Can blood pressure be too low?
Optimal blood pressure is generally defined as 120/80. Low blood pressure (hypotension) generally refers to numbers less than 100/60. Low blood pressure is rarely a problem and in most cases it is beneficial. Studies show that the lower your blood pressure, the lower your risk of cardiovascular disease. Low blood is only a concern if you have symptoms such as dizziness, weakness, fatigue, or palpitations. Many people with normal (or even elevated blood pressure) occasionally experience a temporary drop in blood pressure upon rising quickly. This form of low blood pressure, called orthostatic hypotension, may cause you to feel momentarily faint, or light-headed, but quickly passes. If you experience orthostatic hypotension frequently you may want to take precautions against falling, such as getting up more slowly and holding on to a support.

    This document has been prepared for those who have requested further information about neurally mediated hypotension. What is neurally mediated hypotension? Neurally mediated hypotension is also known by the following names: the fainting reflex, neurocardiogenic syncope, vasodepressor syncope, the vaso-vagal reflex, and autonomic dysfunction. Hypotension is the formal medical term for low blood pressure, and syncope is the term for fainting. Neurally mediated hypotension occurs when there is an abnormal reflex interaction between the heart and the brain, both of which usually are structurally normal. When does neurally mediated hypotension lead to symptoms? Neurally mediated hypotension occurs in susceptible individuals in the following settings:
    • after prolonged periods of quiet upright posture (such as standing in line, standing in a shower, or even sitting up for long periods),
    • after being in a warm environment (such as in hot summer weather, a hot crowded -corn, a hot shower or bath)

    48 YO male with upper central abdominal pain (directly in the stomach), nausea and a history of GERD (under control with Prilosec.)Pain seems somewhat unrelated to food intake but I’m not sure as I’m taking acetaminophen. Have not seen a G.E. yet but have an appointment.
    All blood work has been normal and H Pylori is negative, but GP doc says there still could be an ulcer. He says gastric cancer is very unlikely and that he would expect to see signs of metastatic tumors in the blood work (is this right?) and other organ functions are normal. How reliable an indicator is liver/ pancreas function in a case like this? He also said that stomach cancer doesn’t typically present with pain, which I found bizarre given what I have read.
    I had a hypotension episode on arising (lost consciousness) that my GP thinks is unrelated and was a result of the Seroquel I’m taking. But that day I started having black stools and took a sample in the next day and it was negative for blood. This all seems too coincidental to me. Could there have been a hemorrhage that significant that has clotted and now is not bleeding? He says a hemorrhage bad enough to cause a blackout would be easy to detect. But why the black stools?

    Dear Dr. Dave and Dr. Dee,
    I read your discussion of vertigo and wonder if that is what I’m experiencing. When I get up, say from a chair, I feel dizzy, start seeing black or black dots, and my legs sometimes feel like they are going to give out. Sometimes I have to lean up against something to keep from falling down. I do have trouble with my balance, although there is no spinning. Is this vertigo?

    Dear Having Dizzy Days,
    Although vertigo can be triggered by standing, it's less likely to be vertigo if there's no sensation of the body, or surroundings, moving or spinning. There is another type of dizziness upon standing that's caused by postural hypotension. This type of dizziness, faintness or light-headedness results from a decrease in blood flow to the brain, due to a drop in blood pressure upon standing up.
    At one time or another, we’ve probably all experienced mild postural hypotension from standing up too quickly. As long as it occurs only occasionally, there’s little cause for alarm.
    But chronic postural hypotension can be a debilitating condition, associated with increased risk of injury from accidents and falls, and obviously is a cause for concern. Persistent light-headedness or faintness upon standing may be a symptom of a serious underlying disorder. Anyone with this problem should be seen by their physician.
