Update on Acquired Immune Deficiency Syndrome (AIDS) among Patients with Hemophilia A
Case 1: A 55-year-old severe hemophiliac from Alabama developed anorexia and progressive weight loss beginning in September 1981. He had developed adult-onset diabetes mellitus in 1973, which had required insulin therapy since 1978. He had had acute hepatitis (type unknown) in 1975. In March 1982, he was hospitalized for herpes zoster and a 17-kg weight loss. Hepatosplenomegaly was noted. The absolute lymphocyte count was 450/mm((3)). Liver enzymes were elevated; antibodies to hepatitis B core and surface antigens were present. A liver biopsy showed changes consistent with persistent hepatitis. Evaluation for an occult malignancy was negative. The zoster resolved following 5 days of adenosine arabinoside therapy. In early June, he was readmitted with fever and respiratory symptoms. Chest x-ray showed bibasilar infiltrates. No causative organism was identified, but clinical improvement occurred coincident with administration of broad spectrum antibiotics. Laboratory studies as an outpatient documented transient thrombocytopenia (63,000/mm((3))) and persistent inversion of his T-helper/T-suppressor ratio (TH/TS =0.2). He was readmitted for the third time in early September with fever, chills and nonproductive cough. His cumulative weight loss was now 47 kg. Chest x-ray demonstrated bilateral pneumonia, and open lung biopsy showed infection with P. carinii. He responded to sulfamethoxazole/trimethoprim (SMZ/TMP). His T-cell defects persist.

  • Immunodeficiency is one of the underlying factors for the development of chronic and recurrent sinusitis. Intravenous immune globulin (IVIG) is indicated for use in patients with impaired humoral immunity.
  • Immunodeficiency is one of the underlying factors for the development of chronic and recurrent sinusitis. At the present time, intravenous immune globulin (IVIG) is indicated in the treatment of antibody deficiency syndromes, including X-linked agammaglobulinemia, common variable immunodeficiency, Wiskott-Aldrich syndrome, and hyper-IgM syndrome. The indication for IVIG in other conditions, such as selective antibody deficiency syndromes or IgG subclass deficiencies, is more controversial.

    Newborn screening could detect bubble boy illness early, save lives
    Researchers at the National Human Genome Research Institute (NHGRI), part of the National Institutes of Health (NIH), have developed a new laboratory method that rapidly identifies babies born with inherited forms of severe immune deficiency. The new genetic test, which still must be validated before widespread use, could someday be added to the panel of tests that already screen newborns for a variety of disorders.
    Developed in the NHGRI Division of Intramural Research (DIR), the new test can use the same dried blood samples already collected from newborns and would provide the first accurate, high-throughput screen for immune deficiencies. Prior efforts to identify this disorder by counting white blood cells in newborns proved unreliable and expensive.
    The development of the new test is described in the February issue of The Journal of Allergy and Clinical Immunology. Although the availability of the test raises the question of whether states should begin using it on all newborns, Dr. Puck concluded that the new test is not quite ready for widespread use. It must first be validated.
