Hypertension Treatment and the Prevention of Coronary Heart Disease in the Elderly

T he aging of the U.S. population has raised concerns about the social and economic resources that may be necessary to manage the growing number of elderly persons with cardiovascular disease. Coronary heart disease is the major cause of death in both elderly men and elderly women. Hypertension, either systolic/diastolic or isolated systolic, is considered a major risk factor for coronary heart disease. Treatment of isolated systolic hypertension or systolic/diastolic hypertension in elderly patients is associated with a reduction in adverse coronary outcomes. As late as the 1970s, the prevailing medical opinion was to ignore elevated blood pressures in the elderly, even though epidemiologic data demonstrated the age-related risk of coronary heart disease as a result of hypertension. Into the 1980s many physicians still did not believe that antihypertensive drugs could improve outcome in the elderly, despite abundant data from several clinical trials.

Only about 5 to 10 percent of hypertension cases are thought to result from secondary causes. The ABCDE mnemonic can be used to help determine a secondary cause of hypertension: A ccuracy of diagnosis, obstructive sleep A pnea, A ldosteronism, presence of renal artery B ruits (suggesting renal artery stenosis), renal parenchymal disease ( B ad kidneys), excess C atecholamines, C oarctation of the aorta, C ushing's syndrome, D rugs, D iet, excess E rythropoietin, and E A PDF version of this document is available. Conceptually, it is useful to think of patients with hypertension as having either essential hypertension (systemic hypertension of unknown cause) or secondary hypertension (hypertension that results from an underlying, identifiable, often correctable cause). Although only about 5 to 10 percent of hypertension cases are thought to result from secondary causes, hypertension is so common that secondary hypertension probably will be encountered frequently by the primary care practitioner.

Diagnosis

Treatment Options
There are two kinds of pulmonary hypertension and the disease and treatments are quite different. If the disease exists by itself and has no underlying cause, it is called primary pulmonary hypertension (PPH). Treatments for pulmonary hypertension include medications and, in rare instances, lung transplant If it is caused by another condition or disease such as emphysema, blood clots to the lung (chronic thromboembolic pulmonary hypertension) or congenital heart disease, it is called secondary pulmonary hypertension . Successful treatment of the primary disease may decrease or cure the pulmonary hypertension. Chronic thromboembolic pulmonary hypertension is a type of secondary pulmonary hypertension that is often missed or misdiagnosed.
