Hyperlipidemia in HIV-infected patients: the protective effect of hepatitis C virus co-infection

Received: 9 May 2002; revised: 11 October 2002; accepted: 21 October 2002. The prevalence of hyperlipidemia and factors related to it have been studied in a large sample of patients. We found a significant association between hyperlipidemia and the duration of antiretroviral therapy, serum cortisol levels, the presence of lipodystrophy, certain antiretroviral regimens and, especially, the absence of hepatitis C virus (HCV) infection. Patients co-infected with HCV had considerably lower rates of hyperlipidemia than non-infected patients, and this effect was particularly evident in patients treated with antiretroviral drugs. The presence of hyperlipidemia in patients taking protease inhibitors (PI) compared with PI-naive patients or HIV-negative controls has been reported as a part of the lipodystrophy syndrome [ ], as well as improvements in lipid abnormalities after switching from PI-based to non-nucleoside reverse transcriptase inhibitor (NNRTI)-based regimens [

However, the relationship between hypothyroidism and increased high-density lipoprotein (HDL) cholesterol levels has not been widely appreciated. O'brien and colegue leagues studied the relationship between both primary and secondary thyroid dysfunction and the ratio of total cholesterol to HDL diolesterol and the ratio of low-density lipoprotein (LDL) cholesterol to HDL cholesterol. Lipid profiles were examined in 295 patients with hypothyroidism. None of the patients were being treated for hyperlipidemia the time of initial evaluation. Two hundred sixty-eight patients had primary hypothyroidism and 27 had secondary hypothyroidism. The ratio of total cholesterol to HDL cholesterol and the ratio of LDL cholesterol to HDL cholesterol were calculated for all patients before and after treatment with thyroid hormone.

Staff Reporter Pharmaceutical companies are vying for a bigger share of the fat-reducing medicine market as a growing number of Koreans are dying from coronary sclerosis. According to statistics, the death toll from coronary sclerosis, a disease mainly resulting from hyperlipidemia, surged from 10 in 100,000 Koreans in 1990 to 25 in 100,000 in 2002. The increase was attributed to Westernized dietary habits and immoderate lifestyles. Hyperlipidemia is an excess of fatty substances called lipids (fat), mostly comprised of cholesterol, in the bloodstream. It is a major risk factor for coronary heart disease, which has been afflicting more Koreans recently. The growing number of deaths by hyperlipidemia are creating a booming market for lipid-lowering agents that reduce the amount of bad cholesterol. The local market value of lipid-lowering agents was 39.2 billion won in 1998, but the pharmaceutical industry expects it to reach 140 billion won this year. Lipid-lowering agents have already been a cash cow for multinational pharmaceutical companies.

Causes
This disease is genetic and inherited, although the specific defective genes have not been identified. The person's cholesterol or triglyceride levels become elevated during the teenage years and continue throughout life. The types of elevated lipoproteins may vary between affected family members. Cholesterol deposits in the skin, called xanthomas, which are seen in other disorders of elevated lipoproteins are rarely seen in this disorder. This disorder predisposes the person to greater risk of early coronary artery disease and therefore, heart attacks. People with the condition have a higher rate of obesity and glucose intolerance. The condition is worsened by diabetes alcoholism , and hypothyroidism . Risk factors are a family history of high cholesterol and early coronary artery disease. This is the most common disorder of increased blood fats that causes early heart attacks. The rare person who gets 2 defective genes is at much higher risk for early heart attack due to very high blood fat (cholesterol or triglyceride) levels.

What is going on in the body? This condition may cause high cholesterol , high triglycerides , or both. These are each two types of fat in the body. Normally, the liver makes a low amount of cholesterol. When a person has this disease, the liver produces too much cholesterol. Other problems occur with fat metabolism, too. These problems depend on which specific abnormal gene was inherited. This condition increases a person's risk of having clogged arteries from atherosclerosis , which may lead to a heart attack stroke , or other complications.
