It is the commonest male operation performed in the UK, with the NHS performing over 100,000 hernia repairs every year. What is a hernia? A hernia is caused by a tear to the stomach muscles. In some cases part of the intestine or stomach will bulge through the hole and form a painful lump under the skin. The commonest type of hernia is found in the groin. It is known as an inguinal hernia because the weak spot usually occurs along the inguinal canal, an area where the tube that carries sperm enters the scrotum. Men are more likely to have an inherent weakness along the inguinal canal than women because of the way they develop in the womb. Are there other types of hernia? Yes. Insidious onset of unilateral groin pain is the most common symptom. Concurrent pathologies, such as osteitis pubis and adductor tenoperiostitis, may complicate diagnosis. Plain radiographs and a bone scan can aid differential diagnosis, but herniography is not recommended. Surgery is the preferred treatment. Structured rehabilitation should enable athletes to return to sports activity 6 to 8 weeks after surgery. I njuries to the groin may constitute 5% of all sports injuries (1) in male and female athletes and are particularly common among male and female soccer players, 28% of whom have a history of groin pain in some series (2). Particularly in male athletes, causes of groin pain without a clinically detectable hernia are increasingly ascribed to disruptions to the inguinal canal, especially in cases resistant to conservative treatment. These disruptions are often termed "sports hernia," "sportsman's hernia," or "athletic pubalgia." Little has been published on the prevalence of this condition among athletes, but Lovell (3) found sports hernias during surgery in 50% of 186 male athletesthe study included 186 males and 3 femaleswho had had groin pain lasting longer than 8 weeks. The condition has occurred in athletes in a wide range of sports but appears to be particularly common in activities requiring repetitive twisting and turning movements at speed. These include soccer, ice hockey, field hockey, tennis, and Australian rules football. It has also been infrequently reported in women (4,5), who describe tenderness at the site of the superficial inguinal ring. This infrequency may be a result of either the limited female participation in the sports previously studied, such as soccer, ice hockey, and Australian rules football, or the gender differences in the anatomy of the inguinal canal. An inguinal hernia occurs in the groin (the area between the abdomen and thigh). It is called "inguinal" because the intestines push through a weak spot in the inguinal canal, which is a triangle-shaped opening between layers of abdominal muscle near the groin. Obesity, pregnancy, heavy lifting, and straining to pass stool can cause the intestine to push against the inguinal canal. Symptoms of inguinal hernia may include a lump in the groin near the thigh; pain in the groin; and, in severe cases, partial or complete blockage of the intestine. The doctor diagnoses hernia by doing a physical exam and by taking x rays and blood tests to check for blockage in the intestine. The main treatment for inguinal hernia is surgery to repair the opening in the muscle wall. This surgery is called herniorrhaphy. Sometimes the weak area is reinforced with steel mesh or wire. This operation is called hernioplasty. If the protruding intestine becomes twisted or traps stool, part of the intestine might need to be removed. This surgery is called bowel resection.
