Understanding the H Word...
We are all born with a genetic palate. Some of us were born with a tendency towards certain diseases such as heart, diabetes, certain cancers, circulatory diseases, high blood pressure, stroke, yo-yo weight gain & a variety of addictions from alcohol to smoking to food. This "tendency towards" is a biochemical factor that can cause you to either over-produce a hormone known as insulin or develop a resistance to it. Most people have both problems....over-production followed by resistance on the cell borders.
Insulin is important to life & we must have it to live. However, if the pancreas wears out from the over-production because of the ineffectiveness of the resistant cell borders, then what started out as a tendency towards has moved into hyperinsulinemia or reactive hypoglycemia, and is on it's way to adult diabetes. In a way the disease term "adult diabetes" is a misnomer, because we are now seeing children diagnosed with this killer disease at early ages.

The metabolic syndrome is present when 3 or more of the 5 criteria are met Since the introduction of the concept of syndrome X, considerable new information has evolved relevant to the role of insulin resistance in human disease. This has resulted in two somewhat disparate approaches to thinking about the clinical implications of insulin resistance and its consequences. One view represents an effort to acknowledge that the abnormalities related to insulin resistance have broadened considerably, and the adverse clinical outcomes extend beyond type 2 diabetes and CVD. Since CVD is recognized to be just one of the multiple clinical syndromes associated with insulin resistance, it seems appropriate to replace the term syndrome X with one that incorporates this new information. In this context, the Insulin Resistance Syndrome (IRS) seems to be a reasonable choice to provide a pathophysiological construct with which to view the different clinical syndromes that occur more commonly in insulin resistant individuals. Based upon the above considerations, it is apparent that the concept of the IRS is much broader than that of the ATP III definition of the metabolic syndrome, and not limited to consideration of CVD. Consequently, the focus of this review will be on the abnormalities and clinical syndromes that can usefully be considered under the general heading of the IRS; an approach that will presumably be of greater relevance to the readers of ENDOTEXT.

Last Updated: 2005-08-08 16:00:31 -0400 (Reuters Health) NEW YORK (Reuters Health) - Moderately high insulin levels are associated with increased levels of inflammatory markers and beta-amyloid in cerebrospinal fluid, suggesting a causal link between hyperinsulinemia and Alzheimer's disease, investigators report. Hyperinsulinemia and insulin resistance are recognized risk factors for Alzheimer's disease, Dr. Suzanne Craft and her team note in their report in the October issue of the Archives of Neurology. They theorized that insulin's effects on the inflammatory network mediate its role in Alzheimer's disease. "It is well known that high levels of insulin increase inflammation in the body, so we attempted to determine whether or not high insulin levels also increase inflammation in the brain and the central nervous system," Dr. Craft, of the University of Washington, Seattle, explained in an interview with Reuters Health. To test their theory, they recruited 16 healthy subjects ages 55 to 81 years. Subjects were infused with insulin, along with dextrose to maintain euglycemia, to achieve plasma insulin levels observed in insulin resistance. Plasma and CSF were collected after infusions of approximately 105 minutes.

An increasing number of American children and adolescents are overweight, but children in the North Carolina public health settings are almost twice as likely to be overweight as the national average. There is also growing evidence that overweight children may develop hyperinsulinemia and other metabolic complications observed in adults with 'syndrome X' and the insulin resistance found in type 2 diabetes mellitus. At present, the prevalence of hyperinsulinemia is unknown. A recent study, KIDPOWER, is being conducted in a North Carolina community. This study is attempting to identify clinical correlates associated with childhood overweight, as well as to develop and test lifestyle interventions with children and adolescents identified as having hyperinsulinemia. Investigators on this study describe the prevalence of marked hyperinsulinemia and its clinical correlates among the initial group of children and adolescents referred for lifestyle intervention to KIDPOWER by primary care providers.

WHAT IS INSULIN?
Insulin is a substance, a hormone made by the pancreas, an organ located behind the stomach in the belly. In a simplified sense, insulin is responsible for two major functions: disposing of food and maintaining normal blood sugar levels.

How does hyperinsulinemia develop?
Hyperinsulinemia is almost always caused by an individual’s diet. When a person eats, the body breaks down the food into usable energy. Insulin is a hormone secreted by the body to transport glucose into the cells for their energy source. When the cells no longer respond properly to insulin, this forces the body to produce more insulin to get the necessary response. When the body's cells are resistant to the action of the insulin, it is called insulin resistance (IR). As a result, the pancreas produces much more insulin than normal. Over time this can cause the pancreas to become exhausted and unable to keep up with the body’s demand for insulin, leading to hyperinsulinemia. The condition may also be the result of a tumor of the pancreas, called an insulinoma, which secretes excess insulin. Hyperinsulinemia
Insulin resistance and hyperinsulinemia (elevated insulin levels) are found in approximately 50% of obese (defined as 20% over ideal body weight) PCOS women. Hyperinsulinemia promotes hyperandrogenism (elevated male hormone levels) in PCOS by increasing circulating ovarian androgens and reducing serum sex hormone binding globulin (a protein which binds circulating male hormones). These actions result in increased circulating free testosterone (male hormone) levels.
This condition often results in chronic anovulation (lack of ovulation) and subsequently infertility. Although insulin resistance and hyperinsulinemia correlate with body mass index (BMI), they may also be found in non-obese PCOS patients.
