Hyperthyroid Crisis (Thyrotoxic Storm)
The thyrotoxic crisis is an uncommon (~1%) medical emergency caused by an exacerbation of hyperthyroidism characterised by decompensation of one or more organ systems . Early recognition and aggressive treatment are essential. Thyroid storm can occur in patients with a toxic adenoma or multinodular toxic goitre , but is more often seen in patients with Graves' disease Presentation: Often sudden onset. Severe hyperthyroidism with:
  • Hyperpyrexia, dehydration.
  • Heart rate greater than 140 beats per minute, hypotension, atrial dysrhythmias, congestive heart failure
  • Nausea, vomiting, diarrhoea, abdominal pain.
  • Confusion agitation , delirium, psychosis, seizures or coma
Precipitants: Thyroid storm is most often seen in a thyrotoxic patient with intercurrent illness, trauma or emergency surgery. Common precipitants include

  • Recent trauma, including surgical stress.

People who are hyperthyroid tend to lose weight because their body's metabolic rate is faster than normal. In the body's effort to compensate for burning fuel (food) faster, the appetite and, consequently, food intake, increase. This varies from person to person, sometimes taking the form of larger food portions or more between-meal snacks. Also, there is often a preference for more sweets. If consumption does not keep up with the increased metabolism, weight loss occurs. (It is also possible to increase intake so much that a weight gain occurs.) Whatever the therapy, treatment of hyperthyroidism is aimed at lowering the activity of the thyroid gland. Thyroid hormone levels in the blood will then decrease, and the body responds by lowering the rate at which fuel is consumed (metabolism). When recovery from hyperthyroidism occurs and the metabolism becomes normal, the total amount of food needed will decrease.

Parameters that are assumed to be under direct control by thyroid hormonal state were evaluated in amiodarone-treated, hypo- and hyperthyroid rats. Special attention was paid to evaluating similarities between hypothyroid and amiodarone-treated rats, with special focus on myocardial nuclear triiodothyronine (T3) receptor binding characteristics. Rats were rendered hypothyroid by adding KClO4 to the drinking water for 6 weeks (N = 14). Hyperthyroidism was induced by adding 0.003% L-thyroxine to the drinking water for 6 weeks (N = 14). Fourteen rats were treated orally for 6 weeks with amiodarone, whereas 14 rats served as untreated controls. Equilibrium binding characteristics for T3 nuclear receptors were determined by means of a simple and rapid method with excellent reproducibility and sensitivity. Body temperature and heart rate were lowered in hypothyroid and amiodarone-treated rats. Maximum binding capacities of beta-adrenergic receptors were reduced by 39% and 14% (p

While an increased oxygen consumption is accepted as one consequence of hyperthyroidism, only few data are available on the role of anaerobic processes for the increased metabolic activity in this disease. In this study we evaluated the relative importance of anaerobic and aerobic metabolism for the metabolic activity in lymphocytes from patients before and after treatment for hyperthyroidism. Total lymphocyte heat production rate (P), reflecting total cell metabolic activity, was determined in a plasma lymphocyte suspension using direct microcalorimetry. The contribution from aerobic metabolism (O2-P) was calculated from the product of the lymphocyte oxygen consumption rate and the enthalpy change for glucose combustion, and the anaerobic contribution as the difference between P and O2-P. The total lymphocyte heat production rate P was 3.37 +/- 0.25 (SEM) pW/cell (N = 11) before and 2.50 +/- 0.11 pW/cell (N = 10) after treatment for hyperthyroidism (p

I read somewhere in the internet that effect of soy on our body will be for only 8 hrs after it is consumed and we should take soy twice a day,
can people with hyperthyroid who also need lot of isoflavones take soy on alternate days so that effect of soy is got one day and the anti effect of soy on the other day.
