Download this Patient Note in PDF format VOL 108 / NO 1 / JULY 2000 / POSTGRADUATE MEDICINE Bladder control is a problem for more than 10 million Americans of all ages. In fact, at least one of every 25 Americans has some symptoms related to loss of bladder control. When left untreated, urinary incontinence can be debilitating and can lead to social isolation, psychological distress, skin problems, and even premature admission to a nursing home. What causes incontinence?
There are several types of incontinence, with different causes:
  • Stress incontinence involves leaking of urine during physical activities that cause sudden increases in abdominal pressure, such as coughing, laughing, or sneezing. Weakness of the muscles that control outflow of urine from the bladder is the usual cause. This type of incontinence is often related to stretching of the pelvic muscles during childbirth.
  • Urge incontinence is an uncontrollable need to urinate that may occur both during the day and at night. This type of incontinence may be caused by inflammation or infection of the bladder or a nerve disorder.
  • Overflow incontinence causes frequent or constant dribbling of urine because the bladder is unable to empty completely and overfills. Nerve and muscle disorders or injuries are common causes of overflow incontinence.

Incontinence in women
Incontinence is defined as an involuntary loss of urine that's enough to cause a social or hygiene concern. It is about four times more common in women than in men. By the age of 75, at least 16% of women experience some incontinence, but younger women can also be affected.
Passing urine
Urine is produced by the kidneys and collected in the bladder, which expands like a balloon as the volume increases. When full, the bladder empties to the outside through the urethra. Most people need to pass water every three to four hours during the day and up to once or twice in the night. For normal urination, the muscular wall of the bladder has to contract at the same time as a valve mechanism at the outlet of the bladder relaxes. Most incontinence in adults results from problems with one or other, or a combination, of these processes.
Pelvic organs
Symptoms and causes
Stress incontinence
This causes urine to leak when there is an increase in pressure from within the abdomen such as when coughing, sneezing, laughing or carrying heavy weights. It develops when the normal control mechanism for keeping the outlet of the bladder closed is weakened. This can happen when the urethra moves out of the normal position (prolapses), as it can when the pelvic floor muscles are weakened. Stress incontinence is much more common in women than men and can be triggered by childbirth, hysterectomy and the menopause.

What is urinary incontinence and what causes it?
When you are not able to hold your urine until you can get to a bathroom, you have what's called urinary incontinence (also called loss of bladder control). In contrast, bladder control means you urinate only when you want to. Incontinence can often be temporary, and it always caused by an underlying medical condition. More than 13 million Americans experience loss of bladder control. However, women suffer from incontinence twice as often as men do. Both women and men can have trouble with bladder control from neurological nerve ) injury, birth defects, strokes multiple sclerosis (MS), and physical problems associated with aging. Older women have more bladder control problems than younger women do. The loss of bladder control, however, is not something that has to happen as you grow older. It can be treated and often cured, whatever your age. Don't let any embarrassment about incontinence prevent you from talking to your health care provider about your condition. Find out if you have a medical condition that needs treatment.

http://www.ahrq.gov/clinic/uiovervw.htm
Overview: Urinary Incontinence in Adults: Clinical Practice Guideline Update
In 1992, the Agency for Health Care Policy and Research (AHCPR) released its first guideline on urinary incontinence. Since then the guideline has become the standard of care for incontinence in many settings across the country. This update of the guideline reflects new findings in the rapidly changing field of treatment for urinary incontinence. To develop the update, AHCPR convened a multidisciplinary private-sector panel of physicians, nurses, allied health professionals, and health care consumers to study the effectiveness of diagnostic and treatment procedures for urinary incontinence, their costs, and how they affect patient outcomes. The results of this research show that incontinence can be improved, and in some cases, even cured. Anecdotal evidence shows that long-term care facilities that have adopted the guideline have improved the quality of life of their patients and saved money at the same time.

100. Superabsorbent Polymers Have Become An Important Component Of Diapers During The
Article about the history, properties, applications and commercial uses of superabsorbent polymers for diapers, adult incontinence and feminine hygiene products. Authors E. Pytlik, D. Molino and J. Moritz.
http://www.eng.buffalo.edu/Courses/ce435/Diapers/Diapers.html
Superabsorbent Polymers Erin Pytlik Daryl Molino Joseph Moritz INTRODUCTION Superabsorbent Polymers (SAP): Superabsorbent polymers are primarily used as an absorbent for water and aqueous solutions for diapers, adult incontinence products, feminine hygiene products, and similar applications. Undoubtedly, in these applications, superabsorbent materials will replace traditional absorbent materials such as cloth, cotton, paper wadding, and cellulose fiber. Commercial production of superabsorbent polymers began in Japan in 1978, for use in feminine napkins. This early superabsorbent was a crosslinked starch-g-polyacrylate. Polyacrylic acids eventually replaced earlier superabsorbents, and is the primary polymer employed for superabsorbent polymers today. In 1980, European countries further developed the superabsorbent polymer for use in baby diapers. This first diapers employing this technology used only a small amount of polymer, approximately 1-2 g. In 1983, a thinner diaper using 4-5 grams of polymer and less fluff was marketed in Japan. The use of superabsorbent polymers revolutionized the diaper industry. Diaper manufacturers began to design diapers to take advantage of the amazing liquid retention ability of the polymer. Superabsorbent polymers absorb, and retain under a slight mechanical pressure, about 30 times their weight in urine.
