Hidradenitis suppurativa is a chronic, pus-producing (suppurative), scarring (cicatricial) disease process that occurs due to obstruction of hair follicles and secondary infection and inflammation of certain sweat glands (apocrine glands), particularly those under the arms (axillae) or within the anal/genital (anogenital) region. The disease is characterized by the development of recurrent, boil-like nodular lesions and deep pus-containing pockets of infection (abscesses) that may eventually rupture through the skin. Healing of affected areas is typically associated with progressive scarring (fibrosis). The specific underlying cause of hidradenitis suppurativa is unknown.

HIDRADENITIS SUPPURATIVA (ACNE INVERSA, ACNE ECTOPICA) WAT BETEKENT HIDRADENITIS SUPPURATIVA ? Hidradenitis suppurativa, ook wel acne inversa of acne ectopica genoemd, is een chronische ontsteking van de haarzakjes en talgklieren in vooral de liezen en/of de oksels. Hidradenitis suppurativa betekent etterende zweetklierontsteking, acne inversa betekent acne van de plooien, acne ectopica betekent acne op andere plaatsen dan de gewone acne. Ondanks de naam zijn het niet de zweetklieren maar vooral de talgklieren die ontstoken raken. De talgklieren zijn aangesloten op de haarzakjes en produceren vet dat de huid soepel houdt. Als de gezamenlijke uitvoergang van de haarzakjes en de talgklieren geblokkeerd raakt (door een hoornprop of door zwelling in de omgeving), ontstaat ophoping van talg. De talgklieren zwellen op en raken ontstoken. Diverse bacteriën kunnen zich gaan vermeerderen in de afgesloten ruimten. KLACHTEN Anders dan bij de gewone acne (acne vulgaris), ontstaan bij acne inversa uitgebreide ontstekingen in en rond de talgklieren, met pijnlijke zwellingen als gevolg. Soms zijn verwijde afvoergangen en puskopjes zichtbaar aan de buitenkant. Er kunnen grote en pijnlijke met pus gevulde abcessen ontstaan, die op een gegeven moment doorbreken naar buiten. In de diepte kunnen grote holtes (cysten) ontstaan. Soms zoekt de pus een uitweg via andere wegen dan de oorspronkelijke afvoergang en ontstaan gangen en tunnels (fistels) onder de huid, die soms centimeters verder weer naar de oppervlakte komen. Abcesholtes kunnen met elkaar in verbinding staan via gangen. Abcessen kunnen steeds opnieuw terugkomen op dezelfde plaats. In de ergste gevallen is acne inversa een zeer invaliderende ziekte, vanwege de pijn en de continue uitvloed van talg, vocht en pus uit de ontstoken klieren. Het kan aanleiding geven tot sociaal isolement en depressieve gevoelens.

HIDRADENITIS - The Brits make a big deal about the role of androgens in hidradenitis suppurativa [1-5]. I do not have any personal experience with this approach. I have a 40 y/o female I am treating with hidradenitis, mainly in the buttocks and groin area who responded incompletely to a four month course of isotretinoin at 1 mg/kg and tolerated it poorly because of severe headaches (no papilledema). IL steroids has helped, but we are looking for a remittive drug that would be a viable alternative to a repeat course of Accutane. She has chronic pain, and has developed substance dependency in the past related to this. Her continual demand for pain medication is pushing me to try and do something. Accutane-related headaches made refusing her pain meds even harder, and I am not looking forward to another round of that. The problem is too diffuse for surgical management and she is within 10-15% of ideal body weight. I believe she has had a hysterectomy, so etretinate is thinkable, but I am worried it will produce severe headaches as Accutane did. Anyone have any experience treating hidradenitis with the antiandrogens available in the U.S. [Casodex (becalutamide), Eulexin (flutamide), Lupron (leuprolide acetate)] which are mainly used to treat prostate cancer? What about older, less potent drugs with antiandrogen properties like Aldactone (spironolactone), Nizoral (ketoconazole)? What about cyclosporin (or anything else)? 1. Last week I mentioned a woman with acne and hidradenitits who did not respond adequately to Accutane but is doing very well on Biaxin and Naprosyn. I probably try that combination before CyA, unless she's a total mess, in shich case you might knowck it down with CyA then try to keep it down with Biaxin / Naprosyn. Haven't had much success with antiandrogens for hidradenitis. Here in Canada we have acetretin (Soriatane) - this is not nearly as lipid soluble as etretinate and Accutane, so it doesn't cross the blood-brain barrier to a great extent. Soriatane might be worth trying if headaches are a problem. I had a guy who became violent and depressed on etretinate for psoriasis, but did great on Soriatane, probably because Soriatane didn't get into his CNS. KC Smith MD FRCPC - For hidradenitis I use low dose accutane, 20-40 mg/day to avoid the more problematic sideeffects, and use it for more than 6months. Someone is doing a type of more superficial liposuction in these areas, akin to those being done for hyperhidrosis-I can't remember the reference. I have found Duricef and Ceftin to be excellent antibiotics for this problem as well-why they work I don't know. I am not big on manipulation of androgens systemically in women-or men for that matter-unless they are very intouch with their psyche and can recognize subtle changes. What is the putative mechanism claimed by the British? And have they popped a months worth of these drugs themselves? They may be the same ones who write for hydroxyzine 25mg TID-I'd like to see them take just one and try to function at work. It is hard to imagine that unopposed Estrogen will be healthy, mind or bodywise. Diane Thaler I don't buy their logic re: androgens and hidradinitis. The process is one of aberrant follicular keratinization/occlusion according to Gerd Plewig, and is thus not under androgenic influence. I wish that I had some magic to offer HS patients. I rely on weight loss, rotating antibiotics, IL kenalog, and surgery. Guy Webster - I have tried Cyclosporin in a patient with very severe citatrizing hidradenitis suppurativa (associated with elbow and shoulder contractures) with no response after a several month course. This patient was also resistent to retinoids. Rhett Drugge After reading recently that follicular occlusion is the etiology rather than apocrinitis, I have had great success in hidradenitis with the addition of Retin A gel .01% to previously unsuccessful benzoyl peroxide wash/oral Minocin regimens. IL steroids are a great adjunct for big cysts. I have been disappointed with Accutane in hidradenitis. (I have not tried spironolactone but I presume it would work.) Philip Hughes, M.D. - I second this comment. Topical Retin-A has helped two of my milder hidradenitis patients, both in the axillary and inguinal areas. These patients both had comedones clinically in these areas, in addition to cysts. Gene Sienkiewicz, M. D. - Since recalcitrant comedonal acne patients, non Retin A responders, all respond to Accutane, why do hidradenitis patients respond so well to Retin A and apparently not to Accutane (nb, mine do respond to Accutane, not the same experience as the others on the list it seems). This section is dedicated to alternative (not antibiotic) treatments suggested by discussions with other patients like me on the web and which, OF COURSE, none of us has used since the FDA has not approved them for the treatment of hidradenitis suppurativa.
Those of us who have had this disease for over twenty years are generally immune to most antibiotics (I think that I use Keflex and Cipro as nutrients) or have become sensitive to them (Tetracycline makes me break out in an itchy rash which, when scratched, causes huge black and blue marks which also itch). Some of us have been beaten up by this disease badly enough so that our immune systems for staph and/or strep are partially or totally compromised ( My body starts singing "Its a Wonderful Day in the Neighborhood" when it sees staph and invites it in to tunnel and waste tissue and gives it a map so that it can tunnel through sinus cavities in what parts of my tissue are not scarred by prior tunneling).
The number of massive plastic surgeries many of us have had (mine are well into double digits) serve not to completely rid us of lesions in that area but merely to remove the worst areas of scarring and tunneling to avoid the worst of the complications of long term HS.

TO THE EDITOR: Hidradenitis suppurativa is a chronic inflammatory disease of the apocrine glands that most commonly involves the perianal, perineal, axillary, and inframammary regions. (1) The etiology is poorly understood, with a genetic component, hormonal influence, and obesity being linked to the expression. (2) Characteristic findings include dilated keratin-filled pores, scarring and sinus tract formation, malodorous discharge, and inflammation with secondary infection. (3) Medical management consists of cleaning the areas, application of mild topical steroids with the concurrent use of systemic antibiotics, (4) topical antibiotics, (5) and intralesional steroids. Rarely, with chronic hidradenitis suppurativa, Marjolin's ulcer (a squamous cell carcinoma) develops. (6) We report two cases of hidradenitis suppurativa in which squamous cell carcinoma developed after a latent period of 20 years and 35 years, respectively, despite repeated local excisions.

HISTORY A 59 year old African-American man presented with a 30 year history of painful draining lesions in the axillary, inguinal, and gluteal areas. He developed about 2-3 new lesions each month. He only had temporary relief with monthly intralesional steroids and oral antibiotics. A course of isotretinoin provided relief only for the duration of treatment. He was offered surgery which he refused. EXAMINATION Examination of the axillae revealed multiple scars amidst draining pustules, carbuncles, and sinus lesions with surrounding erythema. The buttocks revealed multiple scars as well as many abscesses and inflamed nodules in various stages. COURSE AND THERAPY After explaining the risks and benefits of electron beam radiotherapy, the patient was referred to radiation oncology for further treatment. He underwent 2 separate courses, 2 months apart, of superficial electron beam radiation with a cumulative dose of 1000cGy during each session. What is your diagnosis?

Abstract: Hidradenitis suppurativa (HS) is a chronic inflammatory disease with recurrent abscesses. In most cases, it involves the axillar and anogenital regions. In industrialised countries, the prevalence is of 0.3% to 4%, and the disease is over-represented in young adult females. Various medical treatments have been used but they are seldom effective. Surgical treatment is recommended as soon as the condition is diagnosed, but wide excisions very well outside the clinical borders of activity, are mandatory. A simple local incision is of no value. According to Hurley's clinical classification, stage I consists of one or more abscesses with no sinus tract and cicatrisation and stage II consists of one or more widely separated recurrent abscesses, with a tract and cicatrisation. The severest cases (stage III) have multiple interconnected tracts and abscesses throughout an entire area.     Squamous cell carcinoma is a rare but serious complication of HS. To investigate this association and the risk of other malignancies, we performed a population-based retrospective cohort study on 2119 HS patients selected from a computerised database of hospital discharge diagnoses in Sweden during 1965-1997. We found a significantly increased risk of non- melanoma skin cancer in patients with HS. The risks of buccal cancer and primary liver cancer were also higher.

In 1864, the French surgeon Verneuil was the first to associate HS with the sweat glands The prevalence of HS is unknown. Jemec reported a point prevalence of 4.1% based on objective findings in a younger adult population. The disease develops mainly in men in the third and fourth decades of life, which coincides with postpubertal increases in androgen levels ]. Onset before puberty is rare. In women, HS sometimes persists into the climacteric; onset after menopause is rare.
