Definition: Hypoparthyroidism is a deficiency of parathyroid hormone that causes abnormal metabolism of calcium and phosphorus.
Causes, incidence, and risk factors: Calcium and phosphorus are important body minerals. They form the mineral component of bones, and they exist as charged particles called ions in the blood and inside cells. Parathyroid hormone (PTH) regulates the amount of calcium and phosphorus in bone and blood. The four small parathyroid glands located in the neck, behind the thyroid, make PTH. Hypoparathyroidism is the clinical condition caused by a lack of PTH. Blood calcium levels fall, and phosphorus levels rise. Low blood calcium levels may cause symptoms such as tingling of the lips, fingers, and toes, and muscle cramps or spasms. The most common cause of hypoparathyroidism is injury to the parathyroid glands during head and neck surgery. Rarely, hypoparathyroidism is a side effect of radioactive iodine treatment for hyperthyroidism . PTH secretion may be impaired when blood levels of magnesium are low or when blood pH is too high, a condition called metabolic alkalosis.

My 7 year old golden retriever was admitted to the Pet E.R. last night, was diagnosed with hypocalcemia within one hour, and treatment was begun immediately. Blood tests were sent off today to the U. of Michigan to confirm the working diagnosis of hypoparathyroidism.
For several weeks he had been licking/chewing at his toes off and on, and several days ago began rubbing his face vigorously with his paws. Two days ago he would limp occasionally -each time on a different leg and but then could run full speed in between the limping episodes. Then, suddenly last night he fell to the ground and had strange posturing of his legs with yelping. Rushed him to regular vet who diagnosed seizures and sent him to the Pet E.R.
We were so lucky. The vet in the E.R. had done a paper in vet school on hypocalcemia, and she said the KEY for her was hearing about the face rubbing, that it is very characteristic of this disease. Same for the toe biting.

    HYPOPARATHYROIDISM
    What is Hypoparathyroidism ?
    The parathyroids are four tiny glands, each the size of a rice grain. They are situated, two on each side, behind the thyroid gland which sits low in the neck. Their function is to regulate the calcium level in the blood. They do this by producing a hormone (parathormone) which withdraws calcium both from the bones as well as from food in the bowel. In hypoparathyroidism (hypo = deficient, parathyroidism = parathyroid activity), an absence or underactivity of these glands allows the calcium level in the blood to fall too low. This may produce no symptoms at all if it is only mild. In more severe cases, a low calcium level makes the nerves of the body very irritable. There may be tingling in the fingers or in the mouth, and spasms may occur in the hands so that the fingers cannot be straightened.
    Why does Hypoparathyroidism occur ?
    In some people, the parathyroid glands undergo an early decay (auto-immune destruction). This disorder is sometimes found in patients who have other endocrine gland damage. The commonest of these is an underactive thyroid gland or adrenal glands. The tendency to this type of glandular damage is inherited. More commonly, the parathyroid glands are damaged at the time of an operation. This may be because they are caught up with the thyroid gland at the time that this is removed by a surgeon for a variety of different reasons. The condition may also occur because the parathyroid glands need to be removed for an overactive parathyroid state. Some patients with this disorder have several glands affected: therefore all of them may need to be removed completely.

    Dear Sir
    Idiopathic hypoparathyroidism is an uncommon condition of unknown etiology. Established criteria for its diagnosis are a) low serum calcium b) high serum inorganic phosphates c) renal insufficiency, steatorrhea, chronic diarrhea and alkalosis must not be present and rickets and osteomalacia must be excluded. In hereditary and acquired hypoparathyroidism, parathyroid hormone (PTH) is either undetectable or in the normal range, and this finding in a hypocalcemic patient is supportive of hypoparathyroidism, as distinct from ineffective PTH action, in which even mild hypocalcemia is associated with elevated PTH levels. Hence a failure to detect elevated PTH levels establishes the diagnosis of hypoparathyroidism, elevated levels suggest the presence of secondary hyperparathyroidism which is more common
    A forty-year-old man presented with spasmodic contractions of upper and lower limbs of the left side, giddiness and ataxic gait of five years duration. The symptoms were insidious in onset and progressive in nature. The frequency of the spasmodic contractions, which occurred once a fortnight gradually increased in frequency to 2-3 times a day lasting for 30-60 seconds. He had no history of injury or surgery, diabetes or hypertension.

    Hypoparathyroidism
    The Parathyroid glands are located next to the thyroid glands, in the neck. However, the thyroid glands and parathyroid glands have nothing to do with each other. The Parathyroid glands, (together with the intestines, kidneys and bones - under the influence of Para Thyroid Hormone, PTH), control very precisely the level of Calcium in the blood. Vitamin D is also required. As usual, Hypo means low. Low levels of PTH lowers the levels of Calcium in the blood, leading eventually to Hypocalcemia. Calcium is used extensively in the body, including nerve conduction and muscle contraction. See Also
