IMPULSE CONTROL DISORDERS WHAT ARE IMPULSE CONTROL DISORDERS? Impulse control disorders are a group of five behavior disorders characterized by losing control of one’s behavior in certain situations. People with these disorders feel tension that builds to a high level before they engage in the behavior. Afterward, they feel a sense of release or perhaps pleasure. Some people experience guilt or shame afterward. The loss of control over one’s behavior may be part of a larger pattern of behavior that includes substance abuse or sexual disorders. Impulse control disorders include the following specific disorders:
  • Excessive anger (intermittent explosive disorder, or IED) Compulsive stealing (kleptomania) Compulsive fire setting (pyromania) Compulsive pulling out of hair (trichotillomania) Pathological gambling
WHAT ARE THE SYMPTOMS OF IMPULSE CONTROL DISORDERS? Excessive Anger (IED) A person with this disorder engages in extreme, unwarranted angry outbursts. These angry explosions are out of proportion with the situation and result in physical assaults onto others or in acts that cause significant damage to property. To be considered IED, the behavior must not be the result of substance abuse or a medical condition. People with IED are often charged with domestic violence or assault and have legal problems as a result. Rarely do they take responsibility for their behavior; instead, they usually blame the victim.

Current Psychiatry Reports Published Abstract Impulse control disorders (ICDs) are characterized by irresistibl-e urges to perform acts that result in a re-duction of tension and possibly gratification, but also have negative consequences. Trichotillomania, an ICD, is characterized by a recurrent failure to resist impulses to pull out one's hair, resulting in noticeable hair loss. Limited information is available about structural and neurochemical differences in individuals with trichotillomania. Cognitive behavioral techniques are promising treatments for trichotillomania. Pharmacologic treatments have focussed on clomipramine and venlafaxine as potentially effective for the short term control of symptoms in trichotillomania. Selective sero-tonin reuptake inhibitors, though promising in open trials, seem to be largely ineffective in reducing hair pulling in controlled studies. Durability of pharmacologic benefit for the symptoms of trichotillomania, both in a small trial and in clinical experience, appears to be poor.

Definition
Impulse control disorders are characterized by an inability to resist the impulse to perform an action that is harmful to one's self or others. This is a relatively new class of personality disorders , and the most common of these are intermittent explosive disorder , kleptomania, pyromania, compulsive gambling disorder, and trichotillomania. Description All of these impulse control disorders involve the loss or lack of control in certain specific situations. The hallmark of these disorders is the individual's inability to stop impulses that may cause harm to themselves or others. Affected individuals often feel anxiety or tension in considering these behaviors. This anxiety or tension is relieved or diminished once the action is performed. Intermittent explosive disorder is more common among men, and involves aggressive outbursts that lead to assaults on others or destruction of property. These outburst are unprovoked or seem to be out of proportion to the event that precedes them. Kleptomania is more common among women, and involves the theft of objects that are seemingly worthless. The act of stealing relieves tension and is seen by the individually to be rewarding. The actual stealing is not pre-planned, and the concept of punishment for the crime doesn't occur to these individuals, although they are aware that what they are doing is wrong.
