Procedure
Surgical repair involves creating an opening for passage of stool. Complete absence of an anal opening requires emergency surgery for the newborn. Surgical repairs are done while the baby is deep asleep and pain-free (using general anesthesia). Surgery for a high type imperforate anus defect usually involves creation of a temporary opening of the large intestine (colon) onto the abdomen to allow passage of stool (this is called a colostomy). The baby is allowed to grow for several months before attempting the more complex anal repair. The anal repair involves an abdominal incision, loosening the colon from its attachments in the abdomen to allow it to be repositioned. Through an anal incision, the rectal pouch is pulled down into place, and the anal opening is completed. The colostomy may be closed during this stage or may be left in place for a few more months and closed at a later stage.

Definition: Imperforate anus repair is surgery to correct birth defects involving malformation of the rectum and anus. Normally, the large intestine (colon) empties into a pouch-like portion of bowel (rectum). Through complex nerve and muscle structures, the rectum releases stool through the anus out of the body. Infants with imperforate anus defect, however, have malformations of the rectum and anus in which abnormal development leads to growth of skin over the anus and incomplete development of the rectum. As a result, these infants have either no opening through which stool can pass, or a very small opening that allows only a small amount of stool to pass out of the rectum. This very small opening is often accompanied by fistulas, which are abnormal channels that connect skin to bowel. Variations of the defect include:

Expectations after surgery: Most anorectal malformations can be corrected successfully with surgery. Normal bowel function can be established in most cases. The long-term expectations are excellent if no other pelvic abnormalities are found.
Convalescence: The child may spend several days in the hospital. Dilatation of the new anus (to improve muscle tone and to prevent narrowing) will be performed in the hospital and will be continued for some months. Use of stool softeners and a high-residue diet will need to continue throughout childhood.

Follow Ups Post Followup UOA Parents of Ostomy Children Discussion #5 FAQ Posted by Heather A. on February 16, 2005 at 22:24:07: We recently adopted a little girl, 18 months old, from China with an imperforate anus. We saw our local surgeon who has referred us to Dr. Alberto Pena. We are waiting to hear from Dr. Pena's office and praying he takes her case. If so, we will fly from New Mexico to New York for the surgery. From reading many of these posts, it appears that this will be tough on an 18 month old. All lab work, EKG, Echo, etc., have been normal. Our surgeon here, though, believes her situation is complicated enough to go to Dr. Pena. Can anyone tell us what we might be up against? We're ready to do anything and everything to enable our new little one to have a normal life. Any feedback would be greatly appreciated.

We also are patients of Dr. Pena. I can not say enough great things about him. He did a wonderful job on our daughter Olivia. It does take a while to here back from him. Do not be surprised when he calls himself to talk to you about your daughter. Does you daughter only have IA or does she have cloaca?
