ABOUT THE IgA NEPHROPATHY SUPPORT NETWORK
Founded in 1992, the IgA Nephropathy Support Network is a non-profit organization that serves as a clearinghouse for information on IgA Nephropathy (IgAN), a disorder of the immune system affecting the kidneys and slowly destroying them in nearly 50% of the cases. Over the years the Network has linked patients with IgAN in different communities, sought to stimulate interest in this disorder among health professionals, and lobbied for research into IgAN. In 1993-94, we were successful in securing $2,000,000 from the National Institute of Diabetes, Digestive, and Kidney Diseases for about a dozen research proposals in IgA Nephropathy. None of this would have been possible without the strong support of our membership.

What is IgA nephropathy?
IgA nephropathy is a chronic kidney disease that may progress over a period of 10 to 20 years, and can lead to end-stage renal disease. It is caused by deposits of the protein immunoglobulin A (IgA) inside the glomeruli (filters) within the kidney. These glomeruli normally filter waste and excess water from the blood and send them to the bladder as urine, however, the IgA protein prevents this filtering process. This may lead to:
  • blood and protein in the urine
  • swelling in the hands and feet
What causes IgA nephropathy?
The IgA protein is a normal part of the body's immune system. It is unknown what causes IgA deposits in the glomeruli. Genetic factors may be a cause. How is IgA nephropathy often treated?

Causes, incidence, and risk factors: IgA nephropathy (Berger's disease) is a form of mesangial proliferative nephritis. Inflammation of the renal glomeruli occurs, and there are IgA (a type of antibody ) deposits in the kidney. The disorder can appear as acute , rapidly progressive, or chronic glomerulonephritis ; or as visible or microscopic hematuria blood in the urine
Berger's disease usually is discovered after one or more episodes of dark or bloody urine in a person with no other symptoms of kidney disorder. Bloody urine may begin during or soon after a respiratory infection.

What is IgA nephropathy?
IgA nephropathy is a chronic kidney disease that may progress over a period of 10 to 20 years, and can lead to end-stage renal disease. It is caused by deposits of the protein immunoglobulin A (IgA) inside the glomeruli (filters) within the kidney. These glomeruli normally filter waste and excess water from the blood and send them to the bladder as urine. However, the IgA protein prevents this filtering process. This may lead to the following:
  • blood and protein in the urine swelling in the hands and feet
What causes IgA nephropathy?
The IgA protein is a normal part of the body's immune system. It is unknown what causes IgA deposits in the glomeruli, however, in more than half of families, IgA nephropathy is inherited by an autosomal dominant gene. Autosomal dominant inheritance means that the gene is located on one of the autosomes (chromosome pairs 1 through 22). This means that males and females are equally affected, and "dominant" means that only one gene is necessary to have the trait. When a parent has a dominant trait, there is a 50 percent chance that any child they have will also inherit the trait. IgA nephropathy is a very heterogeneous condition, which means that the presentation varies greatly between individuals and families. In fact, some persons who inherit the gene may not have any symptoms, but could pass the gene on to the next generation. Many times men are affected more often than women.

Question:
A good friend of mine has been diagnosed of Berger's IG A glomerulonephritis when she was 8.
Her conditions worsened at the end of 1994. Now she has chronic uremia, and she is under daily
peritoneal dialysis from March 1995. The doctors said that there are small possibilities of transplantation
because the disease can affect the new kidney too. A Medline search showed us that
in this disease the survival of the transplanted kidney can reach 66%.
We would like to have an expert opinion about this case.
She is 32, 44 kg, high blood pressure.
We can give any further needed infos, lab findings, etc.
Thank you very much for your assistance. Answer: Physicians in the United States do not hesitate to transplant individuals with IgA Nephropathy. James V. Donadio, M.D., and Joseph P. Grande, M.D., Ph.D. IgA nephropathy is a relatively newly recognized disease, first described by Berger and Hinglais in 1968. It is now generally known to be the most common form of primary glomerulonephritis throughout the world. Primary IgA nephropathy is an immune-complex–mediated glomerulonephritis defined immunohistologically by the presence of glomerular IgA deposits accompanied by a variety of histopathologic lesions. Although primary IgA nephropathy receives the most attention, many other diseases are also associated with glomerular IgA deposits ( Table 1 ). The most common of these is Schönlein–Henoch purpura. This condition may indeed be indistinguishable from primary IgA nephropathy and may represent a systemic form of the disease process.

To the Editor: In their review article, Donadio and Grande (Sept. 5 issue) emphasize the favorable effects of corticosteroids in IgA nephropathy. Unfortunately, a considerable number of cases of IgA nephropathy remain undiagnosed until an irreversible decline in renal function has occurred. As pointed out by the authors, IgA nephropathy typically becomes symptomatic when there is an episode of macroscopic hematuria, often accompanied by loin pain. What is IgA nephropathy?
Glomerulonephritis is the term used for a group of diseases which damage the glomeruli (the filtering system of the kidney). In some of these the body’s own immune system attacks the glomeruli and causes inflammation and scarring. There is still much to be discovered about the underlying causes of glomerulonephritis. It is the commonest cause of kidney damage and failure of kidney function.
There are many different types of glomerulonephritis but IgA nephropathy is the most common serious form in Australia. IgA nephropathy is a chronic kidney disease that may progress over a period of 10 to 20 years, and can lead to end-stage renal disease.
Why is it called IgA nephropathy?
As a French doctor, Jean Berger, was the first to describe the condition, IgA nephropathy is sometimes called Berger’s disease. IgA refers to the antibody IgA (Immunoglobulin A) which is derived from the lining of the throat, air passages and intestine and is found in the kidney associated with kidney damage. Nephropathy simply means kidney disease.
