  • Determine lipoprotein profile (LDL-C, HDL-C, TG, Total Cholesterol) after a 9-12 hr fast. Lipoprotein analysis should ideally be performed when the patient is not in the recovery phase from an acute coronary or other medical event that would lower the usual LDL-cholesterol level. If the first two LDL-cholesterol test results differ by more than 30mg/dL (0.7mmol/L), a third test result should be obtained within 1 to 8 weeks and the average value of the three tests used. Current guidelines by NCEP ATP-III augmented with ADA's update, should prevail in terms of clinical practice

Treatment Introduction: Primary idiopathic hyperlipidemia has been reported to be familial in Miniature Schnauzers and Beagles, and some mixed breeds. It has been seen sporadically in other breeds, e.g. Poodles and Shelties, as well, Infrequently it has been reported in cats. Secondary hyperlipidemia occurs as a result of another disease process. For example, it has been seen in association with diabetes pancreatitis hyperadrenocorticism and hypothyroidism Etiology: This is a metabolically complex disease of lipid -lipoprotein metabolism and the exact etiology is not fully appreciated. The familial type in schnauzers may involve defects lipoprotein lipase and/or Apoprotein C-II , a required cofactor for lipoprotein lipase activity (see figure below for roles of enzyme and cofactors in lipid metabolism). This defect causes a failure to breakdown chylomicrons (most common) and/or VLDL , and results in excessive levels of circulating triglycerides. It is the elevated concentration of triglycerides that is responsible for the clinical signs Classification: Lipids are derived from exogenous chylomicrons ) and endogenous sources. Canine Hyperlipidemia Canine Hyperlipidemia can be a confusing condition and difficult to understand. Compounded on top of this there is limited information available explaining the condition in canines, even in Miniature Schnauzers were it occurs very frequently. It can be a serious, if not a life threatening condition and must be addressed by your vet. I will be giving basic information to enable you to approach your vet and discuss hyperlipidemia and to ask questions, especially as it may pertain to a diabetic dog. Since Miniature Schnauzers are predisposed to hyperlipidemia (and since Tyler is also a MS) my focus will be directed toward Miniature Schnauzers. However, any breed of dog or cat can acquire certain types of hyperlipidemia. It is an important factor to consider and deal with when other diseases are present. With Tyler it has effected his diabetes regulation, has caused additional eye problems and other problems for him. I will discuss these problems at the end of this article. Hyperlipidemia is a condition in which the amount of fats (lipids) in the blood are elevated. This includes both triglycerides and cholesterol being too high . Hyperlipidemia occurs normally for a short time after a meal, then returns to the correct level. However, some dogs have a metabolic problem which prevents the proper cleaning of the fat from the blood stream, resulting in the high lipids. To be clear, there are two other separate conditions; hypertriglycerides (high triglycerides only) and hypercholesterolemia (high cholesterol only). These conditions are not discussed here.

What is hyperlipidemia? Lipid is the scientific term for fats in the blood. At proper levels, lipids perform important functions in your body, but can cause health problems if they are present in excess. The term hyperlipidemia means high lipid levels. Hyperlipidemia includes several conditions, but it usually means that you have high cholesterol and high triglyceride levels. High lipid levels can speed up a process called atherosclerosis, or hardening of the arteries. Your arteries are normally smooth and unobstructed on the inside, but as you age, a sticky substance called plaque forms in the walls of your arteries. Plaque is made of lipids and other materials circulating in your blood. As more plaque builds up, your arteries can narrow and stiffen. Eventually, enough plaque builds up to reduce blood flow through your arteries. Atherosclerosis increases your risk of heart disease, stroke, and other vascular diseases. Fortunately, you may be able to reduce high lipid levels and, therefore, prevent or slow the progression of atherosclerosis. Lifestyle changes like exercising and eating a healthy diet can lower your lipid levels and are often the first step in treatment.

Introduction
Many adults, as they get older, tend to experience an increase in their lipids (hyperlipidemia). "Lipids" is the general term for fats in the bloodstream, most notably cholesterol and triglycerides. Many HIV-positive people are also experiencing increases in their blood lipids. While this shouldn't come as a surprise, given that HIV-positive people are now living longer, it's also clear that many HIV-positive people are experiencing profound increases in their cholesterol and triglyceride levels as a result of the anti-HIV medications they are taking. This has raised fears that HIV-positive people taking these medications are at a higher risk of experiencing a heart attack or a stroke as a result of their hyperlipidemia . But we're also learning how best to manage these increased lipid levels, through delaying anti-HIV treatment until it's absolutely necessary, switching treatments when problems arise, initiating dietary changes and exercise programs, and prescribing lipid-lower drugs to help bring cholesterol and triglyceride levels under control. Switching to atazanavir improves antiretroviral-induced hyperlipidemia Last Updated: 2005-06-28 14:38:04 -0400 (Reuters Health) NEW YORK (Reuters Health) - Switching to atazanavir from other protease inhibitor-based therapy can improve antiretroviral-based metabolic disorders in patients with HIV infection, according to a report in the June 1st issue of the Journal of Acquired Immune Deficiency Syndromes. "Atazanavir is a very good option for patients with hyperlipidemia and a high-risk profile for cardiovascular events," Dr. Ulrike Mobius from Hannover Medical School, Germany, told Reuters Health. Dr. Moebius and colleagues investigated the changes in lipid profiles and the efficacy of atazanavir-containing regimens in 33 HIV-infected patients with severe hyperlipidemia, 26 of whom were on a protease inhibitor-containing regimen before switching to atazanavir. Patients experienced a 45.6% decrease in triglycerides and a 17.9% decline in total cholesterol after 24 weeks of atazanavir treatment, the authors report. Fasting levels showed even greater decreases (50% and 20%, respectively). Non-HDL cholesterol levels decreased 21.7% in the whole study population, the report indicates, and fasting levels declined 23%.
