Approximately 50% of American men over 40 have experienced problems with impotence to varying degrees, according to a study by the New England Research Institute. The ability to have an erection declines with age. A recent study suggests that 19 million men from age 40 to 70 in the U.S., out of a total of more than 37 million, either may be impotent or may have experienced the problem. While it used to be thought that impotence was primarily a psychological problem, doctors now tell us that physical causes predominate. Among the most common: atherosclerotic disease (vascular disease), which usually starts affecting men as they age... in the 50s, 60s, and especially in the 70s; diabetes; alcoholism; neurogenic problems, such as MS (multiple sclerosis); spinal cord injuries; hormonal deficiencies; certain medications, such as those used to treat high blood pressure, tranquilizers, sedatives, and amphetamines. The Marlboro Man May be Impotent According to Denver urologist Lawrence Karsh, M.D., smoking is one of the worst things a young man can do to his body. We already know about smoking's link to lung cancer and heart disease. But smoking, over many years' time, can also damage and block the blood vessels inside the penis, resulting in a failure to sustain a normal erection. In most cases, the damage won't be seen until it's too late sometimes not for 20 - 30 years or longer.

On this page: Erectile dysfunction, sometimes called "impotence," is the repeated inability to get or keep an erection firm enough for sexual intercourse. The word "impotence" may also be used to describe other problems that interfere with sexual intercourse and reproduction, such as lack of sexual desire and problems with ejaculation or orgasm. Using the term erectile dysfunction makes it clear that those other problems are not involved. In older men, ED usually has a physical cause, such as disease, injury, or side effects of drugs. Any disorder that causes injury to the nerves or impairs blood flow in the penis has the potential to cause ED. Incidence increases with age: About 5 percent of 40-year-old men and between 15 and 25 percent of 65-year-old men experience ED. But it is not an inevitable part of aging. ED is treatable at any age, and awareness of this fact has been growing. More men have been seeking help and returning to normal sexual activity because of improved, successful treatments for ED. Urologists, who specialize in problems of the urinary tract, have traditionally treated ED; however, urologists accounted for only 25 percent of Viagra mentions in 1999.

July 2003 Erectile dysfunction (ED), or impotence, occurs when a man has difficulty with either getting an erection or with keeping an erection for long enough to allow satisfactory sexual activity. It is one of the most common sexual problems and affects around half of all men over 40 at some point. It becomes more common and severe as men get older. However, only a fraction of affected men seek help.
Erections
The shaft of the penis has two chambers that fill up with blood during sexual arousal. These are known as the corpora cavernosa. When a man has sexual thoughts or feelings, the brain communicates this sexual excitement through the nerves and these eventually reach the genitals. In turn, the nerves bring about a relaxation of the muscle cells in the walls of the blood vessels entering the penis. This increased flow of blood fills up the two corpora cavernosa resulting in an erection.
Cause of erection problems
There are a number of causes of ED. In around two thirds of cases, there are physical problems affecting the blood supply. However, there is a complex relationship between physical and psychological aspects of sexual function. For instance, physical health problems can cause psychological distress that can have an additional effect on erection problems.

Urological Disorders
Urology is the branch of medicine concerned with the urinary tract in both genders, and with the genital tract or reproductive system in the male. Urogenital is a word that refers to the urinary and genital organs. [The medical specialty of Obstetrics and Gynecology specializes in the reproduction (genital) system of females. Nephrology is the medical specialty concerned with the kidneys.] According to the National Kidney Foundation, kidney and urologic diseases affect more than 13 million people (about 5 percent of the US population) and claim about 260,000 lives annually. The number of people affected by these diseases is expected to grow as the populations of older adults and racial and ethnic minorities, groups disproportionately affected by the diseases, increase. The most serious and debilitating of kidney and urinary tract diseases include:
  • end-stage renal disease (ESRD)
  • stone disease
  • urinary incontinence
  • benign prostatic hyperplasia (BPH)
  • interstitial cystitis
  • urinary tract infection
  • polycystic kidney disease
Other, non-related diseases have a tremendous impact on the disability and death associated with kidney and urologic diseases, including diabetes and hypertension.
