Horner Syndrome

There is unilateral narrowing (partial ptosis) of the palpebral fissure, contraction of the pupil, enophthalmos and reduction of sweating on the ipsilateral side of the face and neck, transitory rise in facial temperature, lacrimation increased or decreased; facial hemiatrophy, and occasionally the development of cataracts; hemifacial anhidrosis (lesion below bifurcation of common carotic artery); depigmentation of iris (when syndrome occurs in children). Due to a lesion of the cervical sympathetic nerve trunk. Inheritance of congenital, primary form is autosomal dominant.

Associated persons: Johan Georg Ræder Description: A neurological disease resembling the Horner syndrome, due to disruption of the sympathetic pathways by a neoplasm of the middle cranial fossa or a primary tumor of the gasserian ganglion. It is characterized by severe unilateral facial pain and headache in the distribution of the ophthalmic division of the trigeminal nerve, in combination with ipsilateral oculosympathetic palsy or Horner syndrome. Reported almost exclusively in males, commonly in their fifth decade of life, Absence of facial sweating differentiates this syndrome from Horner’s. The first patient described by Raeder in 1918 had an incomplete Horner syndrome with preserved sweating on the side of the lesion and severe facial neuralgia. In 1924, Raeder reported this case and four others.

Horner syndrome is a relatively rare disorder characterized by a constricted pupil (miosis), drooping of the upper eyelid (ptosis), absence of sweating of the face (anhidrosis), and sinking of the eyeball into the bony cavity that protects the eye (enophthalmos). These are the four classic signs of the disorder.
The congenital, and more rare, form of Horner syndrome is present at birth but the cause is not known. Most often, Horner syndrome is acquired as a result of some kind of interference with the sympathetic nerves serving the eyes. The underlying causes can vary enormously, from a snake or insect bite to a neck trauma made by a blunt instrument.
