The impact of smoking on sexual, reproductive and child health

This report presents the first focused overview of the impact of smoking on sexual, reproductive and child health in the United Kingdom.
It considers active and passive smoking by both men and women, and summarises the impact on sexual health, conception and pregnancy, as well as effects on the reproductive system.
The impact of smoking on infant and child health is also considered.
The benefits of smoking cessation are presented. The current situation in the UK with regard to both active and passive smoking throughout the life-course is outlined, and the potential impact of smoking prevention and cessation on alleviating the burden of illness highlighted.
The report frames smoking as an important consideration in sexual, reproductive and child health. It makes recommendations to reduce the burden of sexual, reproductive and childhood ill health caused by tobacco, including recommendations for research, healthcare professionals and public policy.

Simvastatin and impotence
The benefits of lowering raised cholesterol concentrations are established in patients with documented coronary artery disease and those at high risk. Inhibitors of 3-hydroxy-3-methylglutaryl coenzyme A reductase (statins) are highly effective agents with few reported adverse effects. However, as their use becomes more common, adverse effects may be increasingly recognised. Impotence, rarely volunteered or asked about, is an important adverse effect of drugs and not currently associated with simvastatin.

Derby, Ph.D., of the New England Research Institutes in Watertown, Mass., and colleagues analyzed data gathered on the health and lifestyles of nearly 600 men as part of the Massachusetts Male Aging Study. Most men experience this at some point in their lives, usually by age 40, and are not psychologically affected by it. Some men, however, experience chronic, complete erectile dysfunction (impotence), and others, partial or brief erections. Frequent erectile dysfunction can cause emotional and relationship problems, and often leads to diminished self-esteem. Erectile dysfunction has many causes, most of which are treatable, and is not an inevitable consequence of aging. Incidence and Prevalence The term "erectile dysfunction" can mean the inability to achieve erection, an inconsistent ability to do so, or the ability to achieve only brief erections. These various definitions make estimating the incidence of erectile dysfunction difficult. According to the National Institutes of Health in 2002, an estimated 15 million to 30 million men in the United States experience chronic erectile dysfunction.

It is estimated that 20 million men in the United States suffer from sexual dysfunction. Many of these men are not aware of the many options that are available for the treatment of this problem. For a large majority of men who experience sexual dysfunction, the cause is physical rather than psychological. A number of diseases such as high blood pressure, diabetes or even old age may alter sexual functioning. For example, half of all men with diabetes experience some degree of sexual dysfunction. In the past, many men were told "It's all in your mind" and sent home. Now we are able to pinpoint the cause of erection problems and successfully treat 90% to 95% of them. The department of urology at Wayne State University has more than 16 years experience treating male sexual dysfunction. We have a physician dedicated to the treatment of male sexual dysfunction and our support staff is also specially trained in testing and patient education. How an Erection Happens
For an erection to occur, a series of events must take place. The first event in this chain is psychosexual stimulation. This is promoted by the male sex drive, also known as libido. The mind then sends impulses down the nerve pathways to the penis. These nerve impulses relax the smooth muscles of the arteries which supply the penis with blood. This muscle relaxation leads to engorgement and erection of the penis. After orgasm, the blood is returned to the general circulation and the penis returns to a soft (flaccid) state. Even a minor disturbance in any of these steps such as decreased blood supply, disturbances of nerve mechanisms or psychosexual problems may lead to erection problems.

Senator Charles Schumer, the leader of a group of 38 Senators at most, effectively declared himself the umpire of the Roberts confirmation process. Indeed, the Senator called two strikes on Judge Roberts yesterday, and warned of a third. The first alleged strike is the White House's refusal to provide memos that Roberts wrote when he was advising the U.S. solicitor general. The second strike is Roberts' apparent refusal to answer Schumer's questions about the merit of past Supreme Court decisions during their interview session, as well as his refusal to answer these kinds of questions during prior confirmation hearings. Schumer threatened to call a third strike if the Senate majority sets a confirmation schedule that would enable Roberts to take his place on the Court when it re-opens for business in October. Normally, three strikes means you're out. But in this case apparently it just means delay. So that's the desperate Dems strategy. Create procedural disputes by demanding that the White House produce documents that every living former solicitor general has said ought not to be produced. Insist on answers to questions that Justice Ginsburg steadfastly refused to answer. Hope that this gets you enough traction to delay the process. Use the delay time to dig for dirt, as they tried to do with Robert Bork and Clarence Thomas.

From staff reports WASHINGTON (CNN) A medical advisory committee recommended Monday that federal approval be given for a new drug to assist men suffering from erectile dysfunction. The panel further recommended that labels go along with the drug, Uprima (pronounced "you-pree-muh"), to warn of possible side effects when a user also consumes alcohol or is taking heart medication. Uprima is being manufactured by TAP Pharmaceuticals, a joint venture between Illinois-based Abbott Laboratories and Japan's Takeda Pharmaceuticals.

Health professionals prefer the term erectile inhibition or erectile dysfunction to the term impotence due to the sweeping negative connotation the term impotence projects onto men. Erectile difficulties can be divided into two types: primary and secondary. A man who has never had an erection sufficient for intercourse suffers from primary erectile dysfunction. A man who has some history of normal erections during intercourse, but has developed a persistent inability to obtain or maintain erections, has secondary erectile dysfunction. Secondary erectile dysfuction is more common, and treatment is more successful than in cases of primary erectile dysfunction. A male's ability to get an erection is an automatic process, a reflex that is not under the man's conscious control (i.e. he can't just "will it" or "make it" happen). When a man has difficulty getting or maintaining an erection, the blood that should flow into the penis and engorge it, making it firm and erect, fails to do so, even though the man may feel excited and stimulated. Causes of Impotence Are Multiple
The causes of erectile difficulty can be physical, psychological, or a combination of both. As we age, our bodies gradually slow down and it is normal for an older male to take longer (and to require more direct penile stimulation) to become erect than a younger male.

Labels on Viagra, Cialis and Levitra to reflect concerns about blindness
WASHINGTON - The government on Friday ordered warnings onto the labels of Viagra and two other impotence drugs that some users have developed a form of blindness - while cautioning that it's impossible to know if the pills are to blame. The Food and Drug Administration's move comes as the agency is under intense pressure to investigate more aggressively and warn the public about drug side effects. At issue is sudden vision loss when blood flow to the optic nerve is blocked, a condition called NAION or non-arteritic anterior ischemic optic neuropathy. NAION is considered one of the most common causes of sudden vision loss in older people, with anywhere from 1,000 to 6,000 cases a year. Moreover, risk factors include diabetes and heart disease, two of the leading causes of impotence. The FDA has 43 reports of NAION among the impotence drug users: 38 for Viagra, four for Cialis and one for Levitra. They include varying degrees of vision loss, including blindness.

May 5, 1998 Web posted at: 11:34 p.m. EDT (0334 GMT) SAN FRANCISCO (CNN) The world's largest organization of eye doctors has called for more study of the popular impotence-treatment drug Viagra, and the physicians warn that patients with some types of eye problems should avoid higher doses. Some patients taking Viagra have experienced temporary vision problems, including light sensitivity and a bluish tinge in their vision. The 23,000-member American Academy of Ophthalmology, meeting in San Francisco this week, is urging people to take the problems seriously. The doctors points out that drug's long-term effects are unknown. "FDA clinical trials show that taking the medication, especially at higher doses, can cause some retinal dysfunction, and affect the way we see for a number of hours," association spokesman Dr. Michael F. Marmor said in a statement. Pfizer, the pharmaceutical company that developed Viagra, said about 3 percent of the men who tested the drug experienced vision problems, but the trouble went away after a few hours. The company said it plans no further testing.

WASHINGTON, Nov. 21, 2003 - A third pill to treat impotence was approved for sale in the United States on Friday, intensifying competition in a billion-dollar slice of the drug market. The new pill will be sold under the name Cialis. It joins Viagra, the oral drug that went on the market in 1998, and Levitra, which was approved earlier this year by the Food and Drug Administration. All three drugs act on an enzyme that helps prompt and maintain erections by relaxing muscles in the penis and blood vessels. The duration and onset of the drug action is different, however, with each pill. Some market studies suggest that about 30 million men over the age of 40 have symptoms of impotence.
