It has affected me emotionally and mentally. But, I read some articles about a cure for it. One is an injection of Botox, and the other is ETS (cutting of a nerve node). I was wondering which way should I go and what's the cost of each one? Thanks. Embarrassed to shake hands Dear Embarrassed to shake hands, This classic Alice! answer to "Walking Sponge," a past Go Ask Alice! reader with similar sweating concerns, has information that you may find helpful in your search for a solution to your excessive sweating. It has updated information, including drug and surgical treatments. Dear Walking Sponge, Call the Health Service at x4-2284 to make an appointment with a health care provider to make sure that what you have is primary hyperhidrosis (rather than an underlying condition that causes profuse perspiration, known an secondary hyperhidrosis, which would require treating the underlying condition first, often resolving the abundant sweating as well). Use this opportunity to get more information and to discuss your treatment options, their possible side effects, and their contraindications. If you have primary hyperhidrosis, your provider may recommend the following treatment possibilities depending on how severe it is:

Hyperhidrosis and Endoscopic Thoracic Sympatectomy (ETS) H yperhidrosis is characterized by excessive sweating that occurs in up to 1% of the population. The excessive sweating can occur in the hands (palmar hyperhidrosis), in the armpits (axillary hyperhidrosis), in the feet (plantar hyperhidrosis) or in the face. No one understands the exact cause of this excessive sweating in specific individuals, but it is known that sweating is controlled by the sympathetic nervous system Although physicians are researching the cause of hyperhidrosis, it is unknown why excessive sweating occurs in certain individuals. We are uncertain whether it is the over activity of the sympathetic nervous chain or the sweat glands. As you know, hyperhidrosis, excessive sweating, can hamper daily activities. It is sometimes brought on by stress, emotion, or exercise, but can also occur spontaneously. The initial treatment for hyperhidrosis is usually medical. There are ointments that are astringents that dry up the sweat glands. Another treatment is iontopheresis. This consists of electrical stimulation to the hands. Patients place their hands in a bath through which an electrical current is passed. This treatment stuns the sweat glands and can decrease the secretion of sweat for periods of 6 hours to one week. One of the newest treatments proposed is the injection of botulinum toxin (Botox) into the area of excessive sweating. The toxin affects nerve endings to decrease the transmission of the nerve impulses to the sweat glands. This results in decreased sweating. It generally requires several injections in the palms or underarms and it remains effective for one to six months. Repeated injections are nearly always required to maintain an adequate level of dryness.

Excessive armpit sweating information and surgical treatment from Dr. Alon Pnini. Based in Los Angeles, California.

Armpit / Underarm Sweating
The presence of axillary (armpit) sweat and odor is normal in all people past puberty. Excessive armpit sweating, also known as Axillary hyperhidrosis , represents a variant of normal axillary sweat. Similarly, axillary bromhidrosis (odor) represents a variation or accentuation of normal axillary odor. These can be distressing problems, with unpleasant sweat and smell and sometimes causing an accompanying social handicap for people who suffer from it. Topical deodorants and antiperspirants may only have a limited effect and are not a permanent solution. Dr. Alon Pnini M.D , a board certified plastic and reconstructive surgeon, offers a surgical alternative to excessive armpit sweating. The surgery, known as Retrodermal curettage . is a "new-old" approach to the problem using an advanced treatment with a success rate of over 90%! Dr.Pnini's procedure is performed on an outpatient basis under general anesthesia. Following recovery the patient is then discharged and has a post-operative visit within 1 to 2 days following the procedure. The dressing is removed 24 to 48 hours after the surgery. Learn more about the surgery This surgery is a Non ETS surgery and should not be confused with the Endoscopic Thoracic Sympathectomy approach which does not have the same results as this very specific surgery. ETS may be the preferred procedure if you suffer from more than just excessive armpit sweating. Its important to note the difference between the two procedures.

P almoplantar hyperhidrosis is a common condition in which the eccrine (sweat) glands of the palms and soles secrete inappropriately large quantities of sweat. The condition may become socially and professionally debilitating. Idiopathic palmoplantar hyperhidrosis begins in childhood and frequently runs in families. Eccrine glands are distributed over almost all of the body surface but are most dense in the palms and soles. These glands are morphologically and functionally normal in patients with palmoplantar hyperhidrosis.

(Requires Real Player) Hyperhidrosis is a condition in which a person experiences excessive sweating in their hands (palmar hyperhidrosis), underarms (axillary), face and/or feet (plantar). Everyone perspires more when they get nervous or excited or when they exercise, but people with hyperhidrosis perspire excessively, often for no apparent reason.
The sympathetic nervous system, which controls involuntary responses such as sweating, blushing, and salivation, simply fails to regulate sweating in the body’s extremities. Consequently, people with hyperhidrosis often experience sweating so severe that it becomes a source of embarrassment, making them reluctant to shake or touch hands. It also can interfere with everyday activities such as writing, driving, taking tests, making presentations or even holding or grasping objects.
The severity of the condition varies from person to person, but in most cases the sweating proves problematic both socially and professionally. The prevalence of hyperhidrosis is not well-documented, but it is estimated to affect up to one percent of the U.S. population. People with hyperhidrosis tend to have a family member with symptoms as well.

Every day, the human body uses perspiration to maintain constant internal body temperature. Perspiration is regulated by the Sympathetic Nervous System, which controls about five million sweat glands in the body, with about half of these being located in the hands. Sweating in the right hand is controlled by branches of the right sympathetic chain that is located within the right chest cavity. Sweating in the left hand is controlled by branches of the left sympathetic chain that is located in the left chest.
In some people, natural perspiration is excessive. This condition is known as hyperhydrosis . The most common areas where hyperhidrosis occurs are the hands, face, feet, and axilla, causing sweaty palms, sweaty feet, sweaty armpits, and a sweaty face. The condition often begins in adolescence but can occur in people of all ages. Its cause is unknown but there seems to be a genetic tendency.
Newport Beach Hyperhidrosis Clinic has partnered with the surgeons of Raney Zusman Medical Group to provide surgical treatment for excessive sweating. Surgical treatment has proven to be an effective method for decreasing sweating in patients who are not responsive to medications or other therapies.
Hyperhidrosis
People with excessively sweaty palms sweaty feet or facial blushing (hyperhidrosis) often seek treatment because the condition causes great distress and may impair quality of life causing numerous psychological, educational, and occupational problems.

A Cure for Excessive Hand and Foot Perspiration The Hyperhidrosis Program at Cedars-Sinai provides a leading-edge permanent cure for excessive sweating of the hands and feet. The procedure offers a 98% chance of cure for hyperhidrosis. Our surgeons, who have performed over 1,000 procedures, are among the world's most experienced with this special procedure. Hyperhidrosis, an overproduction of perspiration in the hands, feet and sometimes the armpits, affects one in 200 Americans. It prevents these otherwise healthy people from doing the things they would like for fear of embarrassment or simply an inability to grasp items firmly. People with hyperhidrosis often:
  • Are embarrassed to give a simple handshake Are afraid to touch another person or hold hands Smudge papers when handling them Have anxiety or difficulty concentrating Wash or wipe their hands constantly Are unable to hold and swing a golf club or baseball bat Are uncomfortable or unable to play a musical instrument Have wet hair and burning eyes due to sweat Have difficulty applying and wearing makeup Have difficulty with a necktie Hide their hands under the dinner table Are embarrassed to handle documents or other items in public
Cedars-Sinai physicians have pioneered research and published articles on modern endoscopic techniques. At the Hyperhidrosis Program, the surgical team uses smaller instruments (a three-millimeter compared to a 10-millimeter endoscope), allowing for smaller incisions, which cause less pain and offer a shorter recuperation period.

The most common therapies for hyperhidrosis include lotions, antiperspirants, oral medications and biofeedback. While these treatments can help many people with hyperhidrosis, they do not work for everyone and their effectiveness can even decrease over time.
New Treatment for Relief
Now, a minimally invasive procedure to cure hyperhidrosis and facial blushing is available at the American Sympathectomy Institute
Until recently a surgical procedure had not been extensively used for the treatment of hyperhidrosis due to the risks involved with traditional or open surgery. However, with a new procedure known as thoracoscopic sympathectomy , or endoscopic transthoracic sympathectomy (ETS), it is now possible for surgeons to locate and cut the right portion of the sympathetic chain with only very small incisions in fact, less than one inch on each side. Using a fiberoptic camera and small surgical instruments, surgeons can perform this relatively simple operation. Most patients require just an overnight stay in the hospital and benefit from less discomfort and a faster recovery.

Hyperhidrosis is a condition that causes abnormal excessive sweating of the hands, armpits, face and scalp. It causes daily embarrassment and affects the quality of one's life. It can affect relationships or career choices. Those who do not have the condition "just don't get it." Facial Blushing Abnormal facial blushing or erythrophobia is a condition that causes an individual to go bright red in response to a non or minimally embarrassing situation eg meeting up a friend at the supermarket, when facing one's boss or friend. It can cause individuals to become completely socially isolated in an effort to avoid embarrassing situations. "Just don't blush is not a solution!" Many people suffer from an equally debilitating flushing of the neck and upper chest which is understandably particularly a problem in young women who would like to wear "V" neck dresses. We have investigated novel non-surgical treatments for this particular problem.
