Partial or complete deficiency of one or several pituitary hormones due either to primary pituitary disease or disorder of the hypothalamus interfering with control of the pituitary. Causes include: Pituitary and parapituitary tumours Cranial radiotherapy Pituitary infarction (apoplexy) Infiltration of the pituitary gland (e.g. sarcoidosis) Empty sella (enlarged pituitary fossa seen on MRI) Infection (TB, abscess) Isolated deficiency of hypothalamic releasing hormone Trauma - surgery, head injury Sheehan's syndrome (haemorrhagic infarction of the enlarged postpartum pituitary due to severe hypotension usually associated with blood loss in postpartum haemorrhage - now rare) Epidemiology Rare. Pituitary tumours have an annual incidence of 0.2-2.8 per 100,000 persons. Presentation Clinical features depend upon the rate of development of deficiency and its severity. In most cases it follows a typical order: firstly GH then gonadotrophins, then TSH and finally ACTH later. Prolactin deficiency rare excepting Sheehan’s syndrome, ADH is seen, but only rarely, with infiltrative disorders and trauma.

    What is hypopituitarism?
    Hypopituitarism, also called an underactive pituitary gland, is a condition that affects the anterior lobe of the pituitary gland - usually resulting in a partial or complete loss of functioning of that lobe. The resulting symptoms depend on which hormones are no longer being produced by the gland. Because the pituitary gland affects the other endocrine organs, effects of hypopituitarism may be gradual or sudden and dramatic.
    What are the symptoms of hypopituitarism?
    Symptoms vary depending on what hormones are insufficiently produced by the pituitary gland. The following are common symptoms associated with reduced production of certain hormones: insufficient gonadotropins production (luteinizing hormone and follicle-stimulating hormone) In premenopausal women, this leads to absent menstrual cycles, infertility, vaginal dryness, and loss of some female characteristics. In men, this deficiency leads to impotence, shriveling of testes, decreased sperm production, infertility, and loss of some male characteristics. insufficient growth hormone production This usually produces no symptoms in adults. In children, this deficiency can lead to stunted growth and dwarfism.

In children hypopituitarism may be caused by the presence of tumours, such as craniopharyngioma , optic chiasm or hypothalamic glioma, or germinoma. Unlike adults hypopituitarism secondary to pituitary microadenoma or macroadenoma is rare in childhood. Hypopituitarism may also be seen after surgery particularly for lesions in the suprasellar cistern. Congenital abnormalities, such as pituitary hypoplasia or aplasia may also occur. Pituitary aplasia is usually fatal at birth. Hypoplasia results in varying degrees of hypopituitarism including adenohypophyseal and neurohypophyseal deficiencies. Many of these patients are found to have a small pituitary gland and fossa and the posterior pituitary may be in an ectopic position, usually in the region of the pituitary infundibulum or hypothalamus. These anomalies may only be detected by high resolution coronal and sagittal MR imaging of the pituitary region. An ectopic pituitary is seen as a bright spot in the region of the tuber cinereum on sagittal T1-weighted sequences with concomitant absence of the normal posterior pituitary bright spot in the pituitary fossa. The pituitary fossa may be small and appear empty, an 'empty sella'. The condition may be associated with

    HYPOPITUITARISM (underactivity of the pituitary gland)
    Causes
    Underactivity of the pituitary gland may result from several different causes. There may be a "congenital" (present at birth) abnormality in the formation of the gland or a congenital tumour (a "craniopharyngioma") in the region above the gland. Various diseases may damage the gland or the function may be disturbed by a head injury or as a side effect of treatment with radiotherapy to the head which may be needed for the treatment of leukaemia and some related conditions.
    Diagnosis.
    Diagnosis depends on the measurement of pituitary hormones in the blood either in the resting state or after stimulation tests to show more clearly the capacity of the gland. Scanning of the head, usually with an MR (magnetic resonance) scan can show the structure of the gland well.
    Treatment.
