers for Disease Control and Prevention. This development is creating a challenge for the emergency-room physicians responsible for distinguishing indigestion from heart attack, influenza from meningitis, bronchitis from pulmonary embolism. With patient loads increasing, misdiagnosis remains a problem despite improvements in diagnostic technology. Yet patients can assist physicians in reaching accurate diagnoses; a look at one condition heart attack helps illustrate how. About five million to eight million Americans each year enter the emergency room complaining of chest pain, and fewer than 1.5 million of them are experiencing heart attacks, according to estimates by cardiologists and emergency-medicine physicians. Physicians fail to accurately diagnose some 2 percent to 5 percent of these cases, translating into at least 11,000 misdiagnosed heart attacks each year, many with fatal implications, according to various studies, including a 2000 article in the New England Journal of Medicine. Misdiagnosed heart attack is among the most common causes of malpractice litigation for emergency-medicine physicians.

According to the National Institutes of Health, more than 60 million American adults suffer from heartburn at least once a month, while 25 million adults experience heartburn every day. Heartburn feels like a burning pain in the middle of your chest that may move up into your throat. Heartburn occurs when the lining of the esophagus is exposed to stomach acid (hydrochloric acid). Symptoms of heartburn include excessive burping, pain in the chest area, acid regurgitation, sour taste, and bad breath. When heartburn is treated, it poses little risk to your overall health. It is more of an uncomfortable inconvenience than a medical problem. Left untreated, acid reflux disease is a complex condition, says James Maher, M.D., UI Health Care gastrointestinal specialist. People experience degrees of severity ranging from only symptoms to erosive esophagitis that damages your esophagus. Causes
    The cause is a weakness of the sphincter muscle between the esophagus and stomach. Normally, this muscle is always contracted, preventing acid from refluxing into the esophagus.     This directed case study provides students with an interesting story about carbohydrates and intestinal gas in a format that is funny, quick, and feasible for even the largest classes. In the constructivist model of learning, the first step to successful mastery of a new topic is to figure out what you already know and then add and organize new information by building on that initial knowledge ( Bransford, Brown et al, 2000 ). Since most Americans are familiar with the hype over low-carb diets, this case allows students to talk about their existing knowledge of carbohydrates (including misconceptions), thus providing a good launching point for learning more detailed information about carbohydrates and biochemistry. Molecules and dieting are topics rife with misconceptions. According to a survey of 1,017 adult Americans commissioned by the Partnership for Essential Nutrition, almost 40% of adults are currently trying to lose weight, and 19% of them are specifically reducing carbohydrates to do so. This is hard to do correctly if you can neither identify a carbohydrate nor give the reason for consuming carbohydrates in your diet. For example, the adults surveyed by the Partnership for Essential Nutrition were unable to identify many carbohydrate-containing foods; only 61% of respondents could name oatmeal as a food rich in carbohydrates, 40% could name bananas, 36% apples, 20% tomatoes, 17% broccoli, and 12% named lettuce. This case helps remedy this ignorance by completing the following objectives.
