Horner Syndrome Definition: A syndrome associated with defective sympathetic innervation to one side of the face, including the eye. Clinical features include MIOSIS ; mild BLEPHAROPTOSIS ; and hemifacial HYPOHIDROSIS (decreased sweating)(see HYPOHIDROSIS ). Lesions of the BRAIN STEM ; cervical SPINAL CORD ; first thoracic nerve root; apex of the LUNG CAROTID ARTERY CAVERNOUS SINUS ; and apex of the ORBIT may cause this condition. General Discussion Horner syndrome is a relatively rare disorder characterized by a constricted pupil (miosis), drooping of the upper eyelid (ptosis), absence of sweating of the face (anhidrosis), and sinking of the eyeball into the bony cavity that protects the eye (enophthalmos). These are the four classic signs of the disorder. The congenital, and more rare, form of Horner syndrome is present at birth but the cause is not known. Most often, Horner syndrome is acquired as a result of some kind of interference with the sympathetic nerves serving the eyes. The underlying causes can vary enormously, from a snake or insect bite to a neck trauma made by a blunt instrument.

Horner syndrome is relatively uncommon among the pediatric population. As in adults, Horner syndrome is caused by a lesion affecting the sympathetic innervation of the lids and pupil. Clinical signs may include relative unilateral miosis, ptosis, anhydrosis and heterochromia. In children, these signs may be more difficult to appreciate due to poor patient cooperation or frequent periods of sleep. The clinical evaluation and management of the pediatric patient with Horner syndrome varies among clinicians. However, most would agree on the importance of differentiating acquired from congenital Homer syndrome in order to guide the diagnostic work up. Etiologies of pediatric Horner syndrome have not been as well described as in adults. The following case report highlights some of the typical dilemmas in the evaluation of these cases as well as an unusual complication. © 2000 The Board of Regents of the University of Wisconsin System

Horner syndrome may be present in Klumpke palsy. Horner syndrome occur in neonates with Klumpke palsy when the anterior root, spinal nerve, and proximal region of the ventral ramus are affected. This is so because these structures carry the sympathetic fibers that innervate the constrictor muscle of the iris and the muscle of Müller. Horner syndrome does not occurs with Kumpke palsy, which occur as the result of an injury beyond the ventral ramus because the sympathetic fibers leave the brachial plexus at this level (Figure 221.1). Figure 221.1. Horner syndrome manifests by ipsilateral ptosis and miosis. The eyelid asymmetry is not present when the patient cries or is asleep (Figure 221.2). The lack of pigmentation in the affected eye leads to a different color of the iris. This color difference is not noted in the immediate neonatal period nor is it noticeable in all patients. The color difference is noted if and when the iris of the eye not affected by the Horner syndrome becomes darker as the result of normal pigmentation. In the absence of Horner syndrome or the classical Klumpke posture of the arm, weakness restricted to the hand raises the possibility of a cortical lesion in the region of the hand. An MRI of the brain may be necessary to eliminate this possibility.

Eye nerves affected
There are many nerves that go to the eye. These nerves are involved in the movements of the eye, eyelid, and the widening (dilation) and narrowing (constriction) of the pupil. If some part of the nerve supply is interrupted, Horner syndrome results. Conditions that may cause Horner syndrome include something pressing on the nerves (such as a tumor or bulging artery), injury to the nerves, or disease of the nerves. In fact, a tumor at the top of one of the lungs is a common cause. Horner syndrome affects both males and females of all ethnic backgrounds. It may occur at any age. Symptoms
The symptoms of Horner syndrome depend on what is the cause of the syndrome, but in most individuals the symptoms are present only on one side of the face. There are three main symptoms that most individuals experience
  • pupil constriction miosis drooping eyelid ptosis inability to sweat on the affected side ( anhidrosis
The individual may not be able to completely open the affected eye, and may have eye pain or severe headache. The face may flush on the affected side, especially with exercise.

    Horner syndrome during lumbar epidural analgesia for obstetrics
    SM Schachner and AC Reynolds Horner syndrome (ptosis, miosis, anhidrosis, and facial and conjunctival vasodilation) is a recognized complication of lumbar epidural analgesia for labor and delivery. Alone, it presents no significant risk to mother or fetus, as resolution is spontaneous and complete. Horner syndrome may, however, be associated with significant maternal hypotension and therefore should be an indication for close maternal and fetal monitoring to provide reassurance.

    HORNER'S SYNDROME SIGNS AND SYMPTOMS
    Horner's syndrome is characterized by an interruption of the oculosympathetic nerve pathway somewhere between its origin in the hypothalamus and the eye. The classic clinical findings associated with Horner's syndrome are ptosis, pupillary miosis and facial anhidrosis. Other findings may include apparent enophthalmos, increased amplitude of accommodation, heterochromia of the irides (if it occurs before age two), paradoxical contralateral eyelid retraction, transient decrease in intraocular pressure and changes in tear viscosity. Horner's syndrome has no predilection for age, race, gender or geographic location. Horner's syndrome of congenital origin is typically around the age of two years with heterochromia and absence of a horizontal eyelid fold or crease in the ptotic eye. Iris pigmentation (which is under sympathetic control during development) is completed by the age of two, making heterochromia an uncommon finding in Horner's syndromes acquired later in life. Old photographs can aid the clinician in distinguishing congenital Horner's by documenting heterochromia present at, or near, birth.
    PATHOPHYSIOLOGY
    The common etiologies of acquired preganglionic Horner's syndrome include, but are not limited to, trauma, aortic dissection, carotid dissection, tuberculosis and Pancoast tumor. Common causes of post-ganglionic Horner's syndrome include trauma, cluster migraine headache and neck or thyroid surgery.
