Impotence affects about one out of every ten American men, yet many of them don't seek help because they're embarrassed, they think there's no solution or they believe the problem is "all in their minds." Unfortunately, these men are missing out on a wide variety of treatment options. I mpotence, the persistent inability to achieve and maintain an erection for intercourse, affects as many as 18 million men in the United States between the ages of 40 and 70. Until recently, doctors thought impotence was mainly rooted in psychological causes. Now it is believed that 50 to 70 percent of all cases are caused by physical problems. Erection difficulties tend to increase with age, but that is not the only or even the most important factor. Your general physical and psychological health, as well as lifestyle habits and certain medications, can all cause impotence, but you don't have to live with this problem. In most cases, impotence can be successfully treated.

FDA APPROVES IMPOTENCE PILL, VIAGRA
The Food and Drug Administration today announced the approval of Viagra (sildenafil citrate), the first oral pill to treat impotence, a dysfunction that affects millions of men in the United States. Unlike previously approved treatments for impotence, Viagra does not directly cause penile erections, but affects the response to sexual stimulation. The drug acts by enhancing the smooth muscle relaxant effects of nitric oxide, a chemical that is normally released in response to sexual stimulation. This smooth muscle relaxation allows increased blood flow into certain areas of the penis leading to an erection. Viagra was evaluated in numerous randomized, placebo controlled trials involving more than 3000 men with varying degrees of impotence associated with diabetes, spinal cord injury, history of prostate surgery, and no identifiable organic cause of impotence. Patients also had a wide range of other concomitant illnesses including hypertension and coronary artery disease.

    One of the real problems with giving advice about smoking to young men is that we are telling them not to smoke now because of health problems they may experience decades in the future. Perhaps a more direct approach on cigarette packets would be more helpful. Not 'smoking kills' but rather 'smoking makes you impotent'. A new systematic review of smoking prevalence among impotent men points in that direction [1].
    Systematic review
    The review searched MEDLINE from 1980 to about 2000 for articles on impotence or erectile dysfunction and which described studies in the United States. The specific US choice was to be able to examine available smoking prevalence data for the general population. From each study the number of impotent men who were current smokers, the definition of impotence, the definition of smoking, ages, and location were taken.
    For each study, a tailored comparison group based on age distribution, time and location was derived from smoking prevalence data from an ongoing surveillance system.

    Impotence is the inability of a man to either have an erection or to keep the penis erect long enough to have sexual intercourse. This is a common problem affecting as many as fifteen million men in the United States alone. By the time most men are in their forties, they have already experienced impotence at some time in their lives. When just a single incident occurs, the best thing to do is forget about it. There is no reason for a man to worry about something that almost all men experience a few times in their lives. Problems arise when this difficulty starts occurring regularly. Men afflicted with repeated impotence tend to be middle-aged and older, with a condition most common in men 60 years of age and older. Regardless of the age, many men who have this problem can be treated successfully.

    July 2003 Erectile dysfunction (ED), or impotence, occurs when a man has difficulty with either getting an erection or with keeping an erection for long enough to allow satisfactory sexual activity. It is one of the most common sexual problems and affects around half of all men over 40 at some point. It becomes more common and severe as men get older. However, only a fraction of affected men seek help.
    Erections
    The shaft of the penis has two chambers that fill up with blood during sexual arousal. These are known as the corpora cavernosa. When a man has sexual thoughts or feelings, the brain communicates this sexual excitement through the nerves and these eventually reach the genitals. In turn, the nerves bring about a relaxation of the muscle cells in the walls of the blood vessels entering the penis. This increased flow of blood fills up the two corpora cavernosa resulting in an erection.
    Cause of erection problems
    There are a number of causes of ED. In around two thirds of cases, there are physical problems affecting the blood supply. However, there is a complex relationship between physical and psychological aspects of sexual function. For instance, physical health problems can cause psychological distress that can have an additional effect on erection problems.

    U rologists are inundated with inquiries about it. The news media is treating it as the hottest since Prozac. Viagra, the pharmacological treatment for impotence, went on the market in April amid a torrent of publicity. Its manufacturer, Pfizer, Inc., pegs the success rates as high as 80 percent. Men are expected to find the drug far more palatable than the penile implants, vacuum pumps, injections and other standard medical treatments for impotence. Such is the way that the treatment of impotence is changing. Once thought to be a largely psychological problem, experts have since discovered that diseases such as diabetes or hypertension—or the drugs used to treat them—are often the cause of erectile dysfunction. And while talk therapy was once regarded as the first line of treatment, impotence now appears to be cured by simply popping a pill. So where does that leave psychologists who have built careers as sex therapists? Has impotence become the domain of urologists and pharmaceutical companies, at the expense of mental health providers?

It was first used to describe loss of sexual power in 1655 in, of all places, a treatise entitled 'Church History of Britain' by Thomas Fuller. Impotence is the inability to obtain or maintain an erection for the satisfactory completion of heterosexual vaginal intercourse. Satisfactory is usually taken to mean an adequate erection, of sufficient hardness, maintained for a sufficient length of time, that ends in a controlled ejaculation and provides sexual satisfaction for both partners. Impotence is a common and distressing condition affecting 10 to 30 per cent of men on a regular basis. All age groups are involved, but due to embarrassment or a mistaken belief that nothing can be done, victims often suffer in silence and despair. Whatever the cause of impotence, 99 per cent of men can get their erections back by one of the many treatment options now available.
