    Overview Hiatal hernias are common, occurring in about one-quarter of people older than 50. They're especially likely to occur in women and in people who are overweight. Most hiatal hernias cause no signs or symptoms. You may never know you have one unless it's discovered during a test for another problem. Although small hernias aren't painful, larger ones may allow food and acid to back up into your esophagus, which can cause heartburn and chest pain. Self-care measures or medications can usually help ease these symptoms, although very large hiatal hernias sometimes need surgical repair. Signs and symptoms In rare cases, the part of your stomach that protrudes into your chest cavity may become twisted. This can lead to bloating, difficulty swallowing or even obstruction of your esophagus. Just as rarely, blood flow to your stomach may become restricted, causing severe chest pain and difficulty swallowing.

STOMA ('Parastomal') HERNIAS A Hernia is a weakness or split in the muscle wall of the abdomen which allows the abdominal contents (usually some part of the intestine) to bulge out. The bulge is particularly noticeable upon tensing the abdominal wall muscles - such as occurs when coughing, sneezing, straining or simply standing. Stomas pose an additional problem. When a stoma is brought out to the surface of the abdomen it must pass through the muscles of the abdominal wall, thus a potential site of weakness is immediately created. In the ideal situation the abdominal wall muscles form a snug fit around the stoma opening. However, sometimes the muscles come away from the edges of the stoma thus creating a hernia - in this case, an area of the abdominal wall adjacent to the stoma where there is no muscle. Factors that can contribute to causing a stoma hernia to occur include coughing, being overweight or having developed an infection in the wound at the time the stoma was made. The development of a stoma hernia is often a gradual phenomenon, with the area next to the stoma stretching and becoming weaker with the passage of time. This weakness, or gap, means that every time one strains, coughs, sneezes or stands up, the area of the abdomen next to the stoma bulges, or the whole stoma itself protrudes as it is pushed forwards by the rest of the abdominal contents behind it. As with all hernias the size will increase as time goes by. Stoma hernias are rarely painful, but are usually uncomfortable and can become extremely inconvenient.

Diagnosis/Definition
  • An abnormal protrusion (swelling) of a body part through a natural or surgically created area of weakness in a muscle or fascial wall. Common subtypes are: INGUINAL (groin): swelling in the upper scrotum or prepubic area. UMBILICAL: hernia through the umbilical ring. INCISIONAL: swelling and weakness in the area of a prior surgical incision. EPIGASTRIC: swelling in the midline between the umbilicus and the xyphoid. VENTRAL: swelling anywhere in the anterior abdominal area (overlaps with umbilical and epigastric hernias, but also includes less common abdominal wall hernias, such as SPIGELIAN).
Initial Diagnosis and Management
  • Diagnosis is made on basis of history of swelling of area in question, usually confirmed by the presence of a physically evident protrusion, which normally increases with straining and becomes less prominent when the patient is recumbent and relaxed.

An inguinal hernia is when a loop of intestine enters the inguinal canal, a tubular passage through the lower layers of the abdominal wall.

Causes, incidence, and risk factors: Usually, there is no apparent cause of a hernia , although they are sometimes associated with heavy lifting. A hernia occurs when part of an organ (usually the intestine) protrudes through a weak point or tear in the thin muscular wall that holds the abdominal organs in place. In men, a hernia can develop in the groin near the scrotum . A direct inguinal hernia creates a bulge in the groin area, and an indirect hernia descends into the scrotum. Inguinal hernias occur less often in women than men. Inguinal hernias are also seen in infants and children. Hernias can occur when a portion of the peritoneum (the lining around all the organs in the abdomen) does not close properly before birth. This causes a small portion of the intestine to push out into the opening (a bulge might be seen in the groin or scrotum). According to the American Academy of Pediatrics, 5 out of 100 children have inguinal hernias (more boys than girls). Some may not have symptoms until adulthood.

What is an inguinal hernia?
An inguinal hernia occurs when the bowel slides through an open canal into a pouch in the groin. This appears as a lump. Sometimes the lump goes down into the scrotum where the testis sits in a boy or into the labia in a girl. Inguinal hernias are common in childhood, especially in boys. They appear as a lump in the baby's groin. Inguinal hernias are not caused by muscle weakness, as in adults. They are not caused by letting the baby cry, although they are more noticeable when your baby is upset. These hernias do not go away on their own. Very young children need an operation as soon as a hernia appears because if the bowel gets caught in the hernia, it can be damaged.
The operation
Your child comes to hospital on the day of the operation and is usually home the same day. During the operation, the doctors stitch the open canal inside and close the opening the groin. This is done under general anaesthetic. The stitches are beneath the skin and dissolve by themselves. There will be a scar in the groin skin crease. As the child grows, the scar will fade but never completely disappear.

What is an inguinal hernia?
An inguinal hernia is a bulge of soft tissue through a weak spot in the abdominal wall in the groin area Inguinal hernias can occur in one or both sides of the groin at the same time or at different times. The bulge usually contains tissue lining the inside of the abdomen (peritoneum) and may include fatty tissue from inside the abdomen, a loop of intestine, or fluid (ascites). There are two types of inguinal hernias: direct and indirect.
  • A direct inguinal hernia occurs when a weak spot develops in the lower abdominal muscles from the normal stresses of living and aging. Tissues push through this weak spot as pressure within the abdomen increases. An indirect inguinal hernia is the most common type of inguinal hernia. Normally, an opening in the inguinal canal closes shortly before birth. An indirect hernia develops when this opening does not close, causing abdominal tissues to push through the inguinal canal. Symptoms may be present at birth or may develop later in life. In men, the hernia may push into the

Questions and Answers about Hernias What is a hernia? A hernia is a weakness or defect in the abdominal wall. It may be present from birth, or develop over a period of time. If the defect is large enough, abdominal contents such as the bowels, may protrude through the defect causing a lump or bulge felt by the patient. Hernias develop at certain sites which have a natural tendency to be weak; the groin, umbilicus (belly button), and previous surgical incisions. Why should hernias be repaired? Once a hernia has developed, it will tend to enlarge and cause discomfort. If a loop of bowel gets caught in the hernia, it may become obstructed or its blood supply may be cut off. This could then become a life-threatening situation. Since hernias can be repaired effectively and with minimal risk, most surgeons therefore recommend that hernias be repaired when diagnosed, unless there is serious medical problem which makes it too risky. On December 22, 1998, I went to the University of Washington medical center in Seattle for a hernia operation. This allowed me to avoid the stress of the pre-Christmas shopping rush by giving me a perfect excuse to do nothing more than lolligag around on the couch, read some P.G. Wodehouse, watch some teevee, and generally veg out. Having taken a camera along for some dental work at Greenlake Family Dentistry with excellent results, I decided to try it again and see what I could get away with. It takes a confident sawbones to let a doped-up geek with a web server and a digital camera into the operating room in this day and age, and with a little cajoling from Dr. Andrew Smith, Dr. Mika Sinanan went for it! Woohoo! It was all pretty interesting to me, and I walked out of there (with a little help from Shirley Nelson, a post-op nurse) about 2 hours after Dr. Visha Kapoor had sewn me up. The operation was performed under local anesthetic, but the anesthesiologist had me nodding off through most of it, probably because they were getting tired of all my inane questions. Dr. Smith also took most of the pictures seen here (thanks Andrew - you ROCK!) and Karen and Tracy, the scrub and circulation nurses, took a few as well. I had such a rollicking good time that I can't wait to get another hernia, and now I jump with squeals of anticipation whenever an opportunity to do some really heavy lifting presents itself. These pics are in chronological order. I'm medically illiterate and have no idea what's going on in them, but I won't let that stop me from captioning every one with a detailed explanation. I put this page together 10 days after the surgery, and after 6 weeks was back to deadlifting old water heaters.

A hernia (rupture) usually appears as a lump, commonly in the groin or in the umbilical region. It is developed when a portion of the tissue lining the abdominal cavity (peritoneum) breaks through a weakened area of the abdominal wall. This can give rise to discomfort as the hernia enlarges and can sometimes be dangerous, if a piece of intestine becomes trapped ('strangulated') inside.
There are various kinds of hernia affecting the various parts of the body and they are named according to the position of its development.
Hiatus hernia is a condition when the upper part of the stomach, which is joined to the esophagus (gullet), moves up into the chest through the hole (called a hiatus) in the diaphragm. This can result in painful burning sensation unlike the other hernias which often appear without symptoms. It is common and occurs in about 10 per cent of people.
