Overview
The disease manifests as clusters of chronic abscesses , often as large as baseballs, that are extremely painful to the touch and may persist for years with occasional to frequent periods of inflammation , culminating in drainage, often leaving open wounds that will not heal. Drainage provides some relief from severe, often debilitating, pressure pain. These flare-ups are often triggered by stress, hormonal changes, or humid heat. Persistent lesions may lead to the formation of sinus tracts, or tunnels connecting the abscesses under the skin. At this stage, complete healing is usually not possible, and progression of the disease in the area is inevitable. Occurrences of bacterial infections and cellulitis (deep tissue inflammation) are likely at these sites. HS

What is hidradenitis suppurativa?
Hidradenitis suppurativa (HS), also known as "Acne Inversa," is a chronic inflammatory skin condition that can occur after puberty. First described in 1839, HS is not contagious but can be painful. It usually develops in areas where there is skin-to-skin contact, such as armpits, the top of the inner thigh, the groin and under the breasts, etc. It can also develop around hair follicles where scent (apocrine) sweat glands or oil (sebaceous) ducts are found. Similar to acne, which is a disease of the oil ducts, the inflammation in HS may be associated with infection, which, in turn, may cause pus to form. There are three clinical stages of HS:
  • Stage 1: The formation of single or multiple areas of inflammation and pus (abscesses), without sinus tracts (tunnels under the skin) and scar formation (cicatrisation).

What is hidradenitis suppurativa? Hidradenitis suppurativa is a chronic and stubborn disease centred on inflammation of the large specialised sweat glands (apocrine glands) that are found mainly in the armpits and groins. These areas show a distinctive mixture of boil-like lumps, areas leaking pus, and scarring. What causes hidradenitis suppurativa It is still not clear why the apocrine sweat glands are affected like this in some people, but mechanical blockage plays a part. As the secretions cannot escape, the tubes containing them may swell up and burst, or become infected. Contributory factors include the following:
  • Hormones are involved in the control of apocrine sweat glands and certainly play a part in the disease. Hidradenitis suppurativa affects women more often than men.

General Discussion Hidradenitis suppurativa is a chronic, pus-producing (suppurative), scarring (cicatricial) disease process that occurs due to obstruction of hair follicles and secondary infection and inflammation of certain sweat glands (apocrine glands), particularly those under the arms (axillae) or within the anal/genital (anogenital) region. The disease is characterized by the development of recurrent, boil-like nodular lesions and deep pus-containing pockets of infection (abscesses) that may eventually rupture through the skin. Healing of affected areas is typically associated with progressive scarring (fibrosis). The specific underlying cause of hidradenitis suppurativa is unknown. DESCRIPTION: Acute, tender, cyst-like abscesses in apocrine gland bearing skin (axillae, anogenital area, pubes, areolae, also apocrine glands scattered around umbilicus, scalp, trunk and face). In chronic cases, fibrotic sinus tracts develop with intermittent drainage and periodic acute abscesses.

  • Traditionally considered a disorder of apocrine glands but now felt to be due to occlusion of terminal follicular epithelium within apocrine gland-bearing skin. Bacteria involvement is not a primary pathogenic event, but secondary.
  • Historically part of "follicular occlusive triad"; acne conglobata, dissecting cellulitis of scalp, hidradenitis suppurativa. Pilonidal sinus later added to form a tetrad.
  • Smoking may be a major triggering factor

In addition to providing HS-specific research, the site will also include a wide variety of other related information to encourage a collaborative, multi-disciplinary approach for future HS research. Long term and large scale funding will be essential in discovering the cause and potential cures of HS. There is a clear and obvious need to develop a standard definition and globally accepted diagnostic procedures for HS, which in turn should promote the development of more effective and less invasive evidence-based treatments. Further research will eliminate decades of perpetual confusion, decrease the morbidity of HS and its economic burden to society, and make great strides into improving the quality of life and quality of care for those suffering from this potentially debilitating disease. A Brief Description of Hidradenitis Suppurativa
Hidradenitis can get worse over a period of time; many of our members have multiple lesions on different parts of the body at the same time. On the other hand, there are members who flare only occasionally and/or don't progress. If HS does progress, fistulas and sinus tracking (under the skin) may form, causing even more problems. The pain and inflammation can be disabling. Experiences with HS are varied - some people go through experimental treatments (radiation), radical surgery (requiring skin grafts), the gamut of medications (long-term antibiotics, accutane), and/or frequent lancings. The sores are not typically infected; in studies, more than 50% of lesions cultured do not show any bacteria. But, it is important to note that cellulitis can take over areas, and this requires immediate medical attention.
As you can imagine, many people who have HS deal with depression and physical pain on a daily basis. There are different theories about what causes HS - there appears to be a strong genetic link, and the cycles of the disease appear to indicate hormonal imbalance(s). HS is often seen in conjunction with thyroid disease, polycystic ovarian syndrome, diabetes, crohn's disease, and several autoimmune disorders. We are always looking for reasons WHY and HOW the disease affects us - some people with HS have literally dedicated their lives doing research and outreach about HS, hoping to find answers.
