Hypopituitarism may be either partial or complete and may result from either pituitary or hypothalamic disease. Its clinical manifestations vary, depending on the extent and severity of the pituitary hormone deficiency. Thus, a patient may present in extremis with acute adrenal insufficiency or profound hypothyroidism, with symptoms indicating a pituitary mass lesion, or with nonspecific symptoms of fatigue and malaise. Most of the hypothalamic-pituitary-target-organ axes are tightly coordinated systems in which hormonal signals from the hypothalamus stimulate or inhibit secretion of anterior pituitary hormones;

We report 12 cases of hypopituitarism diagnosed after the age of 60 (range 63-89, mean 74.9) years over a 10-year period. Aetiology was evident in eight cases, viz. pituitary tumour in six and previous post-partum haemorrhage and giant aneurysm of carotid artery in one each. The presentation was often non-specific with symptoms such as lethargy, pallor, falls, urinary incontinence, confusion, fever and flexion contractures. Subtle clues to the condition were missed in four cases resulting in delayed diagnosis. Although levels of pituitary trophic hormones are generally low in this condition, the thyrotrophin (TSH) was normal in seven cases in the present series. Normal levels of serum sodium and potassium did not exclude cortisol deficiency. Hormone replacement therapy resulted in improved quality of life. Physicians dealing with elderly patients should bear this easily treated condition in mind.

Physical lesions in the region of the suprachiasmatic nuclei, which are the site of a circadian clock, result in abnormal circadian rhythmicity in animals, yet the extent of biological rhythm problems in individuals with anatomic or functional lesions in the hypothalamic-pituitary region are largely unknown. What is hypopituitarism?
Hypopituitarism, also called an underactive pituitary gland, is a condition that affects the anterior lobe of the pituitary gland - usually resulting in a partial or complete loss of functioning of that lobe. The resulting symptoms depend on which hormones are no longer being produced by the gland. Because the pituitary gland affects the other endocrine organs, effects of hypopituitarism may be gradual or sudden and dramatic.
What are the symptoms of hypopituitarism?
Symptoms vary depending on what hormones are insufficiently produced by the pituitary gland. The following are common symptoms associated with reduced production of certain hormones: insufficient gonadotropins production
(luteinizing hormone and follicle-stimulating hormone)
In premenopausal women, this leads to absent menstrual cycles, infertility, vaginal dryness, and loss of some female characteristics. In men, this deficiency leads to impotence, shriveling of testes, decreased sperm production, infertility, and loss of some male characteristics. insufficient growth hormone production This usually produces no symptoms in adults. In children, this deficiency can lead to stunted growth and dwarfism.

What is Hypopituitarism ?
How does Hypopituitarism occur ?
Apart from the tumours mentioned above, other causes of pituitary damage include severe bleeding from the womb after childbirth or damage from other diseases or tumours near near the pituitary gland. Sometimes the pituitary gland just shrinks, especially with advancing age. Reduced pituitary function may also follow either surgery for tumours near the pituitary or result from radiotherapy to the brain for another condition.
Why does Hypopituitarism occur ?
The cause of pituitary tumours is not known. Severe bleeding at childbirth is now rare. When it occurs, however, blood is drained away from the pituitary which then shrinks.
Treatment Involved for Hypopituitarism
During Treatment for Hypopituitarism
Regular appointments to see the specialist for check blood test and sometimes x-rays or scans are usually required. A change of tablet doses is usually needed from time to time. Tablets should be carefully checked after each visit to the pharmacist. Any stress (emotional, accident or infection) calls for a bigger dose of hydrocortisone (or other steroid): double the usual dose for mild problems, triple it for severe conditions, especially with fever. You must contact your general practitioner (or the nearest casualty department) if unwell, particularly with vomiting. It may be that an urgent injection of steroid is then required. A bracelet or pendant identification is a good idea. It needs to state that you are taking cortisone . Remember: steroids (cortisone and similar drugs) should never be stopped unless specifically advised by a specialist that this is safe.
