However the classification of Impulse-Control Disorders is reserved for those disorders in which the defining characteristic is the inability to inhibit an impulse which might be harmful to oneself or others. Impulse Disorders involve the inability to resist an impulse or psychological drive to act in a way harmful to oneself or others. The urge causes anxiety, tension or arousal; its completion gives relief or gratification; regret or guilt may follow. There are five distinct Impulse Disorders. Intermittent Explosive Disorder is evidenced by episodes of acting out aggression and causing bodily harm and/or property destruction. Kleptomania is evidenced by acting out the impulse to misappropriate objects without the motive of monetary gain. Pyromania is evidenced by setting fires for hedonistic purposes.

  • These are disorders where the person acts upon an impulse, drive or temptation that is potentially harmful to either themselves or others, but which they can't resist.
  • The behavior is usually preceded by the building up of anxiety or arousal, once the impulse is acted upon, pleasure, gratification or relief are perceived.

Program background
Gambling is deeply rooted in Brazilian culture, despite its partial prohibition in 1946 by President Dutra's conservative government. The presidential decree banned casinos, but kept lotteries and horse races. In the early 1990s, gambling machines were slowly reintroduced through breaches in a new law that allowed bingo games to foster amateur sports. Currently the bingo label covers a diverse array of electronic devices offered in venues of various sizes (from 20 machines up to 400) that largely resemble casinos, except for the absence of card games and roulette wheels. In the current year, a political scandal involving undeclared funds for electoral campaigns forced a temporary closure of bingo venues. The future of legal gambling in Brazil is an open question and the current debate is intense. While some pathological gamblers feel relieved at the closures, some have already turned to illegal alternatives. Despite these concerns, initiatives to produce epidemiological data on gambling are just beginning. Medical School, the largest university medical center in Brazil with 200 beds and 70,000 outpatient appointments a year. It is one of only two centers specializing in gambling and behavioral addictions in Brazil. AMJO's services are divided into three major areas: research, treatment, and teaching.

Concept of Impulse Control Disorder Historically, the term impulse control disorder was used to refer to such behaviors as firesetting, gambling, paraphilia, kleptomania, trichotillomania, and violence. The concept is proving increasingly useful to explain the etiology of and plan treatment for externalizing disorders common in adolescence. This includes substance use disorders, suicide, conduct disorder (CD), eating disorders, a large subgroup of attention deficit hyperactivity disorder (ADHD), Tourette's disorder and obsessive-compulsive disorder (OCD). These disorders frequently are associated with depression, representing a possible form of "affective spectrum disorder" (McElroy et al., 1992). Utilizing this behavioral concept of impulse control in diagnosis and treatment leads us to shift from thinking nosologically to thinking functionally.

Substance abuse in youth is associated with depression (DeMilio, 1989), suicide gestures and attempts (Brent et al., 1994), eating disorders (Killen et al., 1987), conduct disorder, and ADHD (Wilens and Biederman, 1993). SUDs are often comorbid with other impulse control disorders. The association with suicidal behavior was mentioned earlier. There is a strong association between early conduct disorder and later substance use (Lynskey and Fergusson, 1995). The direct association of substance abuse with attention deficit disorder is controversial, but is often reported as insignificant when conduct disorder is taken into account (Lynskey, 1995).
