    Hypospadias
    Hypospadias is a condition in which the end of the urethra (the tube in the penis through which urine passes out of the body) is not located at the tip of the penis. The condition occurs in approximately 1 out of 200 newborns. The cause of this condition is not known and is usually an isolated congenital defect that is not associated with any other defects in other organs. In more than two-thirds of cases, the defect is located near the end of the penis where the abnormal opening is located either within the glans (the "head" of the penis) or just below the glans on the underside of the penile shaft. In more severe forms of hypospadias, the opening is located further down on the penile shaft or in the scrotum. Hypospadias is also associated with penile curvature in which the penis curves in a downward direction. This is known as "chordee." In general, the more severe the hypospadiac defect, the more severe the chordee. Usually, hypospadias is associated with an incomplete foreskin such that this condition is usually recognized when a newborn is examined. However, in some cases, hypospadias can occur with a full foreskin and is only recognized at the time a circumcision is being performed or after the circumcision has been completed.

What is hypospadias?
Hypospadias is a male birth defect in which the opening of the tube that carries urine from the body ( urethra ) develops abnormally, usually on the underside of the penis. Depending on where the urethral opening occurs, the four types of hypospadias are:
  • Glanular hypospadias . The urethral opening develops just below the end of the penis. This is the most common type of hypospadias. Coronal hypospadias . The urethral opening develops in the groove where the head of the penis (glans) meets the shaft. Penile shaft hypospadias . The urethral opening develops anywhere along the underside of the shaft of the penis. Perineal hypospadias . The urethral opening develops below the shaft of the penis, usually in the scrotum. This is the least common type of hypospadias, and is quite rare.
Hypospadias is a rare disorder, affecting only about 1 of every 250 live male births.

What is hypospadias? Hypospadias is a common birth defect of the penis. Usually, the urethral opening (the opening of the tube that carries urine out of the penis) appears at the very tip of the head (or glans) of the penis. In hypospadias, the opening can appear anywhere on the underside of the penis. In a mild case, the opening may be just below the head of the penis (coronal). In more severe cases, the opening can be anywhere from the middle of the underside of the penis (mid-shaft) to below the place where the penis and scrotum meet (perineal). Frequently there is a downward curving of the penis called chordee. This curvature may be more pronounced when the penis is erect. The foreskin is also incomplete and has the appearance of a dorsal hood (skin covering only the top and sides of the head of the penis). How often does hypospadias occur? This is a relatively common condition that occurs in approximately one out of every 300 male births. Most cases of hypospadias (approximately 90%) are of the milder type. What causes hypospadias?

Hypospadias is a common congenital anomaly, characterized by incomplete fusion of the urethral folds during fetal development, which results in the urethra opening on the ventral surface of the penis or on the scrotum. In their article, Pierik et al. (2004) proposed multiple possible maternal and paternal risk factors related to the development of isolated hypospadias, including genetic, endocrine, and environmental factors. Concerns and alarms have been raised about the potential effects of endocrine disruptors, which include derivatives of poly-aromatic hydrocarbons and pesticides, on the developing male reproductive tract. We have recently published two case-control studies on risk factors and hypospadias, one on a possible association with maternal age (Bianca et al. 2005) and the other on the role of endocrine disruptors (Bianca et al. 2003).

Understanding the normal penis and urethra The urethra is the tube that urine flows through out of the bladder. It passes through the penis. The opening of the urethra (meatus or 'pee-hole') is normally at the end of the penis, partly covered by the foreskin. What is the penis like in hypospadias?
The main problem is that the urethra opens on the underside of the penis instead of at the end of the penis. The opening can be anywhere from just below the normal position (mild) to as far back as the base of the scrotum (severe). Hypospadias may also include the following.
  • A hooded appearance of the foreskin. This is because the foreskin does not develop on the underside of the penis.
  • Tightening of the tissues on the underside of the penis (called 'chordee'). This pulls the penis down and it cannot fully straighten. The further back the urethral opening is the more severe the chordee tends to be. Mild hypospadias may not have any chordee.
